We present a case of an 18-month-old boy who showed severe clinical signs indicative of acute hemorrhagic edema of infancy (AHEI) with painful purpuric skin affection primarily of the face and marked edema of the ears. The histological findings were diagnostic for leukocytoclastic vasculitis and thus met the histological criteria for AHEI. Glucocorticoids (GLs) are the sole therapeutic approach in polymyalgia rheumatica (PMR). An undefined proportion of patients respond only to very high doses of GLs and some seem to respond better to methylprednisolone (MPONE) than to prednisone (PN) or vice versa. Five patients with Wegener's granulomatosis were treated with high-dose intravenous methylprednisolone (MTP) pulse therapy. Eight hospitalized patients with active bullous pemphigoid were treated with high-dose intravenous methylprednisolone pulse therapy followed by moderate maintenance doses of oral prednisone. Blistering decreased within 24 hours of the first pulse dose in seven of the eight patients. When blistering recurred, it was less severe than it had been before pulse therapy was given. Complications of coronary artery surgery were analyzed in a prospective controlled study of 150 patients, one group receiving methylprednisolone before temporary cardiopulmonary bypass. The patient population was comparable in both the groups. BACKGROUND: Topical corticosteroids are essential for the management of inflammatory skin diseases but optimizing efficacy whilst minimizing adverse effects remains critical.
