Treatment of Freiberg's disease using a reverse pedicled metatarsal bone flap: Case report.

Despite no surgical procedures receiving unanimous support for treating Freiberg's disease, several surgical treatment options have been described. For the past few years, bone flaps have been shown in children to present promising regenerative properties. We report a novel technique using a reverse pedicled metatarsal bone flap from the first metatarsal to treat one case of Freiberg's disease in a 13-year-old female. The patient presented 100% involvement of the second metatarsal head, with a 6 × 2 mm defect, unresponsive to 16 months of conservative measures. A 7 mm × 3 mm pedicled metatarsa

Avascular necrosis of the proximal epiphysis of the first metatarsal bone in childhood. Long-term outcome.

Ten-year old boy with an 8-month history of limping for pain in the dorsomedial region of the right midfoot. At examination, there were signs of local swelling and tenderness to palpation, as well as antalgic gait with internal rotation. X-ray results showed widening of the proximal epiphysis of the first metatarsal. One month later, local fragmentation with hypodense and sclerotic areas was observed. MRI showed fragmentation, sclerosis, and collapse in the proximal epiphysis consistent with avascular necrosis of the proximal epiphysis of the first metatarsal bone. Patient was only recommended

A Narrative Review of Metatarsal Bone Stress Injury in Athletic Populations: Etiology, Biomechanics, and Management.

Metatarsal bone stress injuries (BSIs) are common in athletic populations. BSIs are overuse injuries that result from an accumulation of microdamage that exceeds bone remodeling. Risk for metatarsal BSI is multifactorial and includes factors related to anatomy, biology, and biomechanics. In this article, anatomic factors including foot type, metatarsal length, bone density, bone geometry, and intrinsic muscle strength, which each influence how the foot responds to load, are discussed. Biologic factors such as low energy availability and impaired bone metabolism influence the quality of the bon

Osteoid osteoma of the second metatarsal bone diagnosed on MRI: A case report and literature review.

Osteoid osteoma is a relatively common benign bone tumor that is most frequently seen on the appendicular skeleton in adolescents and young adults. Here we present the case of a 14-year-old boy presenting with a 10 month history of pain in his left foot which had been misdiagnosed as stress fracture due to its unusual clinical presentation. Magnetic resonance imaging of the left foot revealed a bone lesion with typical features of the osteoid osteoma on the distal part of the second metatarsal bone. The lesion was surgically removed and the diagnosis of osteoid osteoma was confirmed by post-su

The Fifth Metatarsal Bone Fracture in Athletes - Modalities of Treatment Related to Agility in Soccer Players.

The 5th metatarsal fracture is a common foot fracture which could exclude a player from competition for several months and significantly affect his or her career. This manuscript presents the treatment and rehabilitation of professional soccer players who had acute fractures of the 5th metatarsal bone and a cannulated screw fixation. The main purpose of the analysis was to determine the minimum time necessary for a permanent return to the sport after a 5th metatarsal fracture among professional soccer players. We followed the surgical and rehabilitation path of 21 professional soccer players f

Longitudinal Epiphyseal Bracket of the First Metatarsal Bone: Three Case Reports and a Review of the Literature.

Longitudinal epiphyseal bracket (LEB) is a rare bone dysplasia of the tubular bones. Owing to an abnormal secondary ossification center, the affected bones can develop progressive shortening and angular deformity. The aim of our study was to provide an overview of the reported data regarding epidemiology and surgical procedures available for LEB of the first metatarsal bone in a pediatric population combined with a small case series. We report a retrospective case series of 3 nonsyndromic pediatric patients with different ages and with confirmed dysplasia of the first metatarsal bone. All pati

Proximal first metatarsal opening wedge osteotomy: geometric analysis on saw bone models.

