Accuracy of obstetric laceration diagnoses in the electronic medical record.

INTRODUCTION AND HYPOTHESIS: Patient safety data including rates of obstetric anal sphincter injury (OASI) are often derived from hospital discharge codes. With the transition to electronic medical records (EMRs), we hypothesized that electronic provider-entered delivery data would more accurately document obstetric perineal injury than traditional billing/diagnostic codes. METHODS: We evaluated the accuracy of perineal laceration diagnoses after singleton vaginal deliveries during one calendar year at an American tertiary academic medical center. We reviewed the entire hospital chart to dete

Presymptomatic Testing and Confidentiality in the Age of the Electronic Medical Record.

OBJECTIVE: Recent introduction of a commercial electronic medical record (EMR) system at the authors' institution raised a number of questions about documenting visits for presymptomatic testing for Huntington's disease (HD). Specifically, adoption of the EMR potentially compromised patient confidentiality and the personal delivery of test results, both of which are strongly recommended by professional consensus and lay organizations. METHODS: The authors surveyed peer institutions about their experience with EMR systems in the setting of presymptomatic testing for HD. RESULTS: Answers from

Structured data system for a breast cancer medical record.

We have developed software for an Electronic Medical Record (EMR) to be used by oncologists and researchers. It has rapid, structured data entry, visualization of clinical information and a searchable data base. Interactive, rules-based forms were designed for structured data entry. The web-based forms have been customized for the clinical staff who enter the data. The forms have been tested by oncologists and their office assistants. Modules have been added to upload images and add legends, metadata, and code classifications such as ICD and CPT. Other features include a search interface and a

Mitigating algorithmic unfairness arising from forgetfulness of medical records in clinical artificial intelligence.

Protecting patients' right to be forgotten (RtbF) in the digital era underscores algorithms to delete health records from deployed artificial intelligence (AI) models upon requests, a paradigm also known as machine unlearning. Such forgetfulness, however, will alter AI models' views of subpopulations and could yield discriminating clinical decisions that undermine healthcare equality and endanger patient trust, a common phenomenon confirmed by our thorough investigations. This observation reveals an ethical dilemma in clinical AI: protecting the RtbF of certain patients will compromise the del

Cognitive performance classification of older patients using machine learning and electronic medical records.

Dementia rates are projected to increase significantly by 2050, posing considerable challenges for healthcare systems worldwide. Developing efficient diagnostic tools is critical, and machine learning (ML) algorithms have shown potential for improving the accuracy of cognitive impairment classification. This study aims to address challenges in current systems by leveraging readily available electronic medical record (EMR) data to simplify and enhance the classification of cognitive impairment. The analysis includes 283 older adults, categorized into three groups: 144 individuals with mild cogn

Practice for adolescent stuttering: a review of medical records from a Swedish cohort.

Background: Stuttering, during adolescence, may influence adult life. However, little is known about currently applied clinical procedures for assessment and treatment of stuttering in adolescence. Aim: The aim of the present study was to investigate clinicians' assessment and intervention routines and if choice of intervention was related to factors such as age, gender, or stuttering severity. Method: Medical records from 51 adolescents who had visited a Speech and Language Therapy clinic were included. Group differences were analysed through nonparametric Mann-Whitney U test and correlations

Using electronic medical records to identify patients at risk for underlying cardiac amyloidosis.

BACKGROUND: Identification of transthyretin cardiac amyloidosis (ATTR-CA) patients is largely based on pattern recognition by providers, and this can be automated through electronic medical systems (EMR). METHODS: All patients in a large academic hospital with age > 60, ICD-10 code for chronic diastolic heart failure and no previous diagnosis of any amyloidosis were included. An Epic EMR scoring logic assigned risk scores to patients for ICD-10 and CPT codes associated with ATTR-CA, as follows: carpal tunnel syndrome (score 5), aortic stenosis/TAVR (5), neuropathy (4), bundle branch block (4)

Artificial intelligence-aided data mining of medical records for cancer detection and screening.

