Orthodontic Extraction of High-Risk Impacted Mandibular Third Molars in Close Proximity to the Mandibular Canal: A Systematic Review.

PURPOSE: Extraction of mandibular third molars (M3s) in close proximity to the mandibular canal has some inherent risks to adjacent structures, such as neurologic damage to teeth, bone defects distal to the mandibular second molar (M2), or pathologic fractures in association with enlarged dentigerous cysts. The procedure for extrusion and subsequent extraction of high-risk M3s is called orthodontic extraction. This is a systematic review of the available approaches for orthodontic extraction of impacted mandibular M3s in close proximity to the mandibular canal and their outcomes. MATERIALS AN

A Rare Case of Single-Rooted Mandibular Second Molar with Single Canal.

The root canal anatomy of mandibular second molar teeth is known to be highly variable. Whilst the most frequently seen configuration is two mesial canals and one distal canal, other variations such as four canals, two canals, and C-shaped canal system do also exist. This case report describes the diagnosis and management of unusual root canal configuration of a mandibular second molar, with one canal in a single conical root, using the contemporary advancements in endodontics. Following clinical and radiographic examinations of the case, a diagnosis of symptomatic irreversible pulpitis with s

In Vivo Strain Alterations in Mandibular Molars after Root Canal Treatment Procedures.

INTRODUCTION: The aim of this investigation was to determine the coronal strain variations in mandibular molars under an in vivo bite load after root canal treatment procedures. METHODS: The coronal strain in carious mandibular first molars with symptomatic irreversible pulpitis (experimental group) was compared with that of intact contralateral teeth (control group) in patients 20-40 years old. Experiments were conducted in 2 stages. In stage 1, the maximum bite force on the first molar region was determined on the experimental and control teeth using a customized load cell. In stage 2, stra

Radiographic assessment of the mandibular retromolar canal using cone-beam computed tomography.

OBJECTIVE: The 'retromolar' nerve is a collateral branch of the inferior alveolar nerve. Cone-beam computed tomography (CBCT) provides higher resolution images. This CBCT study reports the frequency of the retromolar nerve. MATERIALS AND METHODS: From 2007-2010 the CBCT study of 233 hemi-mandibles have been examined. The CBCT study was obtained from an investigation of the posterior mandibular region in 187 patients suffering from different pathologies and it was aimed at detecting in patients the presence of a retromolar canal and foramen. RESULTS: Thirty-four retromolar canals with a foram

Comparison of orthopantomography and computed tomography image for assessing the relationship between impacted mandibular third molar and mandibular canal.

BACKGROUND: Permanent mandibular third molar are most commonly impacted teeth. In planning the surgical removal of mandibular third molar, correct diagnosis requires not only their precise spatial location, but also a thorough and accurate assessment of the intimate relationship with adjacent anatomical structures. Various imaging modalities have been used for localizing the mandibular third molar but not satisfactorily. AIM: This prospective study of 30 patients with 42 impacted mandibular third molars was carried out with the aim of finding evidence for justifying the use of computed tomogr

Why Is Mandibular Third Molar Coronectomy Successful Without Concurrent Root Canal Treatment?

PURPOSE: Coronectomy is performed to avoid injury to the inferior alveolar nerve when mandibular third molars (M3s) in close proximity to the inferior alveolar nerve are indicated for extraction. Concern has been raised regarding the fate of submerged roots with exposed pulp tissue and whether this will serve as a nidus for infection. The study purpose was to answer the following clinical question: Among patients undergoing coronectomy, do those treated with concurrent root canal treatment (RCT), when compared with those not treated with RCT, have a decreased frequency of postoperative infecti

The bifid mandibular canal in three-dimensional radiography: morphologic and quantitative characteristics.

The mandibular canal is a prominent anatomical structure and it is of great clinical importance since it contains the inferior alveolar nerve. The clinician is advised to proceed cautiously in the vicinity of the mandibular canal to avoid any damage to its neurovascular content. Based on observations in dry mandibles, in panoramic radiographs, and recently in three-dimensional radiography, various anatomical variations of the mandibular canals have been described. One such variant is the so-called bifid mandibular canal (BMC). Embryologically, multiple canals develop and subsequently fuse to f

Bilateral Mandibular Condylar Fractures with Associated External Auditory Canal Fractures and Otorrhagia.

A rare case of bilateral mandibular condylar fractures associated with bilateral external auditory canal fractures and otorrhagia is reported. The more severe external auditory canal fracture was present on the side of high condylar fracture, and the less severe external auditory canal fracture was ipsilateral to the condylar neck fracture. A mechanism of injury is proposed to account for such findings.