BACKGROUND: For hallux valgus correction, distal first metatarsal osteotomy is generally used for minor to moderate deformities, diaphyseal osteotomy for moderate deformities and basal osteotomy or arthrodesis for severe deformities. With the advent of locking plates, there has been renewed interest in opening wedge basal osteotomy. OBJECTIVE: We undertook this study in order to understand the power and limitations of this osteotomy. METHOD: Proximal opening wedge osteotomies were performed on saw bone models in four orientations, with three different wedge sizes: (1) perpendicular to the gr

Reconstruction of metatarsal bone defects with a free fibular osteomyocutaneous flap incorporating soleus muscle.

Severe traumatic bone and soft-tissue defects are often treated by lower leg amputation. The amputation level becomes a very important factor with respect to the patient's basic daily activities. We report the case of a 51-year-old man who was referred to us with severe traumatic metatarsal bone and dorsum pedis skin and soft-tissue defects. To avoid amputation, a free fibular osteomyocutaneous flap incorporating the soleus muscle was used to reconstruct the second and third metatarsal bones and the soft-tissue defect, respectively. Now, 2 years after the procedure, the patient is able to walk

Surgical repair of propagating condylar fractures of the third metacarpal/metatarsal bones with cortical screws placed in lag fashion in 26 racehorses (2007-2015).

BACKGROUND: Despite the recommendation of plate fixation for propagating condylar fractures of the third metacarpal (McIII) or third metatarsal bone (MtIII), lag screw fixation can be a viable surgical option. OBJECTIVES: To evaluate short-term outcome and long-term racing performance of horses that underwent lag screw fixation of long condylar fractures of the McIII/MtIII. STUDY DESIGN: Retrospective case series. METHODS: Medical records, post-surgical racing performance and outcome of 26 horses with propagating fractures of the medial and/or lateral condyle of McIII/MtIII were reviewed. M

Metatarsal coalition with pseudarthrosis mimicking bone tumor: two case reports and literature review.

BACKGROUND: Metatarsal coalition is an extremely rare congenital anomaly. Its complication with pseudarthrosis is even more exceptional and may present as a palpable dorsal mass or chronic pain, often mimicking a bone tumor. Awareness of this condition is essential for accurate diagnosis and to avoid unnecessary biopsies or overtreatment. CASE PRESENTATION: We report two cases of metatarsal coalition complicated by pseudarthrosis. The first case involved a 25-year-old woman with a 7-year history of a mildly painful dorsal foot mass. Imaging revealed a coalition between the first and second met

Surgical stabilisation versus external coaptation for treatment of metacarpal/metatarsal bone fractures in dogs.

PICO QUESTION: In dogs with metacarpal and metatarsal fractures, does open reduction and surgical stabilisation compared to closed reduction and external coaptation (non-surgical stabilisation) lead to an improved likelihood of bone union and resolution of lameness? CATEGORY OF RESEARCH: Treatment. NUMBER AND TYPE OF STUDY DESIGNS REVIEWED: Five retrospective studies that directly compared surgical intervention to conservative management of metabone fractures were critically reviewed. STRENGTH OF EVIDENCE: Weak. OUTCOMES REPORTED: External coaptation may lead to successful clinical outcome

Ultrasound Findings of a Brown Tumor in the Metatarsal Bone due to Primary Hyperparathyroidism: A Case Report.

Brown tumor is a benign, nonneoplastic lesion resulting from hyperparathyroidism, typically found in long bones, clavicles, ribs, pelvis, and the mandible. We present a case of brown tumor secondary to primary hyperparathyroidism, with the initial presentation in the right metatarsal bone. We then describe and summarize the characteristic ultrasound features of brown tumors observed in this case. Ultrasound can detect characteristic bone destruction and typical cystic formations in the diagnosis of brown tumors, providing valuable information for the diagnosis of this condition.

Metatarsal Aneurysmal Bone Cysts Treated With En Bloc Resection and Reconstruction With Fibular Allograft.

