Uniportal thoracoscopic anterior and lingular (S3+4+5) segmentectomy of the left upper lobe using intravenous indocyanine green.

Although an anterior and lingular (S3+4+5) segmentectomy of the left upper lobe is rarely performed, this type of segmentectomy is very useful to ensure a sufficient margin when the tumor is located at the border between the anterior segment (S3) and the superior lingular segment (S4). However, it might be technically difficult to perform this unusual segmentectomy via a uniportal approach because of the limited angulation and exposure of the peripheral vessels/bronchi. We present the successful results of a patient undergoing uniportal thoracoscopic S3+4+5 segmentectomy of the left upper lobe

Modified round block technique for peripherally located early cancer breast, a technique that fits for all quadrants.

Round block technique (RBT) is an oncoplastic technique used in periareolar lesions, particularly in breasts with moderate ptosis or hypertrophy. However, it has some drawbacks including the possibility of late-onset scar widening, change in areolar shape, and asymmetry of the breasts. Moreover, it is hard to be performed with tumors located in periphery of breast. Modified round block technique (MRBT) is a new technique described to overcome these problems. A circumferential periareolar incision was made around the areola followed by subcutaneous dissection to the entire breast. Wide local ex

Hamartoma of the breast: an underrecognized disease?

Hamartomas of the breast are benign tumors composed primarily of dense, fibrous tissue with variable amounts of fat and associated ducts. A 48-year-old woman had noted a lump in her right breast for a number of years. She came to our clinics because of the recent progressive enlargement of this lump. Physical examination revealed a large, mobile, round, painless mass in the lower inner quadrant of the right breast. There were no palpable axillary lymph nodes. Mammography showed a circumscribed, 4.5 x 2.5 cm mass with a radiolucent periphery and moderately radiopaque center. On the basis of the

Juvenile papillomatosis of the breast in a 9-year-old girl.

Juvenile papillomatosis (JP) is a rare benign, proliferative breast tumor in children. We observed a 9-year-old girl with a large soft-tissue mass at the lower outer quadrant of the left breast. Ultrasonography showed an ill-defined, inhomogeneous mass with numerous small, hypoechoic areas. The tumor was completely excised. Histopathology revealed JP of the breast. The case is presented and the literature is reviewed.

Oncological safety and short-term aesthetic outcome of extreme oncoplasty for breast cancer.

BACKGROUND: For individuals with breast cancer, oncoplastic breast surgery (OBS), is considered an innovative technique of breast conservative surgery (BCS), has emerged as the recommended substitute for mastectomy. BCS and mastectomy have been shown to have comparable locoregional control and overall survival in randomized trials. In order to ensure oncological safety and improved aesthetic results, Extreme Oncoplasty, a development in the field of oncoplasty, has given hope for patient to achieve ultimate conservation before mastectomy. PATIENTS AND METHODS: From October 2021 to May 2023, fi

Survival after breast-conserving surgery and radiotherapy versus mastectomy: propensity score analyses within a randomized anaesthesiology trial.

BACKGROUND: Large randomized studies show equivalent survival after breast-conserving surgery (BCS) + adjuvant radiotherapy (RT) and mastectomy. In contrast, more recent observational studies suggest BCS + RT to be superior, but it is questionable whether patients in these treatment arms are comparable. Here, overall survival (OS) and breast cancer-specific survival (BCSS) after BCS + RT and mastectomy are compared within a randomized trial comparing intravenous and inhalation anaesthesia during breast cancer surgery. METHODS: The patient cohort was recruited from the randomized CAN-study. Pa

Is Breast Conserving Surgery Oncologically Safe for Patients With Multiple Ipsilateral Breast Cancers? A Systematic Review and Meta-analysis.

BACKGROUND: The incidence of multiple ipsilateral breast cancers (MIBC), which includes multifocal and multicentric tumors, is increasing. Despite breast conserving surgery (BCS) being the mainstay of treatment in early breast cancer, it is considered controversial for MIBC due to oncological safety concerns. The aim was to compare oncological outcomes of BCS compared with mastectomy for patients with MIBC. METHODS: A systematic literature search of relevant databases for comparative studies was conducted. A meta-analysis using the Mantel-Haenszel method was performed to calculate risk ratios

A free-style approach to selection of locoregional perforator flaps for unilateral breast restoration.

