Overview of fetal arrhythmias.

PURPOSE OF REVIEW: Though fetal arrhythmias account for a small proportion of referrals to a fetal cardiologist, they may be associated with significant morbidity and mortality. The present review outlines the current literature with regard to the diagnosis and, in brief, some management strategies in fetal arrhythmias. RECENT FINDINGS: Advances in echocardiography have resulted in significant improvements in our ability to elucidate the mechanism of arrhythmia at the bedside. At the same time, magnetocardiography is broadening our understanding of mechanisms of arrhythmia especially as it pe

Magnetocardiographic identification of prolonged fetal corrected QT interval in women receiving treatment for opioid use disorder.

AIM: Pregnant women undergoing treatment for opioid use disorder (OUD) may be exposed to multiple QT prolonging agents. We used magnetocardiography to measure fetal QT intervals in mothers with OUD on buprenorphine therapy. METHODS: Fetal and maternal magnetocardiography was performed in pregnant women receiving buprenorphine-assisted treatment (Disorder group); these were matched by gestational age to pregnant women who were opiate naïve (Reference group). Corrected QT intervals were determined using Bazett's formula and compared between groups. RESULTS: A total of eight women in the Disord

Human cardiac measurements with diamond magnetometers.

The heart produces small magnetic signals, but detecting them without touching the body and without heavy shielding equipment remains a major challenge. In this study, we demonstrate a noninvasive method to measure these signals using compact quantum sensors made from diamond. These sensors operate at room temperature and can detect extremely weak magnetic fields produced by the heart, even outside highly shielded environments. By averaging signals over many heartbeats and using noise suppression techniques, we successfully recorded clear magnetocardiography traces. Although current measuremen

Magnetic field based investigation of Brugada syndrome.

Brugada syndrome (BrS) is an inherited cardiac disorder associated with and increased risk of sudden cardiac death. The syndrome is associated with a complex current distribution and repolarization dispersion. The traditional methods for the diagnosis rely on standard electrocardiographic (ECG) assessment of the cardiac dynamics. Recent advancements in magnetocardiography suggest that the magnetic-field based investigation of cardiac electrophysiology could offer novel perspectives for the investigation of current abnormalities in BrS patients which go beyond the conventional ECG capabilities

Fanconi anemia and breast cancer: what's the connection?

We report a fetus at 33 weeks of gestation with supraventricular tachycardia, which was successfully managed by transplacental administration of an antiarrhythmic agent. Fetal magnetocardiography (fMCG) revealed supraventricular tachycardia of the long RP' tachycardia type. Transplacental administration of sotalol, instead of digoxin, was selected as the first-line drug, and it successfully converted supraventricular tachycardia to sinus rhythm. The diagnosis of the type of supraventricular tachycardia was confirmed by electrocardiography after birth. Sotalol was also effective after birth to

Complex and Novel Arrhythmias Precede Stillbirth in Fetuses With De Novo Long QT Syndrome.

BACKGROUND: Long QT syndrome (LQTS) is a leading cause of sudden cardiac death in early life and has been implicated in ≈10% of sudden infant deaths and unexplained stillbirths. The purpose of our study was to use fetal magnetocardiography to characterize the electrophysiology and rhythm phenotypes of fetuses with de novo and inherited LQTS variants and identify risk factors for sudden death before birth. METHODS: We reviewed the fetal magnetocardiography database from the University of Wisconsin Biomagnetism Laboratory for fetuses with confirmed LQTS. We assessed waveform intervals, heart ra

Can fetal heart rate variability obtained from cardiotocography provide the same diagnostic value like from electrophysiological interbeat intervals?

OBJECTIVE: Fetal heart rate variability (HRV) is widely used for monitoring fetal developmental disturbances. Only expensive fetal magnetocardiography (fMCG) allows the precise recording of the individual fetal heart beat intervals uncovering also highly frequent vagal modulation. In contrast, transabdominal fetal electrocardiography (fECG) suffers from noise overlaying the fetal cardiac signal. Cardiotocography (CTG) is the clinical method of choice, however, based on Doppler ultrasound, improper to resolve single beats concisely. The present work addresses the transferability of established

AI-enabled diagnosis and localization of myocardial ischemia and coronary artery stenosis from magnetocardiographic recordings.

