Protocol for Diagnostic Test Accuracy Study: Evaluation of Cone Beam Computed Tomography (CBCT) in Prediction of Inferior Alveolar Nerve Injury as Compared to Orthopantomography (OPG) Secondary to Surgical Removal of Impacted Mandibular Third Molars.

Introduction Impairment of the inferior alveolar/dental nerve (IAN) is a relatively uncommon complication after lower wisdom tooth removal. Studies report varying incidences of IAN injury, with dysesthesia being noted as particularly distressing and 0-0.9% cases extending for a long duration. Neurosensory disruptions can severely impact speech, chewing, swallowing, and social interactions, leading to chronic pain and a lower quality of life. It also poses a risk of inadvertent injuries during meals. Although orthopantomogram (OPG) is primarily used for diagnosis, but when the lower wisdom toot

[The role of panoramic radiography in assessing the risk of injury to the inferior alveolar nerve before the extraction of mandibular wisdom teeth. The effect of the extent of root curvature and inferior alveolar canal-root tip overlap on the risk assessment].

The aim of the present study was to determine on panoramic radiographic images the effect of the distance between the root curvatures and inferior alveolar canal (IAC) root tip overlap on the surgeon's risk assessment predicting inferior alveolar nerve (IAN) paresthesia after lower third molar removal. In this case-control study 41 cases with postoperative IAN paresthesia and 359 controls without any postoperative complications were involved. Demographic data, root curvatures and the extent of IAC-root tip overlap were registered. The cases of major curvature--larger than 90 degrees (P=0.015;

Refinements in cervical-to-marginal mandibular nerve transfers for late and chronic facial palsy cases: A pilot study.

Marginal mandibular nerve palsy is among the most common facial nerve iatrogenic injuries and has significant functional and aesthetic consequences. In a case series of four patients (n = 4) with iatrogenic and acquired marginal mandibular nerve injuries, we performed a cervical branch facial nerve (VIIc) fascicular transfer to the marginal mandibular nerve (VIIm) for late and chronic facial palsies. Pre-and post-op videos were then analysed to determine the return lower lip depression, using the Terzis lower lip score and smile index was used for objective assessment. Overall, 75% of the case

Useful Method for Intraoperative Monitoring of Facial Nerve in a Scarred Bed.

Facial nerve is the main cranial nerve for the innervation of facial expression muscles. Main trunk of facial nerve passes approximately 1 to 2 cm deep to tragal pointer. In some patients, where a patient has multiple operations, fibrosis due to previous operations may change the natural anatomy and direction of the branches of facial nerve. A 22-year-old male patient had 2 operations for mandibular reconstruction after gunshot wound. During the second operation, there was a possible injury to the marginal mandibular nerve and a nerve stimulator was used intraoperatively to monitor the nerve a

[Peripheral nerve injuries during carotid endarterectomy].

Our experience with patients undergoing carotid endoarteriectomy over a 10 year period has been retrospectively reviewed. Nerve injuries were detected by reviewing postoperative progress and clinical notes. One hundred thirty-four procedures were performed on 120 patients, to 15 of whom (9%) occurred major nerve injuries. These included seven vagal nerve injuries causing ipsilateral vocal cord paralysis and hoarseness, five injuries of the marginal mandibular nerve and three injuries of the hypoglossal nerve. None of the patients with nerve injury had a stroke as a result of carotid operation.

Vascular and Nerve Relations of the Marginal Mandibular Nerve of the Face: Anatomy and Clinical Relevance.

BACKGROUND: The marginal mandibular nerve of the facial nerve is frequently injured during corrective and cosmetic surgery. Recent or emerging techniques such as the injection of filler materials, botulinum toxin, allotransplantation of composite tissues of the face, placement of chin implants, and submental cryolipolysis require in-depth knowledge of this nerve. The studies to date are not in agreement regarding the number of branches of the marginal mandibular nerve and its relationship with the vasculature and other nerves. METHODS: This study involved 62 half-heads from cadavers. RESULTS

A split ostectomy of mandibular body and angle reduction.

Combined mandibular angle resection with angle-splitting ostectomy (ASO) is more effective than conventional simple or multistaged ostectomy. Removal of the outer cortex of the mandibular body by ASO lessens the protuberance of the masseter muscle. In this study, the anatomy of the mandibular canals in seven human cadavers was studied in detail, and a guideline for ASO and mandibular angle ostectomy was set up so as to avoid injury to the inferior alveolar nerve. The most vulnerable area of the inferior alveolar nerve is the line from the gonion (G) to the junction (O) of the intersecting vert

Immediate inferior alveolar nerve reconstruction: Improving warfighter quality of life following mandibulectomy or traumatic avulsion of the mandible.

BACKGROUND: Mandibular reconstruction has historically been challenging due to the complex, highly functional, and esthetic nature of the anatomy. The most common etiologies of these defects requiring resection include trauma, benign tumors, and malignant pathology. Mandibular defects have been treated with little consideration for neural reconstruction, leaving patient's orally incompetent with associated social stigma. Although recent advances in reconstructive techniques improve oral rehabilitation, immediate inferior alveolar nerve (IAN) reconstruction has not been widely adapted. OBJECTI

Lingual nerve vulnerability: risk analysis and case report.

A known neurologic complication of third molar surgery is lingual nerve injury. Certain risk factors can cause lingual nerve injury during the extraction of mandibular third molars. This article will review the existing literature regarding lingual nerve injuries and related risk factors, and it attempts to study lingual nerve vulnerability during extraction of lower third molars, particularly in cases of distoangular impaction. The case report describes exposure of the lingual nerve during the extraction of the lower third molar. Also presented are 4 specific risk factors of lingual nerve inj

Trigeminal ganglion neuronal activity and glial fibrillary acidic protein immunoreactivity after inferior alveolar nerve crush in the adult rat.

Nerve injury to the mandibular division of the trigeminal nerve has been shown to cause satellite cell reactions that extend beyond the mandibular division of the trigeminal ganglion into the maxillary and ophthalmic divisions. The goal of this study was to determine whether any physiological abnormalities correlated with this dispersal of satellite cell reaction. We investigated the electrophysiological and satellite cell glial fibrillary acidic protein immunoreactivity (GFAP-IR) changes that occur within the trigeminal ganglion 3, 10 and 59 days after a crush injury of the inferior alveolar

Marginal Mandibular Nerve Mapping Prior to Nonablative Radiofrequency Skin Tightening.

BACKGROUND: Aging skin and increased skin laxity is a prevalent concern of patients. Nonsurgical treatments, such as radiofrequency, are increasing in popularity due to decreased pain, downtime, and scarring. ThermiRF (Thermi, Irving, TX) is a subdermal radiofrequency treatment for tightening skin. When applying radiofrequency treatments to the neck, it is important to avoid ablating the marginal mandibular nerve and causing nerve trauma. OBJECTIVES: The purpose of this study was to locate and record the position of the marginal mandibular nerve in 72 patients undergoing subdermal radiofreque
