Medication errors with the use of allopurinol and colchicine: a retrospective study of a national, anonymous Internet-accessible error reporting system.

OBJECTIVE: To more closely assess medication errors in gout care, we examined data from a national, Internet-accessible error reporting program over a 5-year reporting period. METHODS: We examined data from the MEDMARX database, covering the period from January 1, 1999 through December 31, 2003. For allopurinol and colchicine, we examined error severity, source, type, contributing factors, and healthcare personnel involved in errors, and we detailed errors resulting in patient harm. Causes of error and the frequency of other error characteristics were compared for gout medications versus othe

Pregabalin Toxicity in Kidney Failure: A Medication Error With Neurological Consequences.

We present a case of pregabalin toxicity in a 50-year-old dialysis-dependent female, who presented with a one-day history of progressive weakness, lethargy, and gait disturbance. They were successfully treated with haemodialysis. The case is notable for the atypical toxidrome seen in kidney failure, which occurred after a community pharmacy error led to a single twelvefold-increased dose. It also demonstrates the role of the emergency department pharmacist, who was crucial in identifying the medication error. The report emphasises the importance of careful medication reconciliation, especially

Medication Errors in Overweight and Obese Pediatric Patients: A Narrative Review.

BACKGROUND: The childhood obesity epidemic in the United States has increased utilization of health care and prescribing of medications in overweight and obese children, yet it is unclear whether this has led to more medication errors. The objective of this study was to review all available literature on incidence and types of medication errors in overweight and obese children. METHODS: A search of MEDLINE, Embase, and Scopus databases was conducted for all studies and oral abstracts through December 2020 reporting medication errors in overweight or obese children aged ≤ 18 years. All studies

[Medication errors: analysis of a digoxin overdose case in a neonatology unit].

Neonatology and pediatrics are units where medication errors occur. Indeed, the complexity of nursing care, the lack of information and marketing authorization for drugs contribute to the occurrence of often underestimated iatrogenic events. Through a case of digoxin overdose in a neonatology unit, each stage of the drug circuit was analyzed. From prescription to administration to dispensation, the accumulation of individual errors put a newborn in danger. The analysis and declaration of such events can improve safety and the quality of patient care.

The role of advice in medication administration errors in the pediatric ambulatory setting.

BACKGROUND: In the pediatric setting, adverse events occurring at the administration stage are the most common type of preventable adverse drug events. Few data are available on the effect of advice from medical professionals on medication safety. METHODS: This is a prospective cohort study of 1685 pediatric patients, 6 office practices in the Boston area. Data were collected from parental interviews, review of duplicate prescriptions, and chart review. Incidents were stratified by type (medication error, near miss, or preventable adverse drug event) and stage of the medication process. Descr

Medication errors with opioids: results from a national reporting system.

BACKGROUND: Errors may be more common and more likely to be harmful with opioids than with other medications, but little research has been conducted on these errors. METHODS: The authors retrospectively analyzed MEDMARX, an anonymous national medication error reporting database, and quantitatively described harmful opioid errors on inpatient units that did not involve devices such as patient-controlled analgesia. The authors compared patterns among opioids and qualitatively analyzed error descriptions to help explain the quantitative results. RESULTS: The authors included 644 harmful errors

Fatality involving vinblastine overdose as a result of a complex medical error.

The purpose of the study is a presentation of a fatal case involving an 83-year-old woman, who died due to an overdose of vinblastine-a cytostatic agent of a vinca alkaloid employed in cancer chemotherapy. The postmortem investigation included an autopsy and histological examination, as well as a toxicological analysis of post-mortem specimens collected in the course of autopsy. The authors performed a toxicological assessment of vinblastine employing liquid chromatography-atmospheric pressure chemical ionization-tandem mass spectrometry (LC-MS-MS-APCI). The determined vinblastine concentratio

Operator error is the key factor contributing to medical laser accidents.

