Improving medicines reconciliation rates at Ashford and St. Peter's Hospitals NHS Foundation Trust.

Medicines reconciliation is integral to patient safety, symptom control and reducing patient anxiety. During a 3-month period on the respiratory ward at St. Peter's Hospital, 54% of drug charts were not reconciled with pre-admission medicines at the point of discharge for admissions up to 17 days. Only 18% were reconciled within 24 hours of admission. 50% of drug charts were missing 0-2 pre-admission medicines and 50% were missing 3-5 pre-admission medicines. The most common medicines that were not reconciled included topical applications which included eye, ear, nasal and skin applications (1

Black Elderberry Exposure in Apparent Idiopathic Mild Acute Pancreatitis: A Case Report.

Drug-induced acute pancreatitis is uncommon and remains difficult to confirm, particularly when the suspected exposure is an herbal or over-the-counter supplement. Black elderberry, derived from Sambucus nigra, is widely marketed for immune support, but published literature linking it to acute pancreatitis is limited. We report a 75-year-old woman with hypertension, hyperlipidemia, and prior cholecystectomy who presented with abrupt severe epigastric pain radiating to the back, diaphoresis, and lipase 3016 U/L. Computed tomography of the abdomen and pelvis on day 0 showed a normal-appearing pa

A Prediction Model to Identify Clinically Relevant Medication Discrepancies at the Emergency Department (MED-REC Predictor): Development and Validation Study.

BACKGROUND: Many patients do not receive a comprehensive medication reconciliation, mostly owing to limited resources. We hence need an approach to identify those patients at the emergency department (ED) who are at increased risk for clinically relevant discrepancies. OBJECTIVE: The aim of our study was to develop and externally validate a prediction model to identify patients at risk for at least 1 clinically relevant medication discrepancy upon ED presentation. METHODS: A prospective, multicenter, observational study was conducted at the University Hospitals Leuven and General Hospital Si

Cardiac Rehabilitation to Optimize Medication Regimens in Heart Failure.

Cardiac rehabilitation (CR) is an inherently patient-centered program that provides holistic care to adults with cardiovascular conditions to promote lifelong health and fitness, facilitate self-care and self-efficacy, and improve clinical outcomes. CR offers an excellent platform for patient-centered optimization of medication regimens for older adults with heart failure through its potential to address several aspects of care that have historically served as major challenges to clinicians-diuretic management, the use of guideline-directed medical therapy, review and reconciliation of noncard

Opioid exit plan: A pharmacist's role in managing acute postoperative pain.

OBJECTIVES: The benefits of a pharmacist's involvement in medication reconciliation and discharge counseling are well documented in the literature as improving patient outcomes. In contrast, no studies have focused on the initiation of a pharmacist-led opioid exit plan (OEP) for acute postoperative pain management. This paper summarizes a pharmacist-led OEP practice model and the potential role that pharmacists and student pharmacists can have at the point of admission, during postoperative recovery, and on discharge in acute pain management patients. SETTING: The pain management team at St.

Medication sleuth: an important role for pharmacists in determining the etiology of delirium.

Delirium is characterized by disturbances of consciousness, attention, cognition, and perception and is the most common reason for acute cognitive dysfunction in hospitalized elderly patients. Causes of delirium can be multifactorial, and a careful medical and medication history can help determine the underlying cause of behavioral disturbances. A 65-year-old patient with a history of chronic pain, insomnia, and multiple medical problems, who presented with altered mental status and aggressive behavior, is described. The patient had taken an overdose of zolpidem prior to admission, and she req

Out of sight: a lesson in drug errors.

A 76-year-old man developed recurrent encephalopathy, visual disturbance, myoclonus, generalised seizures and atonic drop attacks on a background of a gastrectomy for adenocarcinoma and stable chronic lymphocytic leukaemia. He presented to three different hospitals and was admitted twice, with normal investigations. His symptoms transiently improved during each admission (and with starting levetiracetam) but recurred each time on hospital discharge. Subsequent careful inspection of his medication box identified that his community pharmacy had in error been dispensing baclofen 80 mg per day ins

Systematic stroke prevention.

PURPOSE OF REVIEW: : This review summarizes the systematic approaches that can be used to optimize secondary stroke prevention. Systematic secondary stroke prevention involves not only prescribing stroke patients the appropriate medications to manage risk factors, but also optimizing the effectiveness of those drugs by focusing on medication adherence. Medication adherence is defined as the extent to which patients take their medications as prescribed by their providers. RECENT FINDINGS: : Many potential barriers to adherence exist, including relationships among patient, provider, and the hea

Pregabalin Toxicity in Kidney Failure: A Medication Error With Neurological Consequences.

We present a case of pregabalin toxicity in a 50-year-old dialysis-dependent female, who presented with a one-day history of progressive weakness, lethargy, and gait disturbance. They were successfully treated with haemodialysis. The case is notable for the atypical toxidrome seen in kidney failure, which occurred after a community pharmacy error led to a single twelvefold-increased dose. It also demonstrates the role of the emergency department pharmacist, who was crucial in identifying the medication error. The report emphasises the importance of careful medication reconciliation, especially

Pharmacists are initiators in palliative care for patients with rare diseases.

The World Health Organization supports early delivery of palliative care as it reduces unnecessary hospital admissions and the inappropriate use of health care services. A community pharmacist can play a key role in advocating timely access to palliative care. Medication reconciliation must alert them to start communicating with the patient and/or his relatives about refocusing treatment and care as part of palliative and terminal care. Pharmaceutical activities for these patients include dispensing of devices and medicinal products, compounding personalized medication and participating as a m

Severe Lactic Acidosis Due to Inappropriate Use of Biktarvy in a Patient With AIDS.

