BACKGROUND: General anesthesia during pregnancy is not uncommon, for example, for trauma surgery, cerclage, or cesarean delivery. Current recommendations are to maintain maternal partial pressure of carbon dioxide in arterial blood (paCO2) at 30 mm Hg, which is based solely on the average maternal paCO2 in awake pregnant women. However, there is no evidence that this target, compared to other targets, would enable optimal conditions for the fetus during general anesthesia. Maternal paCO2 can affect uterine blood flow, affinity of hemoglobin for oxygen, and fetal CO2 elimination. In this study,

Maternal obesity is increasingly prevalent and is associated with elevated morbidity and mortality rates in both mothers and children. At the interface between the mother and the fetus, the placenta mediates the impact of the maternal environment on fetal development. Most of the literature presents data on the effects of maternal obesity on placental functions and does not exclude potentially confounding factors such as metabolic diseases (e.g., gestational diabetes). In this context, the focus of this review mainly lies on the impact of maternal obesity (in the absence of gestational diabete

Maternal hyperhomocysteinemia (HHcy) is a risk factor for intrauterine growth restriction presumably caused by a decrease in the placental transport of nutrients. We investigated the effect of experimental HHcy induced by daily methionine administration to pregnant rats on the free amino acid levels in the maternal and fetal blood, as well as on morphological and biochemical parameters associated with the amino acid transport through the placenta. HHcy caused an increase in the levels of most free amino acids in the maternal blood on gestational day 20, while the levels of some amino acids in

The present review aims to offer a non-comprehensive outline of the current state-of-the-art and future perspectives on management and therapeutic tools for intrauterine growth restriction (IUGR) and associated prenatal programming in both human and animal species. Animals are used as models for the study of phenomena related to IUGR, but also for research on prenatal therapies with the main objective of designing and developing preventive and therapeutic strategies. The research is currently paying attention on maternal-focused pharmacological treatments and nutritional strategies but also on

Maternal ethanol ingestion may cause fetal injury, particularly impaired somatic and brain growth, by at least two mechanisms: (1) directly, by fetotoxicity from ethanol and/or acetaldehyde; (2) indirectly, by ethanol-induced placental injury and selective fatal malnutrition. There is ample evidence that ethanol and/or acetaldehyde is fetotoxic. In addition, alcoholics are frequently in a state of poor nutrition. However, regardless of maternal nutritional status, ethanol can be placentotoxic, impairing the normal transfer of essential fetal nutrients. Ethanol and/or acetaldehyde has been show

Maternal-fetal blood group incompatibility is common but less commonly results in hemolytic disease of the fetus and newborn (HDFN). HDFN is associated with greater peak bilirubin, at an earlier age, and for longer duration than other causes of hyperbilirubinemia. It poses a substantial risk for kernicterus and accounts for the majority of exchange transfusions for hyperbilirubinemia. Advances in diagnosis and management are described, from identification of the alloimmunized pregnancy by maternal ABO and Rh typing, antibody screen (indirect Coombs test), identification and titration; laborato

Maternal drug intake before delivery may influence the clinical behavior of the newborn. The effects of anesthetic agents and narcotics may cause symptoms of depression in the infant. Infants of substance-abusing mothers may also have altered clinical states. This is a case report of a newborn, very depressed at birth, who apparently was sedated by diphenhydramine (Benadryl) ingested by the mother before delivery. The importance of complete history of drug ingestion by mother before delivery is important in evaluating the behavior of a neonate.

In a group of 15 cases the cortisol concentrations were determined in fetal and maternal plasma during labour and after delivery. In maternal plasma the levels were about twice as high as in fetal plasma and rose up to more than 1000 mug/l during labour (x = 639 +/- 222). After delivery the concentrations decreased. In fetal plasma, cortisol increased during labour from x = 173 +/- to x = 276 +/- 75 at the time of delivery and decreased to x = 106 +/- 36 within the first 23 hours. In a second group of 20 cases maternal and umbilical cord blood and blood of the newborn during the first 28 hours

Microplastics (MPs) are now ubiquitous environmental contaminants that pose major risks to the environment and ecosystem, and in the last decade, concern about their potential threats to the reproductive system has gained widespread recognition. MPs can readily pass through biological barriers such as the placental and blood-brain barriers due to their small size. The ability of MPs to cross the placental barrier causes utmost concern as they could pass to the next generation and affect the health of the offspring. This review addresses the placental biology and its barrier function, as well a

Human listeriosis is a rare disease. It may be foodborne. Listeric infection during pregnancy may give a fatal fetal outcome, caused by transplacental passage of organisms from the maternal gastrointestinal tract. We describe a case of perinatal listeriosis which resulted in preterm stillbirth. Perinatal listeriosis should be considered when flue-like symptoms are presented during pregnancy. Early diagnosis and treatment may improve the outcome.

A review of data reported in the literature and those obtained by the authors on disorders of fetal development in smoking pregnant women, the significance of disturbed maternal adaptation to pregnancy, and maternal, fetal and placental effects of nicotine, carbon dioxide and other constituents of tobacco smoke is presented. The genesis and damaging effect of hypoxia on fetal brain, eventually resulting in mental retardation, are discussed.

Cytomegalovirus (CMV) is nowadays a frequent causal agent in pediatric acquired hearing loss and should become the principal one in the next few years. CMV is a herpes like virus. Eighty percent of all adults are carriers of this virus in the salivary glands, without any symptom. In the pregnant women, CMV can act by reactivation and give a placental and foetal infection. The pregnant woman is generally asymptomatic when infected. In the infant, besides death in utero, the CMV can produce two clinical types. Ten percent of the congenitally infected infants presented symptoms at birth. In a qua

IgG subclass levels in cord serum of unselected European newborns were studied in relation to parental smoking. Cord IgG3 was significantly (P less than 0.01) higher in newborns of smoking mothers (n = 18; geom.mean = 87.0%) compared to those of non-smoking mothers (n = 66; geom.mean = 61.0%). Maternal smoking did not influence (P greater than 0.10) cord IgG2 or cord IgG4. Paternal smoking did not affect any of the three immunoglobulins in cord serum (P greater than 0.10). These data suggest that tobacco smoking affects IgG3 synthesis in some direct or indirect way.

Obstetric emergencies often require intensive care intervention. Amniotic fluid embolism is a rare, unpredictable, and often catastrophic complication of pregnancy that is suspected in a woman who experiences cardiac arrest after a cesarean section. The condition occurs in approximately 1 in 40 000 births and has an average case-fatality rate of 16%. This complication may result from activation of an inflammatory response to fetal tissue in the maternal circulation. Risk factors may include maternal age over 35 years and conditions in which fluid can exchange between the maternal and fetal cir

The placenta serves as a critical interface for maternal-fetal substance exchange, including the active transport of immunoglobulin G (IgG), which is essential for neonatal immune protection. While mechanisms of IgG transfer are well-studied, the impact of therapeutic monoclonal antibodies (mAbs) on placental IgG dynamics during pregnancy remains unclear. This study aimed to predict maternal and fetal drug concentrations of ustekinumab (UST), an IgG1 monoclonal antibody used for treating inflammatory bowel disease (IBD), using a physiologically based pharmacokinetic (PBPK) model. The study uti

Transplacental transfer of maternal antibody in utero is a key component of neonatal immunity. This transfer of maternal antibody across the placenta provides temporary protection from bacterial and viral pathogens that threaten newborn health. Maternal vaccination before and during pregnancy may be used to boost maternal immunity and optimize neonatal protection from infection and serious illness.
