Fibula modeling techniques for mandible reconstruction carry a high risk of bone perfusion impairment and low predictability of osteotomy angles. To restore the parabolic shape of the mandible, the number of osteotomies should be as small as possible to preserve both periosteal and endosteal perfusion. We report our approach with sagittal split osteotomy (SSO) technique for mandibular angle reconstruction.Obwegeser-Dal Pont SSO of fibula flaps was performed on 10 patients who underwent mandibular angle reconstruction. Bone segments were tilted according to stereolithographic template and fixed

Conventional osteotomies and bone grafts are still indicated in the treatment of otomandibular dysplasia. Results are achieved rapidly with a minimum of discomfort for the patient. In the mild cases, a complete correction can be obtained in a single operation.

BACKGROUND AND AIM: Patients with facial asymmetries undergoing maxillo-mandibular surgery may have residual asymmetries in the gonial region, body and blade of the mandible that call for further surgery. Other patients may present asymmetries of the lower third of the face that are not associated with occlusal alterations, for which there are no indications for maximally osteotomy. In these cases the authors propose using osteotomy of the outer mandibular cortex to resolve the aesthetic defect.
METHODS: A retrospective study was made using a clinical and radiographical evaluation of 7 patien

A modification of the osteotomy of the body of the mandible for elongation and rotation of its anterior part is presented. It allows these movements without the necessity for bone grafting, with a reduced risk of damage to the alveolar nerve and increased stability.

A case of typical mandibular asymmetry, probably caused by the development of a primordial cyst in the right ramus of the mandible, is presented. The one stage operation performed, which enabled the enucleation of the cyst and the simultaneous correction of the asymmetry, is described and the case discussed.

This is believed to be the first reported case of carotid artery thrombosis after elective maxillary or mandibular osteotomy. The most likely cause was a sudden, sharp blow to the right carotid artery region either when the mandibular split was completed or when the maxillary tuberosity was separated. The blow was associated with neck hyperextension and some lateral flexion. In a patient with tight, underdeveloped soft tissue, the resultant intimal tear caused thrombosis of the artery. Although this event is associated with a 40% mortality and further 52% serious morbidity, our patient has reg

The aim of this study was to verify the effectiveness of attenuated total reflectance-fourier transform infrared (ATR-FTIR) spectroscopy in the characterization of bone repair in mandibular osteotomy using erbium, chromium-doped yttrium, scandium, gallium and garnet (Er,Cr:YSGG) laser and multilaminate drill on each side. Two mandible bone fragments were removed from 30 rabbits, and the process of bone repair was studied immediately, 3, 7, 15, 21, and 28 days after the surgery. The histological analysis allowed detecting differences in the early stages of tissue repair after bone cutting perfo

Mandibular distraction combining the sagittal split ramus osteotomy and an intraoral curved distractor is efficacious to lengthen the mandibular body and ramus. The sagittal split permits immediate rotation of the proximal segment and lengthening while still providing a large surface interface for regenerative bone. The buried curved distractor enables mandibular lengthening along the normal logarithmic grow spiral as shown by Moss and Rickets. In addition, it is well tolerated by the patient and can remain in place for a long duration to provide support. In summary, this combination of surgic

OBJECTIVE: The purpose of this study was to determine the incidence of temporary and permanent sensory disturbance of the inferior alveolar nerve (IAN) after bilateral sagittal split osteotomy (BSSO) of the mandible.
METHODS: 14 patients were selected for this study. Before BSSO and at 1 week, 1, 3, 6 and 12 months after BSSO, the sensibility of bilateral inferior alveolar nerves were examined using sharp-blunt testing, 2-point discrimination, electronic pain response test (ZGK-1 electrometer).
RESULTS: With conventional sharp-blunt and 2-point discrimination test, electronic pain response t

OBJECTIVE: Bilateral sagittal split osteotomy (BSSO) is widely used for mandibular advancements but is often associated with complications such as nerve disturbances, limited angle projection, and mandibular notching. This study describes a modification, the anterior oblique osteotomy (AOO), within the minimally invasive orthognathic surgery (MIOS) concept, addressing these challenges.
METHODS: A single-center retrospective study was conducted involving 30 patients who underwent bimaxillary surgery using AOO. Pre- and postoperative computed tomography scans were analyzed to measure the increa

The aim of this study was to investigate the stability of the proximal mandibular reference structures that have been proposed in the literature for superimposition. Forty proximal mandibular segments of 20 patients who underwent bilateral sagittal split osteotomy (BSSO) for advancement were reconstructed from a pair of pre- and postoperative (2 years) cone beam computed tomography scans, and spatially divided into the mandibular condyle, the coronoid process, and 20 mandibular ramus regions. To assess the stability of the anatomical regions, the volumetric and surface discrepancy between the

PURPOSE: The purpose of the study is to quantify anteroposterior and transverse facial soft tissue changes with respect to underlying skeletal movements after bilateral sagittal split osteotomy by using cone beam computed tomography (CBCT).
PATIENTS AND METHODS: The study was conducted on 6 patients who required bilateral sagittal split osteotomy for correction of mandibular retrognathism. The patients were scanned using CBCT 1 week preoperatively, and 6 months postoperatively. The differences between pre- and postoperative images were measured and the ratios of different hard to soft tissue

AIM: The purpose of this study was to examine morphological changes of the mandibular region using frontal cephalograms following bilateral sagittal split osteotomy (BSSO) for mandibular setback. Furthermore, we compared the stability between screw and plate fixation.
METHODS: Pre and postoperative frontal cephalograms were taken of 26 patients. Parameters, Angle "A" which is formed by constructed lines along the mandibular ramus, and mandibular width at the angle were calculated. The subjects were analyzed according to the types of bony fixation, whether by screws or plates.
RESULTS: Angle

Exposure of the distal internal carotid artery at the level of the second cervical vertebra required manoeuvers such as division of digastric muscle or mandibular subluxation. These increase the exposure but may not provide adequate access and are associated with significant cranial nerves or temporal mandibular joint complications. Vertical Ramus Osteotomy (VRO) provided access of the internal carotid artery (ICA) up to the base of the skull, with low incidence of cranial nerve injury temporo-mandibular joint (TMJ) pain and no preincision preparation. We report two cases in which vertical div

PURPOSE: The purpose of this study was to compare the changes in temporomandibular joint (TMJ) morphology and clinical symptoms after sagittal split ramus osteotomy (SSRO) and intraoral vertical ramus osteotomy (IVRO) with and without a Le Fort I osteotomy.
PATIENTS AND METHODS: Of 43 patients with a diagnosed jaw deformity, 20 underwent IVRO without internal fixation and 23 underwent SSRO with rigid internal fixation. Some operations were performed in combination with a Le Fort I osteotomy. The TMJ symptoms and joint morphology, including the disc position, were assessed preoperatively and p

Sagittal split osteotomy of the mandibular ramus is performed in close proximity to the inferior alveolar nerve and may result in postoperative neurosensory disturbances. Intraoperative strain on the nerve and other complications in 25 patients undergoing bilateral sagittal split osteotomy were recorded. Neurosensory testing was carried out before and after surgery. The patients reported sensory disturbances in 54% of sites 4 days postoperatively and 42% and 34% of sites at 9 weeks and 6 months, respectively. Objective assessments showed an incidence of 34% at 4 days, 20% at 9 weeks, and 8% at