Assessment of variations of the mandibular canal through cone beam computed tomography.

The neurovascular bundle may be vulnerable during surgical procedures involving the mandible, especially when anatomical variations are present. Increased demand of implant surgeries, wider availability of three-dimensional exams, and lack of clear definitions in the literature indicate that features of anatomical variations should be revisited. The objective of the study was to evaluate features of anatomical variations related to mandibular canal (MC), such as bifid canals, anterior loop of mental nerve, and corticalization of MC. Additionally, bone trabeculation at the submandibular gland f

Radiographic enlargement of mandibular canal as first feature of non-Hodgkin's lymphoma.

Non-Hodgkin's lymphoma has the propensity to affect non-lymphoid tissue including oral tissue. Primary non-Hodgkin's lymphoma of the mandible mistreated as chronic periodontitis with diffuse enlargement of the mandibular canal and ice-cold numbness is very rarely described in English medical literature. A 57-year-old patient presented with a painful swelling on the left side of the mandible with a clinically chronic periodontitis associated with ice-cold numbness. A panoramic radiograph showed a diffuse uniform enlargement of the mandibular canal. Histological examination showed that the lesio

Root canal configuration of deciduous mandibular first molars--an in vitro study.

Root canal morphology of deciduous mandibular first molar was studied using roentgenographic and decalcification and clearing methods. Two groups of 15 teeth were formed. In the roentgenographic group, roots of the teeth were separated from the crown at the level of CEJ. Cut roots of the teeth were radiographed from both mesiodistal and buccolingual directions using separate IOPA films. In the decalcification and clearing group, teeth were decalcified and made transparent. Dye was injected later. The roots of the teeth were examined under 10 times for number, curvature, types and additional fe

[Classification of lower impacted third molars position and their interrelationship with mandibular canal].

Classification of lower Impacted third molars position and their interrelationship with mandibular canal have a great scientific, theoretical and practical importance. An attempt to systematize and classify all anatomical impacted third molars position has been made by many authors. Based on our scientific literature search, we found: 8 impacted third molars classifications, 7 third molars and mandibular canal interposition classifications, as well as 5 classifications of mandibular canal anatomical branching variants. Most classifications are proposed by the author to predict third molar erup

Radiographic study of the mandibular retromolar canal: an anatomic structure with clinical importance.

INTRODUCTION: The retromolar canal is an anatomic structure of the mandible with clinical importance. This canal branches off from the mandibular canal behind the third molar and travels to the retromolar foramen in the retromolar fossa. The retromolar canal might conduct accessory innervation to the mandibular molars or contain an aberrant buccal nerve. METHODS: Patients referred for panoramic radiography were consecutively enrolled, provided a limited cone-beam computed tomography (CBCT) scan had also been taken in the area of interest. Radiographs were retrospectively screened for the pres

[Anatomy of the atrophic mandible. 1. The location of the mandibular canal in the atrophic mandible].

43 atrophic left hemimandibles were divided by 6 saw cuts made between the mental foramen and the third molar to analyze the location of the mandibular canal. Mandibles were classified by the severity of alveolar absorption using Atwood's classification and changes in the distances of the mandibular canal from the superior and inferior borders of the mandibular body were correlated wit the degree of atrophy. Measurements were obtained with a digitizer and a computer and results were analyzed statistically. The following statistically significant observations were made: The distance of the mand

Evaluation of Mental Foramen and Mandibular Canal Course and Location in an Emirati Subpopulation.

INTRODUCTION AND AIMS: Violations of the mandibular canal (MC) and mental foramen (MF) and subsequent injuries to their neurovascular bundle have been reported after surgical and nonsurgical dental procedures. Besides using advanced technologies such as cone-beam computed tomography (CBCT), clinicians should be aware of the anatomy and location of MC and MF in different populations. This study aims to describe the morphologic characteristics of the MF, MC, and its intrabony location in relation to the apices of mandibular posterior teeth in an Emirati subpopulation using CBCT. METHODS: A tota

Automated segmentation of the mandibular canal and its anterior loop by deep learning.

Accurate mandibular canal (MC) detection is crucial to avoid nerve injury during surgical procedures. Moreover, the anatomic complexity of the interforaminal region requires a precise delineation of anatomical variations such as the anterior loop (AL). Therefore, CBCT-based presurgical planning is recommended, even though anatomical variations and lack of MC cortication make canal delineation challenging. To overcome these limitations, artificial intelligence (AI) may aid presurgical MC delineation. In the present study, we aim to train and validate an AI-driven tool capable of performing accu
