Clinical Evaluation of Deployed Military Personnel With Chronic Respiratory Symptoms: Study of Active Duty Military for Pulmonary Disease Related to Environmental Deployment Exposures (STAMPEDE) III.

BACKGROUND: Chronic respiratory symptoms are frequently reported after Southwest Asia deployment in support of combat operations. The full spectrum of clinical lung diseases related to these deployments is not well characterized. METHODS: Military personnel with chronic symptoms, primarily exertional dyspnea, underwent a standardized cardiopulmonary evaluation at two tertiary medical centers. Pulmonary function testing consisted of spirometry, lung volume, diffusing capacity, impulse oscillometry, and bronchodilator testing. Further testing included methacholine challenge, exercise laryngosco

Attachment anxiety and trauma history uniquely and interactively predict adjustment during and following deployment among military partners.

The psychological impact of military deployment on nondeploying partners of service members is only recently gaining attention in the literature, with preliminary findings suggesting that partners of military service members experience significant mental health consequences of deployment, but with little work examining factors that could heighten or attenuate risk for maladjustment in response to deployment. The current study uses attachment theory as a guide to explore the unique and interactive effects of two factors likely to increase risk for maladjustment among nondeploying partners: atta

Norwegian male military veterans show low levels of mental health problems four years after deployment in Afghanistan.

BACKGROUND: Deployment of soldiers is associated with considerable rates of mental disorders after redeployment. AIMS: The aim of this study was to identity prevalence of probable mental disorders and associated factors among male Norwegian soldiers at a mean of 4 years following deployment to Afghanistan in 2001-2011. METHODS: The retrospective questionnaire-based study invited all Norwegian veterans deployed to Afghanistan from 2001-2011 to participate. The response rate was 59%, but only the 3403 men (47.5%) who completed all items of the four different screening instruments were used to

Peace and war: trajectories of posttraumatic stress disorder symptoms before, during, and after military deployment in Afghanistan.

In the study reported here, we examined posttraumatic stress disorder (PTSD) symptoms in 746 Danish soldiers measured on five occasions before, during, and after deployment to Afghanistan. Using latent class growth analysis, we identified six trajectories of change in PTSD symptoms. Two resilient trajectories had low levels across all five times, and a new-onset trajectory started low and showed a marked increase of PTSD symptoms. Three temporary-benefit trajectories, not previously described in the literature, showed decreases in PTSD symptoms during (or immediately after) deployment, followe

Replication and extension of the military family stress model: The after deployment adaptive parenting tools ADAPT4U study.

The military family stress (MFS) model conceptualizes that wartime deployments and post-traumatic stress disorder (PTSD) symptoms are associated with couple, parenting, and child adjustment difficulties. The aim of this study was to replicate and extend the military family stress model by examining the associations among deployment length, PTSD symptoms, marital functioning, parenting practices, and child adjustment in a replication sample of both National Guard and Reserve (NG/R) as well as active-duty service member families. The MFS model is extended to test whether these relationships vary

Associations between health-related behaviors and self-reported cognitive symptoms in U.S. military personnel injured on deployment.

Health behaviors may be core contributors to cognition and mental health following mild traumatic brain injury (TBI). The aims of the present study examined: (1) whether health behaviors including sleep duration, alcohol use, and physical activity differed in injured military personnel with and without deployment-related mild TBI history and (2) the relative contributions of health behaviors and deployment-related mild TBI history to self-reported cognitive, posttraumatic stress disorder (PTSD), and depressive symptoms. Participants included 3076 military personnel injured on deployment partic

Hair cortisol concentrations and cortisol stress reactivity predict PTSD symptom increase after trauma exposure during military deployment.

BACKGROUND: Previous evidence on endocrine risk markers for posttraumatic stress disorder (PTSD) has been inconclusive. Here, we report results of the first prospective study to investigate whether long-term hair cortisol levels and experimentally-induced cortisol stress reactivity are predictive of the development of PTSD symptomatology in response to trauma during military deployment. METHODS: Male soldiers were examined before deployment to Afghanistan and at a 12-month post-deployment follow-up using dimensional measures for psychopathological symptoms. The predictive value of baseline (i

Military occupation and deployment: descriptive epidemiology of active duty U.S. Army men evaluated for a disability discharge.

OBJECTIVE: Physically demanding jobs and history of deployment put Soldiers at increased risk for injury, hospitalizations, and disability. Characterizing differences in disability outcomes by occupation and deployment history may identify specific military populations for targeted prevention and intervention programs as well as potential areas of future research. METHODS: A cross-sectional analysis was conducted on U.S. Army enlisted men evaluated in the Department of Defense's Disability Evaluation System (DES) between fiscal years 2005 and 2011, comparing those assigned a Combat Arms milit

Role of Cancer Surgery in the Improvement of the Operative Skills of Military Surgeons During Deployment: A Single-Center Study.

