Perspective: Application of the American College of Medical Genetics Variant Interpretation Criteria to Common Variable Immunodeficiency Disorders.

Common variable immunodeficiency disorders (CVIDs) are rare primary immunodeficiency diseases (PIDs) mostly associated with late onset antibody failure leading to immune system failure. Patients with CVID are predisposed to disabling complications such as bronchiectasis and systemic autoimmunity. In recent years a large number of genetic defects have become associated with these disorders. Patients with a causative mutation are deemed to have CVID-like disorders, while those with mutations predisposing to or modifying disease severity remain within the spectrum of CVID as defined by current di

Medical therapy, radiofrequency ablation or cryoballoon ablation as first-line treatment for paroxysmal atrial fibrillation: interpreting efficacy through restricted mean survival time and network meta-analysis.

When multiple treatments are available, network meta-analysis can evaluate data to rank the relative effectiveness. We applied this approach to first-line treatments for paroxysmal atrial fibrillation (medical therapy, radiofrequency ablation or cryoballoon ablation). Individual trials were analysed based on the restricted mean survival time (RMST). Randomised controlled trials (RCT) assessing first-line treatments for paroxysmal atrial fibrillation were referenced from PubMed and the websites of regulatory agencies. The primary end-point was atrial fibrillation recurrence-free survival at 12

Medical and health science students' interpretation of miscommunication in simulated scenarios: the role of language, gender, and perceived professional authority.

INTRODUCTION: Little is known about how language, gender, and perceived professional authority simultaneously influence medical communication between doctors and patients. This study addresses this gap by examining Saudi medical and health science students' attribution of miscommunication in four medical simulation scenarios. METHODS: Two groups of students observed two scenarios each, in which the agents' linguistic backgrounds, genders, and professional roles were systematically varied. Using a mixed-method design, students provided written responses to an elicitation task, followed by focu

Interference of Residual 99m Tc-MIBI Activity in Syringe Needle on 99m Tc-DMSA Scan Interpretation; Medical Error.

A 4-month-old infant with a small right kidney underwent a 99m Tc-DMSA scan showing unjustifiable findings including global cortical loss in the left kidney, increased liver and bowel uptake. Retrospective review revealed reuse of a sterile needle contaminated with residual 99m Tc-MIBI, compromising scan quality and falsely elevating right kidney function. A repeat scan showed normal left kidney uptake and significantly reduced right kidney function. This case highlights the importance of employing safe techniques with disposable equipment, thorough workflow review, and transparent error repor

Medical recommender systems based on continuous-valued logic and multi-criteria decision operators, using interpretable neural networks.

BACKGROUND: Out of the pressure of Digital Transformation, the major industrial domains are using advanced and efficient digital technologies to implement processes that are applied on a daily basis. Unfortunately, this still does not happen in the same way in the medical domain. For this reason, doctors usually do not have the time or knowledge to evaluate all alternative treatment options for each patient accurately and individually. However, physicians can reduce their workload by using recommender systems, still having every decision under control. In this way, they also get an insight int

Shared System-Level Drivers of Unprofessional Conduct During Clinical Rotations Among Medical and Dental Students: An Interpretive Description Study.

Introduction Unprofessional conduct in clinical education refers to behaviours that deviate from accepted professional norms. These behaviours undermine learner well-being, patient dignity, and the development of professional identity. Findings from previous literature suggest that unprofessional conduct is common in Pakistani clinical training and may be reinforced within clinical environments, potentially through the influence of negative elements of the hidden curriculum. While individual incidents are often reported, less is known about how institutional and system-level factors interact t

A new medical imaging postprocessing and interpretation concept to investigate the clinical relevance of incidentalomas: can we keep Pandora's box closed?

BACKGROUND: Incidental imaging findings (incidentalomas) are common, but there is currently no effective means to investigate their clinical relevance. PURPOSE: To introduce a new concept to postprocess a medical imaging examination in a way that incidentalomas are concealed while its diagnostic potential is maintained to answer the referring physician's clinical questions. MATERIAL AND METHODS: A deep learning algorithm was developed to automatically eliminate liver, gallbladder, pancreas, spleen, adrenal glands, lungs, and bone from unenhanced computed tomography (CT). Variant Interpretation for Dilated Cardiomyopathy: Refinement of the American College of Medical Genetics and Genomics/ClinGen Guidelines for the DCM Precision Medicine Study.

BACKGROUND: The hypothesis of the Dilated Cardiomyopathy Precision Medicine Study is that most dilated cardiomyopathy has a genetic basis. The study returns results to probands and, when indicated, to relatives. While both the American College of Medical Genetics and Genomics/Association for Molecular Pathology and ClinGen's MYH7-cardiomyopathy specifications provide relevant guidance for variant interpretation, further gene- and disease-specific considerations were required for dilated cardiomyopathy. To this end, we tailored the ClinGen MYH7-cardiomyopathy variant interpretation framework; t

Knowledge level of ECG interpretation on STEMI, its equivalents and mimics among emergency department medical officers: a single-centre study.

BACKGROUND: Accurate electrocardiogram (ECG) interpretation is essential in emergency care, particularly for recognising ST-segment elevation myocardial infarction (STEMI), STEMI equivalents, and STEMI mimics. These patterns guide reperfusion decisions and early management, yet the evolving myocardial infarction paradigm has increased interpretive complexity. This study aimed to assess the ECG interpretation knowledge of STEMI, STEMI equivalents, and STEMI mimics among medical officers in the Emergency Department (ED) and to identify factors associated with their performance. METHODS: A cross

Interpretable CRAM‑Enhanced Lightweight Dual‑Branch CNN for Real‑Time Breast Cancer Histopathology in Internet‑of‑Medical‑Things Environments.

Breast cancer remains a primary global health concern, with histopathological image analysis serving as the diagnostic gold standard. However, manual microscopy is time-consuming and often subjective. While deep learning offers a powerful solution, existing models are typically too complex and opaque for real-time use in Internet of Medical Things (IoMT) environments. To address this, we propose an interpretable and lightweight hybrid deep learning model that combines MobileNetV2 and EfficientNet-B0, enhanced by a novel contextual recurrent attention module (CRAM). CRAM refines fused features

Cancer Counseling of Low-Income Limited English Proficient Latina Women Using Medical Interpreters: Implications for Shared Decision-Making.

In cancer genetic counseling (CGC), communication across language and culture challenges the model of practice based on shared decision-making. To date, little research has examined the decision-making process of low-income, limited English proficiency (LEP) patients in CGC. This study identified communication patterns in CGC sessions with this population and assessed how these patterns facilitate or inhibit the decision-making process during the sessions. We analyzed 24 audio recordings of CGC sessions conducted in Spanish via telephone interpreters at two public hospitals. Patients were refe

Synergistic medical genetic evolutionary optimization and deep convolutional generative augmentation with SHAP-driven interpretability for precise Alzheimer's disease severity grading.

Alzheimer's disease (AD) diagnosis at an early yet accurate stage is critical to support effective treatment or intervention. Still it is not very feasible due to the presence of image data class imbalance, low interpretability of models, and a high computational cost. This research proposes a novel, end-to-end diagnostic framework that considers a Medical Genetic Algorithm (MedGA)-optimized Convolutional Neural Network (CNN) with a Deep Convolutional Generative Adversarial Network (DCGAN) to generate synthetic MRIs and SHapley Additive Explanations (SHAP) to analyse and interpret the model.
