Optical coherence tomography angiography characteristics in diabetic patients without clinical diabetic retinopathy.

PURPOSE: The purpose of this study is to measure optical coherence tomography angiography (OCTA)-defined retinal vascular and choriocapillaris parameters in diabetic patients without clinically evident diabetic retinopathy. METHODS: Retrospective case series of eyes in patients with a documented history of either type 1 or type 2 diabetes mellitus without any clinical signs of diabetic retinopathy seen at the Vanderbilt Eye Institute. All eyes underwent dilated funduscopic examination along with OCTA. OCTA analytics were performed to calculate vessel density, flow area, and foveal avascular z

A large coronary aneurysm and its probable precursor lesions in a patient with autosomal dominant polycystic kidney disease: an implication for the process of aneurysmogenesis.

Coronary artery aneurysms are rare complications of autosomal dominant polycystic kidney disease (ADPKD), and their pathogenesis remains poorly understood. We report an autopsy case of a 64-year-old ADPKD patient with an asymptomatic, large (4 cm in diameter) saccular aneurysm arising from the left circumflex (LCX) branch of the coronary artery with only mild atherosclerotic changes. Autopsy also revealed small, focal defects of media with or without microaneurysm formation in the LCX, mesenteric and renal arteries, and a fibromuscular dysplasia-like lesion with microaneurysm in the common ili

[Simultaneous, bilateral hypertensive intracranial hematomas].

The reported incidences of bilateral intracerebral hemorrhages due to systemic arterial hypertension are exceptionally rare in Japan. Unilateral hemorrhages, on the other hand, are less uncommon. Recently, we have examined two patients with bilateral intracerebral hemorrhages due to hypertension. The first case involved bilateral thalamic hemorrhages; and in the other, a contralateral hemorrhage developed postoperatively, subsequent to the evacuation of a primary hematoma. The characteristic neurological manifestation of bilateral intracerebral hemorrhages include quadriparesis, bilateral Babi

Acute renal infarction caused by Behçet's disease.

A case of acute renal infarction due to Behçet's disease is described. Selective renal arteriography revealed multiple microaneurysms of both kidneys and arterial obstruction distal to an aneurysm in the left kidney. Bilateral microaneurysms were no longer seen on the follow-up angiogram 5 years later. Behçet's disease should be considered in the differential diagnosis of acute renal infarction in young adults.

Developing an intelligent model to detect the level of diabetic retinopathy using blood vessel pattern extraction in retinal images.

Diabetic retinopathy is a chronic and progressive eye disease in which the human retina is affected by an increase in the amount of insulin in the blood. Diabetic retinopathy, if not detected and treated in time, threatens the patient's vision and eventually causes complete blindness. Among various clinical symptoms, microaneurysm appears as the first sign of diabetic retinopathy. Accurate and reliable detection of microaneurysms is a challenging problem due to its small size and low contrast. The successful detection of microaneurysms will be more useful for the proper treatment of the diseas

Polyarteritis nodosa presenting as perirenal hemorrhage.

Though renal involvement with microaneurysm formation is common in polyarteritis nodosa (PAN), spontaneous perirenal hematoma (SPH) is rare. We report a patient in whom SPH was the presenting manifestation of PAN. SPH is a potential life-threatening complication of PAN and its early recognition is emphasized.

Suspended scattering particles in motion using OCT angiography in branch retinal vein occlusion disease cases with cystoid macular edema.

We aimed to investigate the clinical implication of suspended scattering particles in motion (SSPiM) using optical coherence tomography angiography (OCTA) among branch retinal vein occlusion disease (BRVO) cases with macular edema (ME). Medical records of BRVO patients were reviewed. Central retinal thickness (CRT), ME type, and cyst size on optical coherence tomography images were evaluated before and after intravitreal bevacizumab injection. Nonperfusion area, SSPiM, and microvascular abnormalities in OCTA images were evaluated using a Heidelberg machine. SSPiM was identified in 24 of 56 cas

Neurofibromatosis type 1 first presented as hypertensive crisis.