INTRODUCTION: The maintenance of military surgeons' operative skills is challenging. Different and specific training strategies have been implemented in this context; however, little has been evaluated with regard to their effectiveness. Cancer surgery is a part of military surgeons' activities in their home hospitals. This study aimed to assess the role of oncological surgery in the improvement of military surgeons' operative skills. METHODS: Between January and June 2019, the surgical activities of the departments of visceral, ear, nose, and throat, urological, and thoracic surgery were ret

A prospective study on personality and the cortisol awakening response to predict posttraumatic stress symptoms in response to military deployment.

Few prospective studies on pre-trauma predictors for subsequent development of posttraumatic stress disorder (PTSD) have been conducted. In this study we prospectively investigated whether pre-deployment personality and the cortisol awakening response (CAR) predicted development of PTSD symptoms in response to military deployment. Furthermore, we hypothesized that potential effects of age, childhood trauma and previous deployment on development of PTSD symptoms were mediated via pre-deployment personality, CAR and PTSD symptoms. Path analysis was performed on data from 470 male soldiers collec

Establishment and evaluation of a smoking cessation programme on the prevalence and intensity of smoking during a 6-month military deployment.

INTRODUCTION: Military personnel smoke more frequently and intensely than civilians, with deployments often destabilising smoking behaviours and influencing long-term tobacco use. This study evaluated smoking prevalence and intensity at the start and end of a deployment among personnel with access to a smoking cessation service, identifying factors affecting cessation using the capability, opportunity, motivation, behaviour (COM-B) model. METHODS: During a 6-month deployment (January-June 2019), a smoking cessation programme was established and evaluated through a prospective longitudinal ser

Reunion After Deployment: Describing Findings From a Study of Military Couples Funded by the U.s. Department of Defense.

INTRODUCTION: The transition from deployment to reintegration can present unique stressors for returning service members and at-home partners. To better understand the relationship dynamics of military couples upon homecoming, we conducted a project funded by the U.S. Army Medical Research and Materiel Command (USAMRMC) Military Operational Medicine Research Program (MOMRP) to track returning service members and at-home partners across the transition. Our goal in this article is to describe findings from the project and lessons learned about collecting data from military couples upon reunion.

The contribution of prior psychological symptoms and combat exposure to post Iraq deployment mental health in the UK military.

This study assessed the contribution of baseline psychological symptoms, combat exposure, and unit support in the etiology of posttraumatic stress disorder (PTSD), and psychological distress. From 2004-2006, 67% of a random sample of 2,820 participants who had been assessed for psychological symptoms in 2002 were reassessed. Baseline psychological symptoms, combat exposure, and unit support factors were associated with the outcomes and the effect sizes for combat exposure were marked for PTSD symptoms. Adjustment for baseline psychological symptoms did not modify the pattern of association of

Development of Self-Directedness and Cooperativeness in Relation to Post-Traumatic Stress Disorder Symptom Trajectories After Military Deployment.

BACKGROUND: Personality traits, such as the character traits self-directedness and cooperativeness, might play a role in the risk of developing post-traumatic stress disorder (PTSD) after deployment to a combat zone. However, it is unclear whether these traits are preexisting risk factors or if event-related changes might also be associated with PTSD symptoms over time. Therefore, the current aim was to assess if military deployment is associated with changes in cooperativeness and self-directedness and to study how these traits are related to PTSD symptom trajectories. METHODS: In a large co

Altered lateralization of the cingulum in deployment-related traumatic brain injury: An ENIGMA military-relevant brain injury study.

Traumatic brain injury (TBI) in military populations can cause disruptions in brain structure and function, along with cognitive and psychological dysfunction. Diffusion magnetic resonance imaging (dMRI) can detect alterations in white matter (WM) microstructure, but few studies have examined brain asymmetry. Examining asymmetry in large samples may increase sensitivity to detect heterogeneous areas of WM alteration in mild TBI. Through the Enhancing Neuroimaging Genetics Through Meta-Analysis Military-Relevant Brain Injury working group, we conducted a mega-analysis of neuroimaging and clinic

Crawl, Walk Fast, Run Hard: Military-Civilian Partnership Training at the Army Trauma Training Course Improves Perceived Deployment Readiness.

INTRODUCTION: The Army Trauma Training Course (ATTC) has held the responsibility of training deploying U.S. Army Forward Resuscitative and Surgical Detachments (FRSDs) since September 2001. The program of instruction (POI) consists of didactics and skill stations (Crawl), a mass casualty (MASCAL) situational training exercise (STX) (Walk), and clinical integration into an urban Level I trauma center (Run). Over the program's 25-year existence, there has been no report on the subjective assessment of the rotator's perceived effectiveness of the program as a whole. We hypothesized that a post-co
