Sensory complaints in the area of the mandible and mouth often escape notice or remain undiagnosed. Using electromyographic recording of the trigeminal reflexes and motor responses, we sought trigeminal dysfunction in 50 patients with peripheral neuropathy, and tried to gain pathophysiological information on the mechanisms provoking trigeminal damage. Trigeminal reflex recordings (early and late blink reflex after supraorbital stimulation, early and late masseter inhibitory reflex after mental stimulation, and jaw jerk) disclosed abnormalities caused by sensory trigeminal neuropathy in 8 out o

This paper documents the incidence of inferior alveolar nerve injury and resultant sensory disturbance encountered in the removal of 100 consecutive impacted mandibular third molars. Five cases of anesthesia and/or paresthesia resulted and all but one of these resolved within six months. The lack of direct correlation between surgical exposure of the neurovascular bundle intraoperatively and the proximity of radiographic images of tooth root and mandibular canal, with the occurrence of sensory deficit, is noted. Our findings support the 1979 Recommendations of NIHCDCRTM*, upon which the policy

Inferior alveolar nerve transposition is a useful adjunctive surgery in implant dentistry when there is insufficient bone between the ridge crest and the inferior dental canal. However, if this surgery is done carelessly, complications such as mandibular fracture and permanent lower lip numbness can occur. This article reports the first case of bilateral mandibular body fracture occurring during inferior alveolar nerve transposition. The surgical management of a complicated bilateral displaced mandibular body fracture is explained herein. A literature review of mandibular fracture after inferi

More than 70% of dentists have experienced patients with postoperative paresthesia/dysesthesia or anesthesia as a result of dental procedures. The most common nerve affected is the mandibular nerve, which may be injured during either implant surgery or bone grafting procedures. However, no organized protocol directed specifically for general dentists, or for the vast majority of specialists, has been published. The protocol presented in this article is divided into 5 periods: during surgery (1) nerve injury is suspected or (2) known nerve transection, post-operative period (3) 1 week and (4) 1

INTRODUCTION: It is mandatory to know the anatomic path of the lower alveolar nerve and its intramandibular connections for numerous odontology, stomatology, and maxillofacial surgeries. We present a computed tomography study of the lower alveolar nerve intramandibular path.
MATERIAL AND METHOD: The computed tomography of 14 mandibles was performed (vestibulolingual cross-section) and four landmarks were defined behind the mental foramen. At each landmark we measured the distance between the alveolar canal and the two cortical layers in the vestibulolingual plane, and the distance between the

The marginal mandibular branch of the facial nerve (MMB) is a surgically relevant extracranial branch of cranial nerve VII, vulnerable during submandibular, parotid, and cervicofacial procedures. Its relationship to the facial vessels and the inferior border of the mandible (IBM) is variable and has been studied for more than six decades. This review analyses the course of the MMB as it crosses the facial artery and vein, focusing on its vertical position relative to the IBM, together with branching patterns, fascial relationships, anastomoses, landmarks, injury, and recovery. The MMB usually

UNLABELLED: A technique is presented for severely impacted and dentigerous-cyst-related mandibular third molar removal, avoiding any associated risks.
CASE PRESENTATION: A healthy patient complained about unpleasant smell and taste in the mouth and unilateral extraoral palpation of a protuberance, at the right corner of the mandible. Restricted field CBCT showed a severely impacted third molar, which penetrated the inferior cortical bone of the mandibular corner and was associated with a dentigerous cyst that had resorbed considerable bone in the region. Surgical exposure of the impacted toot

Herpes zoster virus (HZV) infections are caused by reactivation of the varicella zoster virus. Reactivation symptoms commonly affect the thoracolumbar trunk, and rarely affect the mandibular branches of the trigeminal nerve. When the mandibular branches are involved, lesions appear proximal to the innervation area. This condition may be associated with exfoliation of the teeth and osteonecrosis of the jawbone. We report a case of mandibular osteomyelitis after herpes zoster infection and we present a review of the literature on mandibular-branch involvement of HZV-related osteonecrosis.