The application of artificial intelligence methods to electronic patient records paves the way for large-scale analysis of multimodal data. Such population-wide data describing deep phenotypes composed of thousands of features are now being leveraged to create data-driven algorithms, which in turn has led to improved methods for early cancer detection and screening. Remaining challenges include establishment of infrastructures for prospective testing of such methods, ways to assess biases given the data, and gathering of sufficiently large and diverse datasets that reflect disease heterogeneit

Streamlining documentation in patient electronic medical records: An example of chlamydia consultation shortcuts.

BACKGROUND: Australian general practitioners encounter a vast array of health issues in their clinical practice. High-quality clinical record keeping is crucial to support continuity of care for patients and decision making for clinicians. Many clinical software programs used in general practice contain shortcut features that allow insertion of pre-stored, editable text into progress notes. These can be used to support documentation of specific health issues. OBJECTIVE: Drawing on our experience within a research project seeking to strengthen chlamydia management in general practice, this art

Examining multimorbidity differences across racial groups: a network analysis of electronic medical records.

Health disparities across ethnic or racial groups are typically examined through single behavior at a time. The syndemics and multimorbidity health disparities have not been well examined by race. In this study, we study health disparities by identifying the networks of multimorbidities among individuals from seven population groups based on race, including White, African American, Asian, Hispanic, Native American, Bi- or Multi-racial and Pacific Islander. We examined a large electronic medical record (EMR) containing health records of more than 18.7 million patients and created multimorbidity

Assessing Information Congruence of Documented Cardiovascular Disease between Electronic Dental and Medical Records.

Dentists are more often treating patients with Cardiovascular Diseases (CVD) in their clinics; therefore, dentists may need to alter treatment plans in the presence of CVD. However, it's unclear to what extent patient-reported CVD information is accurately captured in Electronic Dental Records (EDRs). In this pilot study, we aimed to measure the reliability of patient-reported CVD conditions in EDRs. We assessed information congruence by comparing patients' self-reported dental histories to their original diagnosis assigned by their medical providers in the Electronic Medical Record (EMR). To

Extracting Primary Open-Angle Glaucoma from Electronic Medical Records for Genetic Association Studies.

Electronic medical records (EMRs) are being widely implemented for use in genetic and genomic studies. As a phenotypic rich resource, EMRs provide researchers with the opportunity to identify disease cohorts and perform genotype-phenotype association studies. The Epidemiologic Architecture for Genes Linked to Environment (EAGLE) study, as part of the Population Architecture using Genomics and Epidemiology (PAGE) I study, has genotyped more than 15,000 individuals of diverse genetic ancestry in BioVU, the Vanderbilt University Medical Center's biorepository linked to a de-identified version of

Detecting pregnancy use of non-hormonal category X medications in electronic medical records.

OBJECTIVES: To determine whether a rule-based algorithm applied to an outpatient electronic medical record (EMR) can identify patients who are pregnant and prescribed medications proved to cause birth defects. DESIGN: A descriptive study using the University of Pennsylvania Health System outpatient EMR to simulate a prospective algorithm to identify exposures during pregnancy to category X medications, soon enough to intervene and potentially prevent the exposure. A subsequent post-hoc algorithm was also tested, working backwards from pregnancy endpoints, to search for possible exposures that

A comparison of pregnancy history recall and medical records. Implications for retrospective studies.

Using data from the Diethylstilbestrol-Adenosis (DESAD) Project, a study of the effects of diethylstilbestrol (DES) exposure during fetal life, the authors compared prenatal records with obstetric history from mother's questionnaires completed 10 or more years after the birth of their daughters. Except for the history of hospitalization and trunk x-ray, no differences were observed in agreement (questionnaire compared with record) between the group of DES-exposed mothers identified through review of their prenatal records and the comparison group of mothers who were not exposed.
