Mitochondrial cytopathy is a multisystemic disease that requires different pharmacological and specialist approaches; although most therapies are usually of scarce effectiveness. We describe a clinical management of a very young girl with Pearson's syndrome that developed the symptoms of Kearns-Sayre syndrome. Many of symptoms were temporarily improved by the replacement therapy with hydrocortisone introduced to treat the partial adrenal insufficiency. During her life, she showed an ample clinical spectrum of symptoms because of multiple organs involvements: firstly bone marrow and, thereafter

The need for clinical awareness and diagnostic precision of glycogen storage disease type 2 (GSD2) has increased, as enzyme replacement therapy has become available. So far, only small series have reported the muscle pathology of late-onset GSD2. We reassessed 43 muscle biopsies of 38 GSD2 patients. In all patients the diagnosis of GSD2 has been established by biochemistry and/or mutational analysis of the GAA gene. Additionally to the expected morphological features, ultrastructural analysis revealed a high incidence of autophagic vacuoles, lipofuscin debris, structural Z-line disorganization

Mitochondrial dysfunction is a fundamental mechanism leading to drug nephrotoxicity, such as gentamicin-induced nephrotoxicity. Mitochondrial therapy (mitotherapy) or exogenous mitochondria transplantation is a method that can be used to replace dysfunctional mitochondria with healthy mitochondria. This method can help in the treatment of diseases related to mitochondria. In this research, we studied the transplantation effect of freshly isolated mitochondria on the toxicity induced by gentamicin on renal proximal tubular cells (RPTCs). Furthermore, possible gender-related effects on supplying

BACKGROUND: Cisplatin-induced nephrotoxicity has been linked to a fundamental mechanism of mitochondrial dysfunction. A treatment called mitochondrial transplantation therapy can be used to replace damaged mitochondria with healthy mitochondria. Mitochondrial-related diseases may benefit from this approach.
OBJECTIVES: We investigated the effect of mitochondrial transplantation on cisplatin-induced nephrotoxicity using freshly isolated mitochondria obtained from renal proximal tubular cells (RPTCs).
METHODS: Based on our previous findings, we hypothesized that direct exposure of healthy mito

Nicotine replacement therapy (NRT) is currently recommended as a safe smoking cessation aid for pregnant women. However, fetal and neonatal nicotine exposure in rats causes mitochondrial-mediated beta cell apoptosis at weaning, and adult-onset dysglycemia, which we hypothesize is related to progressive mitochondrial dysfunction in the pancreas. Therefore in this study we examined the effect of fetal and neonatal exposure to nicotine on pancreatic mitochondrial structure and function during postnatal development. Female Wistar rats were given saline (vehicle control) or nicotine bitartrate (1 m

Retinal ganglion cell (RGC) degeneration represents a cardinal etiology of irreversible vision loss in glaucoma, where efficacious regenerative therapies remain scarce. RGC replacement therapy holds promise for visual function restoration, yet its therapeutic efficacy is constrained by the hostile glaucomatous microenvironment, dominated by oxidative stress that compromises transplanted RGC survival. Here, we report the design of multifunctional lithium-epigallocatechin gallate nanoparticles (Li-EGCG NPs) to synergistically enhance antioxidant capacity and neuroprotection. These NPs exhibit un

Mitochondrial neurogastrointestinal encephalomyopathy (MNGIE) is caused by mutations in the gene encoding thymidine phosphorylase (TP). All MNGIE patients have had severe loss of TP function and prominent plasma accumulations of the TP substrates thymidine (dThd) and deoxyuridine (dUrd). Here, we report for the first time to our knowledge three MNGIE patients with later onset, milder phenotype, and less severe TP dysfunction, compared with typical MNGIE patients. This report demonstrates a direct relationship between the biochemical defects and clinical phenotypes in MNGIE and supports the not

Sepsis-associated acute kidney injury (SA-AKI) is a complex, life-threatening condition resulting from a dysregulated immune response to infection, leading to organ dysfunction. It is common in critically ill patients and involves multifactorial pathophysiology including inflammation, endothelial damage, mitochondrial dysfunction, and other pathways. SA-AKI increases the need for renal replacement therapy and is linked to poor long-term outcomes. Risk stratification and emerging subphenotyping methods aim to improve diagnosis and treatment. Current management focuses on prevention and supporti

This article provides an overview of neuromuscular diseases in childhood for pediatric neurologists, highlighting conditions with available specific treatments. It focuses on spinal muscular atrophy (SMA), where disease-modifying therapies have changed the natural history of the disease. Congenital myasthenic syndromes are addressed next, emphasizing the importance of genetic diagnosis for tailored therapies. In the field of muscular dystrophies, we will highlight advances in Duchenne. Mitochondrial myopathies are also reviewed, with mention of treatments such nucleoside for timidine kinase de

BACKGROUND: Hypoxia disrupts corneal stromal physiology and alters cellular energy metabolism. This study aimed to elucidate the molecular role of the cellular energy sensor AMP-activated protein kinase (AMPK) in modulating the energy metabolism of human keratocytes (HKs) under hypoxic conditions and to explore a potential therapeutic strategy.
METHODS: In vitro, HKs were exposed to 1% O₂ for 2 h and 24 h to simulate hypoxia. In vivo, a mouse corneal alkali burn model was established, in which hypoxia is recognized as an important contributing factor to tissue injury. The expression of AMPK a