PURPOSE: To assess the long-term recovery of sensation in the lower lip after mandibular resection without reconstruction of the inferior alveolar nerve.
MATERIAL AND METHODS: Thirty patients who had mandibular resection carried out without reconstruction of the inferior alveolar nerve were examined after an interval ranging from 6 to 33 years.
RESULTS: Only 1 patient, seen 10 years after resection, was totally numb over the distribution of the inferior alveolar nerve. The other 29 patients had some return of sensation and many had a significant return, though it may take several years to re

During mandibular distraction osteogenesis (DO), the inferior alveolar nerve (IAN) is damaged during distractor activation, but spontaneously recovers during consolidation. Although many neurotrophic factors are known to play critical roles, there have been few studies on the mechanism of peripheral nerve recovery after DO. The aim of this study was to observe the expression pattern of p75NGFR (low-affinity receptor of NGF) and to detect autocrine growth activity in IANs following mandibular DO. Unilateral mandibular distractions (0.5mm each, twice per day for 10 days) were conducted on eight

PURPOSE: This article evaluates a new intraoperative recording technique for measuring the sensory nerve conduction velocity (SNCV) of the human inferior alveolar nerve (IAN) during orthognathic surgery of the mandible to assess the effects of intraoperative strain on function of the IAN.
MATERIALS AND METHODS: The new test was successfully applied in 10 patients during bilateral sagittal split osteotomy of the mandible (20 IANs). The recordings were made with active wire electrodes at foramen ovale and stimulation was done at the mental foramen with two monopolar needle electrodes. The senso

Inferior alveolar nerve (IAN) injury is a concern in mandible distraction osteogenesis (DO). We have previously demonstrated that repeated local injections of human nerve growth factor beta (NGF-beta) have significantly enhanced the histologic recovery of the IAN in a rabbit model of DO. This study was to further test the effect of a single injection of human NGF-beta delivered via a collagen/nanohydroxyapatite/kappa-carrageenan gel to the recovery of the IAN in DO. Rabbits underwent mandibular DO at a rate of 0.75 mm/12 h for 6 days. At the end of the distraction period, injections were perfo

Tensionless adaptation of nerve ends is a challenging task in the repair of damaged inferior alveolar nerve (IAN). A new technique is introduced with posterior ostectomy of a mandibular distal segment after sagittal splitting for nerve bundle traction to tensionless anastomosis of nerve ends. We were able to create tensionless anastomosis of an IAN defect without autogenous or alloplastic graft using this method. This method is suitable for neurorrhaphy after neuroma removal in cases of IAN damage during dental procedures.

In a 25-year-old woman with episodic periorbital-temporal pain who eventually developed a sixth nerve palsy, magnetic resonance imaging revealed a lesion predominantly in the Meckel cave that was found to be a capillary hemangioma arising from the mandibular division of the trigeminal nerve. Hemangiomas of the Meckel cave must be considered in cases of facial pain with a sixth nerve palsy. even if there are no clinical findings of trigeminal neuropathy.

Antiresorptive agent-related osteonecrosis of the jaw (ARONJ) causes severe pain that cannot be controlled well by common analgesic drugs. This is a first case report of intractable mandibular pain due to ARONJ that was eliminated by a continuous mandibular nerve block. A 72-year-old woman with osteoporosis had been administered bisphosphonate. One year after extraction of her tooth, she was diagnosed as having ARONJ. Jaw pain was so severe that she was unable to open her mouth and eat. We performed a continuous mandibular nerve block through an indwelling catheter with levobupivacaine for pai

Our aim was to explore the relation between the site of the mandibular canal and neurosensory impairment after extraction of impacted mandibular third molars. We organised a retrospective study of 537 extractions in 318 patients in which the affected tooth was intersected by the mandibular canal. This was verified by cone-beam computed tomography (CBCT), and we analysed the relation between the site of the canal and the likelihood of injury to the inferior alveolar nerve (IAN) after extraction of the third molar. The relation between the position of the root of the tooth and the mandibular can