Early diagnosis and localization of myocardial ischemia (MS) and coronary artery stenosis (CAS) play a crucial role in the effective prevention and management of ischemic heart disease (IHD). Magnetocardiography (MCG) has emerged as a promising approach for non-invasive, non-contact, and high-sensitivity assessment of cardiac dysfunction. This study presents a multi-center, AI-enabled diagnosis and localization of myocardial ischemia and coronary artery stenosis from MCG data. To this end, we collected a large-scale dataset consisting of 2,158 MCG recordings from eight clinical centers. We the

The Prenatal Diagnosis and Perinatal Management of Congenital Long QT Syndrome: A Comprehensive Literature Review and Recent Updates.

Congenital long QT syndrome (LQTS) is a group of heritable conditions that are associated with cardiac repolarization abnormalities characterized by QT prolongation on electrocardiogram and the risk of life-threatening arrhythmias. The prenatal detection of LQTS presents significant challenges for clinicians, and a multidisciplinary approach is required for optimal prenatal and postnatal management. In this comprehensive literature review, we describe strategies for the fetal diagnosis of LQTS with variable initial presentation, genetic testing in suspected fetal LQTS, the utility of fetal mag

Rapid atomic-signal-based in-situ magnetic compensation for unshielded spin-exchange relaxation-free atomic magnetometer.

We propose a novel method, which leverages the atomic signal itself, to achieve rapid in situ magnetic field compensation in an unknown geomagnetic environment for an unshielded spin-exchange relaxation-free atomic magnetometer. This method employs a strong bias magnetic field aligned with the pump beam direction to maximize the strength of the magnetic field resonance signal. The compensation process is divided into three sequential stages, each focusing on one of the x, y, and z axes by adjusting the magnitude of the bias magnetic field. As a result, the zero magnetic field is identified wit

Fetal arrhythmias: Surveillance and management.

Fetal arrhythmias warrant sophisticated surveillance and management, especially for the high-risk pregnancies. Clinically, fetal arrhythmias can be categorized into 3 types: premature contractions, tachyarrhythmias, and bradyarrhythmias. Fetal arrhythmias include electrocardiography, cardiotocography, echocardiography and magnetocardiography. Oxygen saturation monitoring can be an effective way of fetal surveillance for congenital complete AV block or SVT during labor. Genetic surveillance of fetal arrhythmias may facilitate the understanding of the mechanisms of the arrhythmias and provide th

BACKGROUND: Intrauterine growth restriction (IUGR) is a fetal condition characterized by growth-rate reduction. Afflicted fetuses tend to display abnormalities in heart rate. OBJECTIVE: To study the differences in the heart-rate variability of low-risk fetuses and IUGR fetuses during different behavioral states. METHODS: A total of 40 fetal magnetocardiograms were analyzed from 20 low-risk and 20 IUGR fetuses recorded using a 151-sensor SQUID-array system. The maternal cardiac signals were attenuated using signal-space projection. Fetal R waves were identified using an adaptive Hilbert trans

Noninvasive muscle activity imaging using magnetography.

A spectroscopic paradigm has been developed that allows the magnetic field emissions generated by the electrical activity in the human body to be imaged in real time. The growing significance of imaging modalities in biology is evident by the almost exponential increase of their use in research, from the molecular to the ecological level. The method of analysis described here allows totally noninvasive imaging of muscular activity (heart, somatic musculature). Such imaging can be obtained without additional methodological steps such as the use of contrast media.

[Cardiac arrhythmias in the pregnant woman and the fetus].

For the acute treatment of supraventricular (SVT) and ventricular tachycardias (VT) in pregnant women, electrical cardioversion with 50-100 J is indicated in all unstable patients. In stable SVT the initial therapy includes vagal maneuvers or intravenous adenosine. For long-term therapy, beta-blocking agents with beta(1)-selectivity are first-line drugs or specific anti-arrhythmic drugs. An implantable cardioverter-defibrillator is another approach. In patients with symptomatic bradycardia, a pacemaker can be implanted using echocardiography at any stage of pregnancy. Evaluation of fetal arrhy
