Medical countermeasures for the hematopoietic-subsyndrome of acute radiation syndrome in space.

More than 50 years after the Apollo missions ended, the National Aeronautical and Space Administration (NASA) and other international space agencies are preparing a return to the moon as a step towards deep space exploration. At doses ranging from a fraction of a Gray (Gy) to a few Gy, crew will be at risk for developing bone marrow failure associated with the hematopoietic subsyndrome of acute radiation syndrome (H-ARS) requiring pharmacological intervention to reduce risk to life and mission completion. Four medical countermeasures (MCM) in the colony stimulating factor class of drugs are no

The humanitarian and outreach ethics charter in ENT. Ethics at the heart of missions.

Humanitarian action, in the broad sense, including global outreach missions by ENT physicians, has been effective for many years. These volunteer missions have not had any specific regulatory framework, be it medical, social or above all ethical. The testimony of ENT physicians in certain "recipient" countries, in particular during the 2017 Paris Congress of the International Federation of Otorhinolaryngological Societies (IFOS), revealed the importance of adequate coordination between "Источник" and "recipient" teams. Several ethical reports have been made, but few specifically focused on ENT

[Immersion in a surgical training program in Burundi for posterior perineal reconstruction].

This article describes a humanitarian teaching mission dedicated to managing complex posterior perineal injuries in Burundi. These conditions, which result from prolonged labor in low-resource settings, may lead to fecal incontinence, causing social exclusion and loss of dignity. Through the perspective of medical students, this article offers an exploratory view of these pathologies and the model of knowledge transfer, highlighting the importance of sharing skills with the next generation of healthcare professionals. Beyond technical expertise, this approach aims to restore patients' dignity

Helicopter inter-hospital transfer for patients undergoing extracorporeal membrane oxygenation: a retrospective 12-year analysis of a service system.

BACKGROUND: Patients undergoing extracorporeal membrane oxygenation (ECMO) are critically ill and show high mortality. Inter-hospital transfer of these patients has to be safe, with high survival rates during transport without potentially serious and life-threatening adverse events. The Swiss Air-Rescue provides 24-h/7-days per week inter-hospital helicopter transfers that include on-site ECMO cannulation if needed. This retrospective observational study describes adverse events of patients on ECMO transported by helicopter, and their associated survival. METHODS: All patients on ECMO with in

[Experience of DMAT rescue activity by doctor-helicopter in Tohoku Area after the earthquake].

We operated rescue activities in Tohoku area after the earthquake of March 11th, 2011. From our hospital, a doctor-helicopter flew to the staging care unit at Hanamaki airport with two members of the disaster medical assistance team (DMAT), one of whom was an anesthesiologist. The helicopter carried ten patients by nine flight missions, who were the victims of tsunami after the earthquake. There were seven doctor-helicopters from all over Japan and did the missions based at Hanamaki airport. The missions was quite different from our usual job as an anesthesiologist, but we could transfer the p

Pharmacologic considerations for Shuttle astronauts.

Medication usage by crewmembers in the preflight and inflight mission periods is common in the Shuttle Program. The most common medical reports for which medication is used are: space motion sickness (SMS), sleeplessness, headache, and backache. A number of medications are available in the Shuttle Medical Kit to treat these problems. Currently, astronauts test all frequently used medications before mission assignment to identify potential side-effects, problems related to performance, personal likes/dislikes, and individual therapeutic effect. However, microgravity-induced changes in drug phar

Applications of the ocular trauma score (OTS) as a framework for spaceflight related ocular trauma (SROT) triage and management.

Spaceflight presents several unique challenges for the diagnosis and management of in-flight ocular trauma. Constrained medical resources, microgravity, and delayed access to advanced medical care require novel and unique approaches to ocular trauma. The Ocular Trauma Score (OTS), an evidence-based clinical tool, aids in establishing a prognosis for ocular injuries and offers a method of informed triage and management. As governmental space agencies and private space companies (e.g., SpaceX, Blue Origin, and Virgin Galactic) rapidly increase the number and length of human spaceflight missions,

Look right! A retrospective study of pedestrian accidents involving overseas visitors to London.

INTRODUCTION: Research within the European Union has shown international visitors to have a higher injury mortality than residents. Traffic accidents are the leading cause of injury-related death among overseas visitors and evidence suggests overseas visitors are at a greater risk of being involved in road traffic accidents than the resident population. Little information looks specifically at pedestrian injuries to overseas visitors. Pedestrian deaths account for 21% of all UK road deaths. METHODS: A retrospective database review of London helicopter emergency medical service (HEMS) missions

Medication use by U.S. crewmembers on the International Space Station.

The environment on the International Space Station (ISS) includes a variety of potential physiologic stressors, including low gravity, elevated exposure to radiation, confined living and working quarters, a heavy workload, and high public visibility. This retrospective study examined medication use during long-duration spaceflights (>30 d). Medication records from 24 crewmembers on 20 missions longer than 30 d over a 10 yr period were examined for trends in usage rates, efficacy, and indication, as well as adverse event quality, frequency, and severity. Results were compared with those from cr

Exploring the experiences of cardiovascular perfusionists during philanthropic cardiac surgery in low-income countries.

Background: Medical mission trips address the global burden of cardiac disease, particularly in Sub-Saharan Africa, where limited access to cardiac surgery exacerbates challenges faced by affected individuals. However, a lack of literature exists on the experiences of perfusionists involved in these missions, despite their crucial role in delivering cardiac surgery services. Therefore, the purpose of this descriptive study was to evaluate the experiences of cardiovascular perfusionists involved in philanthropic cardiac surgery in low-income countries. Methods: A descriptive survey research desi