BACKGROUND: Treatment of Primary metatarsal aneurysmal bone cyst (ABC) with curettage and bone grafting unfortunately has a high recurrence rate, particularly in short tubular bones. This study presents a 16-year experience treating ABCs in the bones of the foot at an orthopaedic oncology referral center. Treatment involved en bloc resection and reconstruction of the defect with fibular allograft in all cases. Retrospectively collected data were used to document the outcomes. METHODS: This retrospective review includes patients with primary metatarsal ABC treated en bloc resection at a single

Is Use of Bone Cement for Treatment of Second Metatarsal Stress Fractures Safe? A Case Report.

Metatarsal stress fractures are common injuries of the foot and can be a source of chronic pain without appropriate management. Conservative management is first line, but surgery may be indicated in athletes, cases of nonunion, and fractures of the fifth metatarsal. We report a case of a 34-year-old female who presented to clinic for intractable pain of the left foot secondary to a stress fracture of the left second metatarsal, which had been previously treated with injectable acrylic bone cement. Calcium sulfate hydroxyapatite cement has a multitude of applications in orthopedic surgery, but

Effect of Weight-Bearing in Conservative and Operative Management of Fractures of the Base of the Fifth Metatarsal Bone.

BACKGROUND: There is no established principle regarding weight-bearing in conservative and operative management of fifth metatarsal base fractures. METHODS: We reviewed 86 patients with acute fifth metatarsal base fractures. Conservatively treated late or early weight-bearing patients were assigned to Group A or C, respectively. Operatively treated late or early weight-bearing patients were assigned to Group B or D, respectively. Results were evaluated by clinical union, bone resorption, and the American Orthopaedic Foot and Ankle Society (AOFAS) and Visual Analogue Scale (VAS) scores. RESUL

Fixation of split anterior tibialis tendon transfer by anchorage to the base of the 5th metatarsal bone.

Split anterior tibialis transfer is a procedure, frequently performed in combination with triceps surae lengthening, to treat equinovarus foot deformity in adult hemiplegia patients. The authors present their surgical technique, with tendon fixation by an anchor to the base of the fifth metatarsal. Although bone-tendon anchor fixation is widespread in orthopedic surgery, it is original in this type of indication. It is simpler, and less traumatic for the bone than a bone tunnel and less traumatic for the sole than transplantar fixation. The very distal attachment to the base of the 5th metatar

Application of bone graft to the medial side of the first metatarsal head in the treatment of hallux varus.

BACKGROUND: Hallux varus deformity is not frequent, is usually acquired, and is poorly tolerated by patients. A common cause is the resection of an excessive amount of the head of the first metatarsal during an operation performed to correct a hallux valgus deformity. The purpose of this study was to evaluate the results of application of bone graft to the medial aspect of the first metatarsal head in order to restore missing bone after resection of an excessive amount of bone during a bunionectomy. METHODS: Of thirty patients who had a hallux varus deformity that was treated operatively, eig

The treatment of monostotic fibrous dysplasia of the first metatarsal with free vascularized fibular bone graft.

Historically, patients suffering from fibrous dysplasia of the metatarsals faced amputation or resection as the treatments of choice. Although the lesion itself is not malignant, pain, swelling, and disability dictated the treatment. With recent refinements in microvascular surgery, vascularized bone grafting has offered a viable alternative for this and similar conditions that offers the potential for a functional, pain-free foot.

Circulatory disturbance of the first metatarsal head after Chevron osteotomy as shown by bone scintigraphy.

Distal osteotomies, such as Chevron osteotomies, have a potentially high risk for circulatory disturbance, since they transect part of the circulatory apparatus. An increased risk of up to 40% of avascular necrosis diagnosed radiographically has been reported when the osteotomy is combined with adductor tenotomy. On the other hand, new circulatory studies indicate that the circulation does not go in direct proximity to the adductor tendon. In this prospective study, 38 consecutive patients (41 feet) were randomized to Chevron osteotomy alone or Chevron osteotomy with adductor tenotomy. They we