BACKGROUND: Evolution in breast oncology has increased the use of breast-conserving therapy and expanded the role of locoregional autologous reconstruction. While bilateral reduction is common for patients with large, ptotic breasts, those with small to moderate symmetric breasts often prefer unilateral volume restoration. This study demonstrates a free-style approach to flap selection and design for partial and total breast reconstruction, emphasizing intraoperative decision-making and preservation of muscle and nerve function. METHODS: A retrospective review was performed on patients underg

Interpectoral and Pectoserratus Plane Block vs. Local Anesthetic Infiltration for Partial Mastectomy: A Prospective Randomized Trial.

BACKGROUND: Patients undergoing breast surgery are at risk of severe postoperative pain. Several opioid-sparing strategies exist to alleviate this condition. Regional anesthesia has long been a part of perioperative pain management for these patients. AIM: This randomized study examined the benefits of interpectoral and pectoserratus plane block (IPP/PSP), also known as pectoralis nerve plain block, compared with advanced local anesthetic infiltration. METHODS: We analyzed 57 patients undergoing partial mastectomy with sentinel node dissection. They received either an ultrasound-guided IPP/P

Adenoid cystic carcinoma of the breast with late recurrence and high-grade transformation.

Breast cancers have broad histological subtypes with varied molecular expression, which determines the management. Although ductal and lobular breast carcinomas constitute the significant 90% of these tumours, other subtypes constitute about 10% of breast cancers. Adenoid cystic carcinoma (AdCC) is a salivary gland-type tumour described in breast, constituting less than 1% of all breast carcinomas. These tumours have a favourable prognosis and are surgically managed by either lumpectomy or mastectomy. Solid-basaloid and AdCC with high-grade transformation are the sporadic subtypes of AdCC, in

Breast conservation surgery versus total mastectomy among women with localized breast cancer in Soweto, South Africa.

PURPOSE: Breast conserving surgery (BCS) has become the preferred surgical option for the management of patients with nonmetastatic breast cancer in high-income countries. However, little is known about the distribution and determinants of BCS in low-and middle-income countries, especially those with high HIV prevalence. METHODS: We compared demographic and clinical characteristics of female patients who received BCS and those who received total mastectomy (TM) for nonmetastatic invasive carcinoma of the breast in Soweto, South Africa, 2009-2011. We also developed a multivariable logistic reg

Completion mastectomy after breast conserving surgery.

BACKGROUND: Breast conserving surgery (BCS) is increasingly offered to suitable patients diagnosed with early stage breast cancer. Occasionally the pathological margins on specimens following BCS are positive. The objective of this study is to assess the proportion of patients within our unit who required completion mastectomy after BCS and to determine if predictive factors could be identified to assist the breast surgeon identifying those patients at risk of positive margins following BCS. METHODS: All patients diagnosed with breast cancer between 2001 and 2005 were reviewed. Patients under

[Histopathological study on the safety of conservative breast surgery in terms of the degree of axillary dissection for breast cancer].

The subjects were 15 patients with primary mammary cancer who underwent radical mastectomy. Axillary dissection (level I, II and III) with an incision 7 cm below the axilla was performed. After radical mastectomy, the presence/absence of remaining lymph nodes and fatty tissue was determined. All operations were performed by the same surgeon. The number of lymph nodes dissected by means of subaxillary incision ranged from 10 to 32 (mean 20.4). The proportion of remaining lymph nodes found after radical mastectomy was 0/142 (0%) for level I, 0/113 (0%) for level II and 3/54 (5.6%) for level III.

A Look at the Other Side: High-Risk Lesions and Occult Contralateral Malignancy in Symmetry Procedures for Patients Undergoing Oncoplastic Breast-Conserving Surgery.

BACKGROUND: The incidence of occult breast cancer among patients undergoing reduction mammoplasty or risk-reducing mastectomies ranges from 1% to approximately 10%, respectively. Identification of incidental cancer often mandates subsequent mastectomy due to ambiguous margins. This study aimed to determine the incidence of contralateral malignancy among patients undergoing oncoplastic breast-conserving surgery (OBCS) with concurrent symmetry procedures. METHODS: The authors reviewed their prospectively maintained institutional database of patients with unilateral breast cancer who underwent O

[Phyllodes tumor of the breast: analysis of 8 patients].

INTRODUCTION: Phyllodes tumors of breast are rare and usually benign. These are histologically fibro-epithelial tumors similar to fibroadenomas. Histological confirmation on the operative specimen is required to establish the diagnosis and histological pronostic of phyllode tumors. MATERIALS AND METHODS: We reviewed 8 cases of phyllodes tumors and the literature to report the circumstances of occurrence of these tumors, and their specific clinical diagnosis, therapeutic, prognostic features. RESULTS: 62.5% of patients were nulliparous. The mean age at diagnosis was 33.4 years. Mean tumor siz
