INTRODUCTION: Importance: Vertical maxillary excess (VME) is a common dentofacial deformity that causes downward growth of the maxilla, leading to an elongated lower face, anterior open bite, and excessive gingival display. Although conventional Le Fort I osteotomy remains the gold standard for correcting vertical maxillary excess, it is associated with certain risks, including possible injury to the descending palatine artery (DPA). To improve surgical safety and treatment predictability, several modified osteotomy techniques have been introduced.
OBJECTIVES: To systematically review and com

The size and the position of the sinus antrostomy play a key role in making sinus grafting surgery more predictable and effective with less complications. A cone beam computed tomography and intraoral scan of the maxilla were taken for a patient who is missing maxillary first molar tooth with limited residual bone. Data were exported to the dynamic navigation (DN) system software. Sinus lateral window osteotomy position and dimensions were determined and planned using four 1.5 mm diameter implants placed on the maxillary sinus lateral wall. The osteotomy was initiated following the planned fou

The aim of this study was to evaluate the surgical accuracy of Le Fort I surgery compared to the three-dimensional (3D) virtual planning. Fifty-five patients (29 males, 26 females; age range 15-58 years) with skeletal class III malocclusion, who underwent bimaxillary surgery were included. A validated 3D accuracy assessment tool was utilized to assess the surgical accuracy of the maxillary positioning. For translational movements, the least amount of error was associated with mediolateral translation, whereas the surgical accuracy for anteroposterior and superoinferior translation showed a ten

PURPOSE: The correction of maxillomandibular deformities may require maxillary osteotomy procedures that usually present low rates of postoperative complications, such as maxillary sinusitis. The present study evaluated the incidence of maxillary sinusitis after Le Fort I osteotomy in 21 adult patients who underwent maxillary surgery (Le Fort I osteotomy) or bimaxillary surgery (Le Fort I osteotomy plus sagittal mandibular osteotomies) for correction of dentofacial deformities.
PATIENTS AND METHODS: Verification of the presence of maxillary sinusitis was assessed through a brief questionnaire

The purpose of the study was to investigate the clinical application and long-term stability of maxillary setback in Le Fort I osteotomy using maxillary tuberosity removal or intentional pterygoid plate fracture (IPPF). Eighty adult class II patients who underwent Le Fort I osteotomy with bilateral sagittal split ramus osteotomy by the same surgeon between January 2013 and January 2019 were included in this retrospective study. Traditional maxillary tuberosity removal was performed in 40 patients (group I), and the other 40 patients (group II) underwent IPPF to set back the maxilla according t

BACKGROUND: Maxillary avascular necrosis (AVN) after Le Fort I osteotomy is rare but potentially devastating. Although injury to the descending palatine artery has traditionally been regarded as a major cause, the relative contribution of individual vascular and surgical factors remains unclear. This systematic review aimed to synthesize the clinical evidence on maxillary AVN after orthognathic surgery and to evaluate whether postoperative ischemic complications are better explained by isolated arterial injury or cumulative vascular compromise.
MAIN BODY: A systematic literature search of Pub

Epistaxis after Le Fort I osteotomy is one of the relatively common postoperative complications. It can be controlled with conservative treatment, such as nasal packing, and will usually improve in a few days. However, if the epistaxis is repeated, the outcome can be life-threatening. A 22-year-old woman underwent Le Fort I osteotomy in order to correct her malocclusion. Postoperatively, pseudoaneurysm was formed in the descending palate artery, causing repeated epistaxis. Then, angiography and embolization were performed. Before the onset of epistaxis, there was discomfort around the nasal ar

A systematic review was conducted to investigate the available evidence on maxillary complications related to piezoelectric and conventional surgery. Seven databases were searched. A total of 996 maxillary osteotomies were analysed, 864 performed with conventional tools and 132 with a piezoelectric device. One hundred and fifty-six complication events were reported. The complications, in descending order of overall prevalence, were as follows: neurosensory disturbance (64.7%), haemorrhage (8.3%), oroantral communication (7.7%), soft tissue injury (7.7%), tooth injury (5.1%), infection (3.2%),

PURPOSE: Two- and 3-dimensional studies on changes of the pharyngeal airway after a Le Fort I osteotomy have been conducted, but there are few studies on the change of nasal structure. The nasal cavity is the first passage through which air enters during respiration. Movement of the maxilla by Le Fort I osteotomy affects the shape of the nasal cavity and the shape of other parts of the airway. This study compared preoperative and postoperative changes of the nasal cavity volume after a maxillary Le Fort I osteotomy.
MATERIALS AND METHODS: Pre- and postoperative computed tomograms of 32 patien

The purpose of the present study was to evaluate upper airway changes related to craniofacial changes induced by a facemask (FM) combined with Le Fort I osteotomy without down-fracture in Class III maxillary retrusion subjects (group 1) when compared with a similar group of subjects treated with traditional rapid palatal expansion and FM therapy. Pre- (T1) and post- (T2) protraction cephalometric radiographs of group 1 (10 females and 6 males; mean age 12.75 ± 1.91 years) and group 2 (7 females and 9 males; mean age 12 ± 1.7 years) were traced. The treatment duration was 149 ± 14 days (approxi

Five Macaca irus monkeys were used in this study to determine cephalometric and histologic changes after total maxillary impactions. In the three experimental animals, the maxilla was impacted 5 to 7 mm. and lateral cephalograms were taken over a 150-day period. By superimposing on craniofacial implants, osseous and dental changes were measured. Excellent skeletal stability was demonstrated during the postoperative period. Histologically, there was normal osseous healing of the osteotomized maxillas. Accompanying the counterclockwise rotation (overclosure) of the mandible, there was extensive

Orthognathic surgery is a typical approach for treating maxillofacial deformities. However, orthognathic surgery results in positional changes in the condyles. In a previous review, the effects of orthognathic surgery on temporomandibular joints (TMJs) are not provided. Hence, in this study, we investigate the morphological changes in TMJs after mandibular and bi-maxillary osteotomies. The relationship between the morphological parameters of TMJs and symptoms of temporomandibular disorders (TMDs) is discussed. Finite element contact stress analysis is performed, and the results show that the t

The purpose of orthognathic surgery procedures is to manipulate the elements of the facial skeleton to reestablish normal anatomy and function. These elements could be repositioned with the technique known as Le Fort I osteotomy. This technique can be modified to be adapted on each patient, depending on the anomaly presented and the planned objective. The versatility, offered by the modifications of the Le Fort I osteotomy, has taken us to design one that allows the capability to correct the malar region hypoplasia in the same surgical procedure. A total of 9 patients were included with malocc

The use of virtual surgery, patient-specific saw and drill guides, and custom-made osteosynthesis plates is rapidly spreading from deformity surgery to orthognathic surgery. Most of the commercially available systems are using computer-aided design/computer-aided manufacture (CAD/CAM) wafers to produce patient-specific saw guides. However, most plate systems provided are still the conventional "in stock" mini plates that can be individually designed by pre-bending according to the stereolithographic model of the patient. Custom made three-dimensional (3D) printed implants have earlier been dem

BACKGROUND: Abnormal facial growth is a well-known sequelae of cleft lip and palate (CLP) resulting in maxillary retrusion and a class III malocclusion. In 10-50% of cases, surgical correction involving advancement of the maxilla typically by osteotomy methods is required and normally undertaken in adolescence when facial growth is complete. Current evidence for the impact of the surgery on velopharyngeal function is weak and mixed.
AIMS: The first objective of the study was to investigate the nature of the effect of maxillary osteotomy on the perceptual outcomes of velopharyngeal function in
