A Multimodal AI Framework for Medical Education: Integrating Adaptive Image Retrieval, Fast Synthesis, and LLM-Based Clinical Auditing.

Access to reliable medical images is essential for clinical training. To address this need, this paper presents an extended version of MIRAGE, a multimodal retrieval and generation system that utilizes a shared latent space to process medical queries by retrieving real images from the ROCO dataset, generating synthetic scans, and providing LLM-based clinical descriptions alongside dual-concept visual comparisons. To overcome previous computational limits and the lack of clinical validation, we introduce three core enhancements: first, an Auto-α module to dynamically weight visual and textual s

Clinical audit examining the impact of benzalkonium chloride-free anti-glaucoma medications on patients with symptoms of ocular surface disease.

BACKGROUND: Ocular surface disease (OSD) is relatively common in glaucoma patients. OSD symptoms could be linked to prolonged exposure to preservatives in anti-glaucoma medications, especially benzalkonium chloride (BAK). The OBSERVE clinical audit was designed to track the impact of intraocular pressure lowering medications in patients with evidence of OSD to test the hypothesis that BAK-free anti-glaucoma preparations offer clinical advantages over BAK-containing products. DESIGN: Prospective clinical audit from March 2012 to April 2013, open to ophthalmologists practising in Australia. PA

Surgical rib fixation as an alternative method of treatment for multiple rib fractures: an audit of results compared with traditional medical management.

BACKGROUND: Rib fractures are a common cause of morbidity and chronic pain, delaying return to normal activities. Reports suggest that surgical fixation improves acute and long-term outcomes. METHODS: A single centre retrospective review of multiple rib fractures, comparing the outcomes of cases managed using surgical fixation with cases managed only with best medical therapy (BMT) over 2 years. RESULTS: Thirty-five patients with rib fractures were admitted over the study period. The most common causes of rib fractures were motorcycle crashes (34.2%) and falls (31.4%). Fourteen patients had

Improving 24-Hour Medication Reconciliation Through Audit and Feedback: A Multicycle Quality Improvement Study at a Scottish District General Hospital.

Introduction Medication reconciliation (MR) is a key patient safety process during hospital admission. MR ensures an accurate list of a patient's medications is compiled on hospital admission, enabling safe downstream prescribing and reducing adverse events. This responsibility falls primarily to the admitting clinician in paper-based systems with limited pharmacy input at the point of admission. This study aimed to assess local compliance with the national benchmarks and evaluate whether a structured audit-feedback intervention could improve MR documentation and the recording of reasons for w

Manual chart review reveals bisphosphonate exposure misclassification in atypical femur fracture patients: auditing medical records in two international cohorts.

UNLABELLED: We examined if inaccuracies in reported BP treatment in two large cohorts could explain the wide discrepancy observed between different patient cohorts with atypical femur fractures. Assessing BP treatment through manual review of medical and pharmacy records increased the discrepancy, indicating that other factors likely contribute to the observed variation. PURPOSE: Variation in results across fracture studies can stem from differences in study design, case definitions, data sources and healthcare settings, but in atypical femur fracture (AFF) studies, the reported proportion of

Outpatient management of urinary tract infections by medical officers in Nairobi, Kenya: lack of benefit from audit and feedback on adherence to treatment guidelines.

INTRODUCTION: Acute uncomplicated urinary tract infections are common in outpatient settings but are not treated optimally. Few studies of the outpatient use of antibiotics for specific diagnoses have been done in sub-Saharan Africa, so little is known about the prescribing patterns of medical officers in the region. METHODS: Aga Khan University has 16 outpatient clinics throughout the Nairobi metro area with a medical officer specifically assigned to that clinic. A baseline assessment of evaluation and treatment of suspected UTI was performed from medical records in these clinics. Then the m

First-line treatment with ceftriaxone for Neisseria gonorrhoeae infection less likely to be prescribed to patients with a penicillin allergy label: a retrospective audit of medical records.

Background Gonorrhoea notifications have increased substantially in Australia over the past decade. Neisseria gonorrhoeae is already highly resistant to several antibiotics and so, alternatives to first-line treatment are generally strongly discouraged. The penicillin allergy label (AL) on patient medical records has previously been shown to influence prescribing practices, to the detriment of best-practice management and antimicrobial stewardship. This study aimed to understand how the penicillin AL influences antibiotic selection for gonorrhoea treatment at Canberra Sexual Health Centre. Met
