OBJECTIVE: Traumatic meniscal injuries can cause acute pain, hemarthrosis (bleeding into the joint), joint immobility, and post-traumatic osteoarthritis (PTOA). However, the exact mechanism(s) by which PTOA develops following meniscal injuries is unknown. Since meniscus tears commonly coincide with hemarthrosis, investigating the direct effects of blood and its constituents on meniscus tissue is warranted. The goal of this study was to determine the direct effects of blood and blood components on meniscus tissue catabolism.
METHODS: Porcine meniscus explants or primary meniscus cells were exp

Injuries to the avascular region of the knee meniscus-the inner "white-white" zone-pose a significant therapeutic challenge due to its lack of vascular supply and limited intrinsic healing capacity. While conventional meniscal repair techniques often fail in this region, recent advances in regenerative medicine have turned to explant culture systems as a bridge between simple cell cultures and complex in vivo models. This review synthesizes the current literature on the role of explant cultures in understanding and advancing avascular meniscus repair. Explant cultures maintain native extracell

PURPOSE: The assisted inferior anterolateral portal under anterior horn of the lateral meniscus (UAHLM portal) was applied to treat the lateral discoid meniscus injury conveniently and the clinical outcomes were evaluated.
METHODS: A retrospective review was conducted on 60 patients who underwent arthroscopic surgery with a symptomatic discoid lateral meniscus. Normal anterolateral/anteromedial portals assisted with UAHLM portal (1-2 cm inferior to the anterolateral portal) were used. All patients were followed up for 24-48 months (median, 33 months) and evaluated by MRI images and clinical o

The root of the meniscus, as a core structure maintaining the biomechanical stability of the knee joint, plays a decisive role in delaying the occurrence of osteoarthritis by restricting abnormal displacement of the meniscus and ensuring the integrity of circumferential tension. Although various repair techniques for posterior root tears of the medial meniscus have achieved the goal of anatomical reconstruction, postoperative meniscus extrusion is still common. This pathological displacement is significantly associated with joint space narrowing and osteophyte formation. This technique describ

BACKGROUND: Medial meniscus posterior root tears (MMPRTs) lead to extrusion of the meniscus during weightbearing as well as loss of the ability of the meniscus to generate hoop stress. This loss of load-sharing ability leads to progressive arthritic changes. However, there have been no studies that correlate the correction of meniscus extrusion with clinical outcomes.
HYPOTHESIS: Decreased meniscus extrusion is associated with better clinical and radiographic outcomes compared with increased meniscus extrusion after MMPRT pullout fixation.
STUDY DESIGN: Case-control study; Level of evidence,

Discoid lateral meniscus (DLM) is a variant of meniscal histomorphology that often presents in young, active pediatric populations. Patients may present with mechanical symptoms, joint line pain, swelling, and loss of motion especially with lack of terminal knee extension. DLM are prone to tearing, most frequently with horizontal patterns in children and complex patterns in adults. The PRiSM classification and assessment of DLM provide a comprehensive approach to evaluating DLMs arthroscopically, focusing on the following four factors: (1) meniscal width, (2) meniscal height, (3) stability, an

PURPOSE OF REVIEW: The purpose of this review is to synthesize current science on meniscus anatomy and biomechanics and repair techniques to create an empirical foundation for postoperative rehabilitation precautions and guidelines, including timelines, clinical and performance-based criteria for return to activity, to maximize both meniscal healing potential and patient recovery.
RECENT FINDINGS: Recent literature has focused on meniscus repair rather than debridement, and rehabilitation protocols should be designed to optimize healing. Complex, unstable tears, like root and radial tears, di

Meniscus tears are a common orthopedic pathology and planning a single, effective treatment is challenging. The diagnosis of meniscal tears requires detailed history-taking, physical examinations, special diagnostic tests, and most likely magnetic resonance imaging (MRI) to confirm the lesion. A good understanding of the meniscal structure including vascularity, zones, function, and affected movements with associated symptoms plays a crucial role in establishing an optimal management plan. A careful assessment of the patient's characteristics, comorbidities, post-repair rehabilitation, and pat

