National estimates of enrollment in disease management programs in the United States: an analysis of the National Ambulatory Medical Care Survey data.

Disease management (DM) programs have demonstrated improvement in clinical and economic outcomes for patients with chronic conditions; however, the extent of utilization of these programs in the United States is not known. The rate of enrollment in DM programs was estimated using data from the National Ambulatory Medical Care Survey (NAMCS). Logistic regression analysis was used to identify predictors of enrollment in DM programs. The complex sampling design of the NAMCS was accounted in all analyses. The final study sample included 14,405 patient visits and indicated that only 21.3% of patien

The Effects of a Comprehensive Care Management Program on Readmission Rates After Acute Exacerbation of COPD at a Community-Based Academic Hospital.

Acute exacerbation of chronic obstructive pulmonary disease (AECOPD) is one of the leading causes of hospitalization in the United States. Prior investigations suggest clinical and physiological parameters are important determinants for AECOPD readmissions. Strategies aimed at addressing these factors have not resulted in a major reduction of readmissions. We compared patients readmitted after an index AECOPD admission with non-readmitted patients. Patients' age, gender, body mass index, comorbidities (obstructive sleep apnea, chronic hypercapnia, congestive heart failure, lung cancer, pulmona

Empowered management for pelvic pain: The experiences of women with persistent pelvic pain participating in an online self-directed self-management program while they wait for interprofessional care.

BACKGROUND: Persistent pelvic pain is pain sensed in or around the pelvis and is often associated with negative cognitive, behavioral, sexual, and emotional consequences. The lack of interprofessional persistent pelvic pain management programs that address the complex interplay of biopsychosocial factors result in lengthy wait times and negative health outcomes. Limited access to evidence informed self-management educational resources contributes to poor coping strategies. Evidence shows that self-management education and strategies support patients while they wait for care. However, very few
