Prosthetic rehabilitation of resorbed mandible with two implant-supported overdentures using ball attachments.

The aim of this study was to describe a technique of fabrication of two implant-supported mandibular overdentures. There are several treatment modalities for completely edentulous patients including conventional complete dentures (CDs), implant-supported overdentures (IODs), and implant-supported fixed prosthesis. Patients with a severely resorbed mandible often experience problems with their conventional CDs. Treatment concepts involving IODs have proven to be as satisfactory as implant-supported fixed prosthesis with added advantage of ease in hygiene maintenance and less cost. This clinical

[Preferred sites of accumulation of microbial plaque as shown by the color of the exterior surface of complete dentures].

PURPOSE: The observation of the denture plaque for complete dentures caused many clinical works and researches, since 30 years. Most of the studies were interested, by the prosthetic under-surface. New data show that the polished (exterior) surface on the denture palate is colonized more than hard palate. The aim of our study is to locate the favorite zones of the accumulation of the denture plaque on the level of the suction polished faces of the acrylic resin. MATERIAL AND METHODS: We proceeded by immersion during 24 hours of 62 used full dentures belongs to 33 patients, with mucous membran

Five-year results with fixed complete-arch mandibular prostheses supported by 4 implants.

This study examined whether it is possible to restore an edentulous mandible with a complete-arch fixed prosthesis retained by only 4 implants without decreasing the survival rate. One hundred nineteen patients received complete-arch mandibular prostheses retained by 4 implants. Most patients were followed for 3 years or more. All patients followed a routine protocol, including annual check-ups and regular radiographic examinations. Twenty-one patients dropped out. Radiographic measurements used the threads of the implants as a basis for comparison. No indication was found that the number of s

Report of four familial cases with congenitally missing mandibular incisors.

Heredity is one of the possible factors associated with congenitally missing mandibular incisors. We report four cases--a mother and three daughters--in an attempt to examine this hypothesis. Different treatment modalities have been used to treat malocclusions stemming from this problem. The treatment philosophy of consolidating mandibular spaces and obtaining a stable occlusion, thereby preventing the removal of maxillary teeth or the use of a mandibular prosthesis, is presented.

Secondary reconstruction of the mandible with an aluminum oxide prosthesis.

A case is presented where secondary mandibular reconstruction with an aluminum oxide ceramic prosthesis was performed for a patient who had had an earlier hemimandibulectomy and primary bone graft for management of an ameloblastoma. Various factors that were considered in the construction of the prosthesis and problems encountered during operation are discussed. Although the prosthesis was found to be quite useful for the restoration of facial contour, the results of a long-term follow-up are needed to obtain a final evaluation of the reconstruction.

Fibular Graft-Effective Panorama for Prosthetic Rehabilitation of Resected Mandible.

Surgical management of extensive tumors in the mandibular region leads to massive disfigurement of the face. Also the prosthetic outcome of such patients rehabilitated with free soft tissue flaps is very poor. Reconstruction of extensive defects to overcome the disfigurement is a challenging procedure and can be achieved with free fibula flap. Free fibula graft provides sufficient length of bone for the reconstruction of the postsurgical defects. Excellent vascularity of fibula flap allows for easy uptake of the graft and osseointegration of the dental implants. The addition of a skin island a

One-stage microvascular mandible reconstruction and alloplastic TMJ prosthesis.

Severely deformed or absent temporomandibular joints (TMJ) benefit from total alloplastic joint replacement and large mandibular defects from revascularized free tissue transfer for reconstruction. However no cases of their combined one-stage placement with outcomes can be found in the literature. We present two cases with different indications and reconstruction. The first patient required mandibular body and ascending ramus reconstruction after previous sarcoma resection. This was with a condyle-bearing reconstruction plate which resulted in significant dysfunction, leaving the patient unabl

The Cat Mandible (II): Manipulation of the Jaw, with a New Prosthesis Proposal, to Avoid Iatrogenic Complications.