Medial meniscus posterior root tears are common injuries, especially in the Asian world. This injury must be repaired where indicated to restore knee biomechanics and prevent arthritis. Suturing the meniscus using suture tapes provides good hold of the tissue. The use of a 70° arthroscope and creation of a transseptal portal improve visualization of the posterior compartment. Creation of a high posteromedial portal achieves the correct trajectory for the suture anchor insertion. A knotless suture anchor can be used to fix the posterior root at its anatomic attachment site. This obliterates the

Meniscus tears are commonly encountered in the athletic population and can result in significant loss of playing time. Current treatment methods for acute tears consist of meniscectomy and meniscal repair, whereas meniscal allograft transplant is reserved as a salvage procedure for symptomatic meniscectomized patients who desire a more functional knee. This review describes the postoperative rehabilitation protocol for each procedure and evaluates the outcomes in existing literature as it pertains to the athlete.

Although many meniscus tears can be successfully repaired, not all are salvageable, especially if considerable tissue damage has occurred. Meniscus transplantation has been shown to be an acceptable procedure for younger patients. The primary candidate is a patient younger than age 50 years who has had a total meniscectomy and who either has pain in the tibiofemoral compartment, arthroscopic evidence of articular cartilage deterioration, or both. Contraindications are advanced knee joint arthrosis with flattening of the femoral condyle, concavity of the tibial plateau, and osteophytes that pre

Reports of ring menisci are rare. This article describes a case of a congenital lateral ring meniscus and includes a literature review. The anatomy of the menisci of various vertebrates is described with an attempt to discuss the possible origin of the anomaly. Although ring menisci are asymptomatic, they are clinically significant in that they need to be differentiated from old bucket-handle tears. It is also easy to fail to visualize the intercondylar bridge between the two "horns" with conventional magnetic resonance imaging.

Posterior medial meniscal root tears and meniscal extrusion have come under increasing focus in recent years, with management that prevents or delays advancing degenerative changes in the medial compartment being the ultimate goal. Although meniscal root repair can restore meniscal function and slow degeneration, it does not eliminate osteoarthritis progression, with conversion to total knee arthroplasty reported as high as 33%. Augmenting posterior medial meniscal root repair with centralization techniques appears to reduce short-term postoperative extrusion and may mitigate early radiographi

BACKGROUND: The knee meniscus has limited intrinsic repair and regenerative capacity. Safe and effective biotherapeutic products remain lacking, and enhancing cell migration and angiogenesis remains a major challenge. This study aimed to develop a novel self-assembled meniscus-specific extracellular matrix membrane (saECM-M) and evaluate its performance in scaffold-free meniscus repair and scaffold-guided whole-meniscus reconstruction, in comparison with chemically processed decellularized ECM (cpd-ECM).
METHODS: Meniscal fibrochondrocytes (MFCs) were cultured under membrane-inducing conditio

PURPOSE OF REVIEW: The management of horizontal cleavage meniscus tears (HCT) has evolved in recent years. Historically, HCTs have been treated with partial meniscectomy, but multiple studies have shown the long-term risks of development of arthritic changes in the knee following meniscectomy. These findings have renewed interest in meniscal preservation whenever possible, even in older patients not previously considered for meniscus repair. This article provides an overview of current management of horizontal cleavage meniscus tears and describes modern surgical techniques for repair of HCTs.

Meniscus injury has been one of the most common knee injuries in current society. The research on artificial meniscus implants as substitutes in meniscus reconstruction therapy has become global focus in order to solve clinical problems such as irreparable meniscus injury and symptoms after full or partial meniscectomy. At present, researches on artificial meniscus implants mainly focus on biodegradable meniscus scaffolds and non-biodegradable meniscus substitutes. Although the commercialized meniscal implants, such as CMI ®, Actifit ® and NUsurface ®, have been applied in the clinical, none o