Useful lessons can often be learned from studying laser accidents. Although most countries require such accidents to be reported to the relevant authority, often such information is not accessible to laser users. Consequently, it was decided to collect detailed accounts of incidents from the British Medical Laser Association's Internet mailing list. Twelve incidents were reported by e-mail after a request for information sent to the BMLA mailing list in 2003. Although these are of a diverse nature, operator error accounted for 67% of the cases. This factor should be recognized when developing

Impact of Clinical Pharmacist-Led Interventions on the Interception of Medication Errors and Error-Related Toxicity in Pediatric Oncology Patients.

BACKGROUND: Pediatric oncology patients are highly susceptible to medication errors, because chemotherapeutic agents are high-alert drugs with a narrow therapeutic index requiring individualized, weight-based dosing. Prospective intervention-based evidence from resource-limited settings is scarce. This study evaluated the effect of clinical pharmacist interventions on medication errors in pediatric oncology inpatients. METHODS: In a prospective pre-post study in the central teaching hospital of pediatrics in Baghdad, Iraq (September 2025-April 2026), 125 patients receiving high-alert medicati

Towards improved understandings of medical manslaughter: fatal medical error, accountability, and wider paths to learning.

Criminal liability for fatal errors made by healthcare professionals, medical manslaughter (MM), has prompted much academic and professional debate. There is concern that doctors are increasingly vulnerable to this offence when they make inadvertent clinical errors in complex healthcare systems where mistakes are common and often arise from systemic causes. Yet, these discussions have been dominated by a few appellate court cases and focused on doctors, not victims. This highlights a key data gap and suggests that there may be patterns in MM cases not fully understood. This article analyses 19

Medication Errors and Gaps in Medication Discharge Planning for Hospitalized Older Adults: A Prospective Cohort Study.

BACKGROUND: Hospitalized older adults are commonly discharged with changes to antihypertensive and glucose-lowering (cardiometabolic) medications. Though adverse drug events remain a leading cause of readmissions, there is little contemporary data on how medication discharge planning is communicated and how often medication errors occur post-discharge. OBJECTIVE: To assess older adults' post-hospital medication use and ambulatory follow-up after receiving cardiometabolic medication changes during hospitalization. DESIGN: Prospective cohort study from 11/2022 to 01/2024. PARTICIPANTS: Adults

How Surgeons and Surgical Leaders Manage Complications, Medical Errors, Malpractice, and Second Victim Syndrome.

Complications are inevitable for practicing surgeons, and when you lead surgeons, negative outcomes and consequences will result from some of their professional work. The implications of the undesired, unintended, or unexpected changes to a child's health due to surgical intervention can be either transient or permanent, ranging from minor to major, devastating, or even deadly adverse events. Just as surgeons strive to maximize their surgical knowledge and expertise before performing an operation, surgeons and their teams need to practice and learn how to improve their non-technical leadership

Effect of reducing interns' work hours on serious medical errors in intensive care units.

BACKGROUND: Although sleep deprivation has been shown to impair neurobehavioral performance, few studies have measured its effects on medical errors. METHODS: We conducted a prospective, randomized study comparing the rates of serious medical errors made by interns while they were working according to a traditional schedule with extended (24 hours or more) work shifts every other shift (an "every third night" call schedule) and while they were working according to an intervention schedule that eliminated extended work shifts and reduced the number of hours worked per week. Suitability of Hair Testing in Medical Dispensing Errors: A Fatal Case of Gliclazide Poisoning.

A patient was prescribed a new treatment, 40 mg furosemide. Her pharmacist mistakenly dispensed 40 mg gliclazide instead. After 3 weeks of treatment, the patient was found in a coma, was hospitalised and died after a week. Hair sample was collected during post-mortem examination and tested for gliclazide on three 2 cm sections, starting at the root end. Hair strands were decontaminated, segmented and incubated in the presence of diazepam-d5, and a solid-liquid extraction has been performed. Finally, toxicological analyses were performed by UFLC Shimadzu Prominence - MS/MS Sciex 6500 QTRAP. Gli