Duchenne muscular dystrophy (DMD) is an X-linked neuromuscular disease caused by a pathogenic disruption of the DYSTROPHIN gene that results in non-functional dystrophin protein. DMD patients experience loss of ambulation, cardiac arrhythmia, metabolic syndrome, and respiratory failure. At the molecular level, the lack of dystrophin in the muscle results in myofiber death, fibrotic infiltration, and mitochondrial dysfunction. There is no cure for DMD, although dystrophin-replacement gene therapies and exon-skipping approaches are being pursued in clinical trials. Mitochondrial dysfunction is o

Friedreich's ataxia (FRDA) is an autosomal recessive neurodegenerative disease caused by a GAA repeat expansion in the frataxin (FXN) gene, leading to reduced frataxin, a protein essential for mitochondrial function. We developed a replacement strategy using a fusion protein containing secretion and cell-penetrating sequences fused to the frataxin precursor. In vitro studies confirmed secretion, cellular penetration, mitochondrial localization, and rescue of biochemical defects and apoptosis in cells from patients with FRDA. The therapeutic cDNA was cloned into a lentiviral vector and used to

Mucopolysaccharidosis type II (MPS II) is a rare lysosomal storage disease (LSD) involving a genetic error in iduronic acid-2-sulfatase (IDS) metabolism that leads to accumulation of glycosaminoglycans within intracellular lysosomes. The primary treatment for MPS II, enzyme replacement therapy, is not effective for central nervous system (CNS) symptoms, such as intellectual disability, because the drugs do not cross the blood-brain barrier. Recently, autophagy has been associated with LSDs. In this study, we examined the morphologic relationship between neuronal damage and autophagy in IDS kno

Huntington's disease (HD) is caused by a CAG expansion in the huntingtin gene. Expansion of the polyglutamine tract in the huntingtin protein results in massive cell death in the striatum of HD patients. We report that human induced pluripotent stem cells (iPSCs) derived from HD patient fibroblasts can be corrected by the replacement of the expanded CAG repeat with a normal repeat using homologous recombination, and that the correction persists in iPSC differentiation into DARPP-32-positive neurons in vitro and in vivo. Further, correction of the HD-iPSCs normalized pathogenic HD signaling pat

Premature ovarian insufficiency (POI) is a heterogeneous disorder characterized by loss of ovarian function before age 40, causing infertility, hypoestrogenism-related complications, and significantly impaired quality of life. Although hormone replacement therapy (HRT) remains the standard treatment for symptom relief and long-term health protection, it cannot restore ovarian reserve or natural fertility. This review summarizes the etiology and pathogenesis of POI, emphasizing mitochondrial dysfunction, oxidative stress, chronic inflammation, and progressive deterioration of the ovarian microe

The biennial 15th symposium on Purine and Pyrimidine metabolism was held in Madrid, June 2013 (PP13). During the meeting, several novel developments on the diagnosis, pathophysiology, and treatment of several inborn errors of purine and pyrimidine metabolism were presented. These ranged from new drugs for gout to enzyme replacement therapies for mitochondrial diseases. A relatively novel aspect in this meeting was the interest in purine and pyrimidine metabolism in nonmammalian systems, such as parasites, mycoplasms, and bacteria. Development of novel analogs for parasite infections, cardiovas

Coenzyme Q10 (CoQ10) deficiency can manifest diversely, from isolated myopathy to multisystem involvement. Immune dysregulation has not been reported as a feature of the disease. We report a four-year old girl with failure to thrive, recurrent infections, developmental delay with hypotonia, and CoQ10 deficiency with impaired immune function, which improved after CoQ10 and immunoglobulin replacement therapy. Immune dysfunction in CoQ10 deficiency should be considered and treated appropriately.

Alzheimer's disease (AD) is the primary cause of dementia, characterized by a progres-sive decrease in mental abilities and the accumulation of amyloid-beta (Aβ) peptides in the brain. The combination of these peptides leads to the development of neuritic plaques and neurofibrillary tangles that disrupt neural communication and eventually lead to the loss of neurons. One of the fac-tors that are involved in the development of AD is mitochondrial dysfunction. Disrupted function-ing of mitochondria leads to the production of less energy by the cells, increased oxidative stress, and accelerates t

Hypophosphatasia (HPP) is an inherited metabolic disease caused by deficient tissue-nonspecific alkaline phosphatase (ALP) activity and characterized by skeletal and nonskeletal symptoms, including muscle weakness and fatigue. We hypothesized that mitochondrial respiration is impaired in muscle in HPP, independent of skeletal manifestations, and that the second-generation ALP enzyme replacement therapy (ERT) efzimfotase alfa improves respiration. Akp2GW-/- mice were used for this purpose. Body weight, bone mineralization, and survival were validated in Akp2GW-/- mice versus Akp2-/- mice, an es