Bictegravir-emtricitabine-tenofovir alafenamide is an approved medication for the treatment of acquired immunodeficiency syndrome (AIDS). This medication, also called Biktarvy, includes an integrase strand inhibitor combined with nucleoside reverse transcriptase inhibitors (NRTIs) to prevent viral DNA synthesis and lead to improvements in disease progression and mortality in patients with AIDS. A rare but previously documented adverse effect of NRTIs present in Biktarvy is lactic acidosis. NRTIs can cause lactic acidosis through mitochondrial impairment, as mitochondria depend on DNA polymeras

Evaluation of the role and usefulness of clinical pharmacists at the Fangcang Hospital during COVID-19 outbreak.

BACKGROUND: Fangcang Hospitals (cabin hospitals) played a key role in isolation and control of the infection sources during COVID-19 epidemic. Many patients at Fangcang Hospitals had complications or mental stress. As the doctors, nurses and paramedics presented in the emergency, there was a growing demand for clinical pharmacists to provide pharmaceutical care for the affected patients with chronic diseases via telemedicine. OBJECTIVE: This study was a retrospective study to evaluate the usefulness of clinical prevention and control measures of clinical pharmacists at Jianghan Fangcang Hospi

Possible Bupivacaine Induced Hepatitis Following Wide Awake, Local Anesthesia, No Tourniquet Carpal Tunnel Surgery: A Case Report.

In this case report, we present a novel occurrence of acute hepatitis 2 weeks after local bupivacaine injection for wide awake, local anesthesia, no tourniquet carpal tunnel release. Laboratory and biopsy analysis confirmed cholestatic, drug-induced hepatitis that was successfully managed with conservative treatment. With a paucity of potential bupivacaine-induced hepatitis cases reported within the literature, the importance of broad differential diagnosis, meticulous medication reconciliation, and consideration of this rare complication should not be understated by the astute hand surgeon.

Factors influencing the anticholinergic burden of medical prescriptions for older patients hospitalized for delirium.

BACKGROUND: Delirium is a frequent cause of admission to the emergency department (ED) for older patients. Iatrogenic morbidity, particularly the hidden anticholinergic effects of many treatments, plays a significant role. The clinical impact of AB reduction on early rehospitalization remains unclear. However, reducing the anticholinergic burden (AB) of medical prescriptions (MP) could be beneficial for patients. OBJECTIVE: This study primarily aimed to investigate whether reducing AB during hospitalization influences early rehospitalization rates (within 30 days) in older patients admitted fo

A new medication-based prediction score for postoperative delirium in surgical patients: Development and proof of feasibility in a retrospective patient cohort.

Structured risk screening for postoperative delirium (POD) considering prehospital medication is not established. We aimed to develop a POD-risk prediction score based on known risk factors and delirium-risk increasing drugs to be used by pharmacists during medication reconciliation at hospital admission, and to test for feasibility in a retrospective cohort of surgical patients. Therefore, established POD-risk factors and drugs were extracted from the literature and a score was generated. Following this, the score was tested for feasibility in a retrospective 3-month-cohort of surgical patien

Medication Safety Strategies for Geriatric Patients in the Emergency Department: A Narrative Review.

Population aging is reshaping emergency care. Older adults already account for 15%-25% of U.S. emergency department (ED) visits and, compared with younger patients, present with greater numbers of comorbidities, higher resource utilization, longer stays, and markedly higher admission rates (35%-45%), return visits, and mortality. Medication exposure is a central driver of risk with an estimated ~10% of older-adult ED visits being medication-related. We conducted a narrative review of the literature surrounding medication safety for geriatric patients presenting to the ED and strategies that ca

Virtual clinical pharmacy services: A model of care to improve medication safety in rural and remote Australian health services.

PURPOSE: To describe a virtual clinical pharmacy service as a model of care to support rural and remote Australian hospitals that otherwise would not have access to onsite pharmacists. SUMMARY: Many small hospitals in Australia do not have an onsite hospital pharmacist and struggle to support and optimize patient care. To increase access to a hospital pharmacist's specialized skills and medication knowledge, a virtual clinical pharmacy service was designed and implemented in 8 hospitals across rural New South Wales, Australia in 2020. The virtual clinical pharmacy service focuses on the core

The Importance of Electronic Health Record Access For Accurately Completing Drug Regimen Reviews.

OBJECTIVE: To challenge the standard of practice by evaluating the identification of medication discrepancies found depending on type of access to an electronic health record (EHR). In other words, is there a difference in the number of discrepancies between a pharmacist with only access to the postacute long-term care (PALTC) EHR (ie, single-access pharmacist [SAP]) compared with a pharmacist with access to both the PALTC and hospital EHRs (ie, dual-access pharmacist DAP) In October 2018, the Improving Medicare PostAcute Care Transformation (IMPACT) Act mandated admission drug review (DRR) up

Phentermine-Associated Atrial Fibrillation: A Case Report and Literature Review.

UNLABELLED: Obesity has become a major public health problem with increased prevalence and is associated with cardiovascular mortality. Phentermine is approved for short-term obesity treatment in conjunction with lifestyle modifications. Palpitations are a well-documented side effect of phentermine, but atrial fibrillation (AF) is rarely reported. We present a case of new-onset AF in a healthy woman who had been recently started on phentermine for weight loss. LEARNING POINTS: Atrial fibrillation usually occurs secondary to intrinsic disorders such as hypertension, coronary artery disease, ab