Neurofibromatosis type 1 (NF1) is known to cause vascular complications including stenotic and aneurysmal disease. Here, we report a case of a woman in her early 20s, who presented with unilateral facial nerve palsy and hypertensive crisis, and was later found to have multiple bilateral intrarenal microaneurysms along with classic cutaneous manifestations of NF1. A causal relationship between the pathophysiology of NF1 and the development of renal artery microaneurysm and hypertension is proposed in this case report.

Multimodal imaging biomarkers associated with recurrence of diabetic macular edema during aflibercept treat-and-extend therapy.

This prospective, non-randomized study aimed to identify multimodal imaging biomarkers predictive of treatment response in diabetic macular edema (DME) patients undergoing intravitreal aflibercept therapy using a treat-and-extend (T&E) regimen. Twenty-eight eyes with center-involved DME received five monthly aflibercept injections followed by an 18-month T&E phase. Imaging biomarkers included hyperreflective foci (HRF) and choroidal vascular index (CVI) on OCT, leakage area and microaneurysm count on fluorescein angiography (FA), and intraretinal microvascular abnormalities (IRMA) on OCT angio

First microangiographic abnormalities in childhood diabetes--types of lesions.

An analysis by fluorescein angiography of the first signs of retinopathy in 161 diabetic children showed that microaneurysm-like spot dilatations, microaneurysms, focal capillary dilatations, leakage, generalized capillary dilatations, retinal hemorrhages, areas of capillary non-perfusion and capillary remodelling, in decreasing order of frequency, were features of the onset of retinopathy. However, microaneurysm-like spot dilatations and both focal and generalized capillary dilatations were considered to be too subjective for use in further quantitative analysis. Retinopathy was not found in

Endovascular management of ruptured choroidal aneurysms associated with moyamoya syndrome: A two-patient case report & review of the literature.

Ruptured choroidal microaneurysms associated with moyamoya syndrome are rare entities. Their small size and deep/distal location make them challenging to treat. Both surgical and endovascular treatment options carry significant risk of stroke. Here we present a two-patient case report of ruptured choroidal microaneurysms treated with Nbutyl cyanoacrylate (nBCA) endovascular embolization utilizing neuromonitoring and provocative testing during one of the cases. We also present a review of the literature evaluating surgical and endovascular treatment of these aneurysms.

Delayed progression of diabetic retinopathy by divided insulin administration: a further follow-up.

We have previously reported a slower rate of progression in the number of microaneurysms in insulin dependent diabetic patients treated by divided insulin injections when compared to patients receiving a single daily insulin injection [Diabetes 25, 463--469 (1976)]. This observation is strengthened by the present analysis which uses complementary data obtained after a further year follow up: the mean yearly increase in the number of microaneurysms was 9 +/- 1 in the single injection group and 3 +/- 1 in the multiple one (p less than 0.001).

Polyarteritis nodosa associated with necrosis of the tongue: a rare presentation in an adult patient.

Polyarteritis nodosa is a rare form of vasculitis of the medium-sized vessels, which leads to the formation of microaneurysms, thrombosis, organ ischaemia, and necrosis. Involvement of the oral cavity is rare. To our knowledge, this is the first case of localised polyarteritis nodosa in an adult with ulceration of the tongue and necrosis at the onset of the disease. The patient made a full recovery after the use of strong immunosuppressants.

Near-infrared spectrophotoscopy of finger venules in assessment of autonomic dysfunction.

The authors evaluated morphologic changes in the venules of the finger using near-infrared spectrophotoscopy in patients with autonomic dysfunction, such as familial amyloidotic polyneuropathy and multiple-system atrophy. Abnormalities of the venules, such as tortuosity, irregular venous caliber, and microaneurysm-like change, and a linear negative correlation between the degree of orthostatic hypotension and the degree of vasoconstriction of the venules were observed.