The cat mandible is relatively small, and its manipulation implies the use of fixing methods and different repair techniques according to its small size to keep its biomechanical functionality intact. Attempts to fix dislocations of the temporomandibular joint should be primarily performed by non-invasive techniques (repositioning the bones and immobilisation), although when this is not possible, a surgical method should be used. Regarding mandibular fractures, these are usually concurrent with other traumatic injuries that, if serious, should be treated first. A non-invasive approach should a

Three-dimensional Printing Technology for Deep Circumflex Iliac Artery Flap: From Recipient to Donor Sites.

The deep circumflex iliac artery (DCIA) flap is one of the most commonly used vascularized free flaps for jaw reconstruction; however, its clinical application is limited by donor site complications. We aimed to describe a new technique of using 3-dimensionally (3D) printed patient-specific devices for mandibular reconstruction with DCIA flap and simultaneous dental implants, and for donor site restoration after harvesting the DCIA flap. One patient with mandible ameloblastoma underwent mandibular reconstruction using a DCIA flap with the "jaw-in-a-day" approach. The 3D-printed patient-specifi

Stability analysis of stock alloplastic temporomandibular joint prostheses based on clinical cases of screw loosening.

Screw loosening is one of the common complications that affect stability after stock alloplastic temporomandibular joint (TMJ) replacement. Based on clinically observed cases of screw loosening, this study aimed to evaluate how prosthesis installation patterns influence screw loading and to provide recommendations for installation safety. A three-dimensional finite element model of unilateral total TMJ replacement was constructed to analyze Biomet standard TMJ prosthesis stability under the following situations: 1) fossa component installed at different angles (0°-20°) relative to the bone sur

Positioning appliance for an implant-retained prosthesis for a patient with surgically and radiation-induced microstomia: A clinical report.

When treating a patient with microstomia, prosthesis design can become challenging and often requires modifications. The patient's ability to insert and remove the appliance may also be a problem. This clinical report describes the fabrication of a positioning appliance for a patient with microstomia. The appliance used the maxillary teeth as a guide to orient the prosthesis appropriately and engage the mandibular LOCATOR abutments.

Implant-assisted removable prosthetic rehabilitation after distraction osteogenesis in a patient with ameloblastoma recurrence: A case report.

INTRODUCTION: A resected mandibular edentulous ridge resulting from an ameloblastoma and marginal mandibulectomy is a restorative challenge. To maintain oral hygiene, recurrent examinations, and for long-term maintenance, a removable dental prosthesis is preferred to an implant-supported fixed dental prosthesis. PATIENT CONCERNS: A 28-year-old Asian man was referred for evaluation of a radiolucent area on the right side of the mandible. The right mandibular area had increasingly enlarged over a period of ≥5 months. Marginal resection and inferior alveolar nerve repositioning of the mandible w

Correction of a mandibular asymmetry after fibula reconstruction using a custom-made polyetheretherketone (PEEK) onlay after implant supported occlusal rehabilitation.

This study describes an unusual case of mandibular asymmetry after fibula free flap reconstruction in a young man following major facial trauma that was corrected using a custom-made polyetheretherketone prosthesis. There is little information in the literature on the use of alloplasts to correct mandibular asymmetry as interest in 'aesthetic re-modelling' has traditionally focused on nasal, zygomatic and chin regions. This report demonstrates that this technique can be used successfully to address selected cases of mandibular asymmetry.

Mandibular Reconstruction Utilizing the Reamer-Irrigator-Aspirator to Obtain Nonvascularized Femur Grafts.

Between July 2021 and February 2024, 6 patients with biopsy-confirmed benign pathologic lesions had mandibular defects reconstructed using a reamer-irrigator-aspirator at the University of Cincinnati Medical Center. Patients' ages ranged from 34 to 73, 5 of which were males and one female. Primary end points were bony continuity of the mandible, recovery time, and ability of the graft to receive implants. The reamer-irrigator-aspirator obtained 40 to 100 cm3 of uncompressed bone from donor sites. All patients ambulated on the day of surgery or postoperative day one. No neurosensory disturbance
