Foundation models enable wearable signal screening for cardiovascular disease among people living with HIV.

BACKGROUND: Cardiovascular disease screening faces significant challenges in resource-limited settings, where infrastructure and computational constraints preclude advanced assessment. These constraints are particularly acute for people living with human immunodeficiency virus (HIV), who experience elevated cardiovascular risk yet often receive care in clinics without specialist diagnostic capacity. Pretrained physiological foundation models offer potential for low-cost screening using wearable sensors, though their applicability in resource-constrained settings remains unclear. METHODS: We e

From task-specific AI to cardiovascular foundation models: a new era of multimodal interpretation.

Artificial intelligence (AI) is reshaping cardiovascular medicine, evolving from task-specific supervised models towards multitask and foundation models capable of broader, more scalable interpretation. Early AI systems achieved high performance in electrocardiographic interpretation, echocardiographic measurement and disease diagnosis but their dependence on predefined outputs and labelled datasets limits flexibility and confines them to individual tasks. Foundation models address these limitations by learning reusable representations from large-scale data, often through self-supervised or co

Prediction Models for Postoperative Delirium After Cardiovascular Surgery: A Systematic Review and Critical Appraisal.

BACKGROUND: Postoperative delirium of cardiovascular surgery (PODOCVS) is an acute brain dysfunction, which will lead to increased postoperative complications and high mortality rates. Using prediction models for early identification and intervention could improve outcomes and resource utilisation. However, the quality and applicability of existing models remain unclear, and healthcare professionals are often unsure which model should be recommended to cardiovascular surgery (CS) patients in a specific setting. AIM: To systematically review and critically evaluate the development, performance

Circulating Humanin Improves the Prognostic Accuracy of Cardiovascular Risk Models in Chronic Hemodialysis Patients.

INTRODUCTION: Cardiovascular morbidity and mortality (CVMM) remain highly prevalent among end-stage kidney disease (ESKD) patients undergoing chronic hemodialysis (HD). Nevertheless, risk prediction in this setting is limited by the complexity of factors involved and the individual patients' characteristics. In this study, we evaluated whether circulating levels of Humanin, a micropeptide reflecting mitochondrial dysfunction, could refine CVMM prediction when included in established risk models for CVMM in ESKD-HD. METHODS: Eighty-three chronic HD patients (mean age 67 ± 12 years; 74.7% male)

The heart of the matter: modeling HIV-associated cardiovascular comorbidities in nonhuman primate models.

With the advent of antiretroviral therapy (ART) that effectively suppresses HIV replication, and reduced AIDS progression, the clinical spectrum of HIV infection has dramatically changed. Currently, the people living with HIV (PLWH) who receive ART have a nearly normal prognostic of survival, yet they still experience higher morbidity and mortality than age-matched uninfected subjects. The higher risk of death in PLWH is linked to persistence of residual systemic inflammation and T-cell activation. These factors contribute to accelerated aging and higher incidence of HIV-associated non-AIDS co

Experimental Models to Study Endothelial to Mesenchymal Transition in Myocardial Fibrosis and Cardiovascular Diseases.

Fibrosis is a common feature of cardiovascular diseases and targets multiple organs, such as the heart and vessels. Endothelial to mesenchymal transition is a complex, vital process that occurs during embryonic formation and plays a crucial role in cardiac development. It is also a fundamental process implicated in cardiac fibrosis and repair, but also in other organs. Indeed, in numerous cardiovascular diseases, the endothelial-to-mesenchymal transition has been shown to be involved in the generation of fibroblasts that are able to produce extracellular matrix proteins such as type I collagen

The Cardiovascular and Metabolic Effects of Chronic Hypoxia in Animal Models: A Mini-Review.

Animal models are useful to understand the myriad physiological effects of hypoxia. Such models attempt to recapitulate the hypoxemia of human disease in various ways. In this mini-review, we consider the various animal models which have been deployed to understand the effects of chronic hypoxia on pulmonary and systemic blood pressure, glucose and lipid metabolism, atherosclerosis, and stroke. Chronic sustained hypoxia (CSH)-a model of chronic lung or heart diseases in which hypoxemia may be longstanding and persistent, or of high altitude, in which effective atmospheric oxygen concentration

Machine Learning Models Predicting Cardiovascular and Renal Outcomes and Mortality in Patients with Hyperkalemia.

Hyperkalemia is associated with increased risks of mortality and adverse clinical outcomes. The treatment of hyperkalemia often leads to the discontinuation or restriction of beneficial but potassium-increasing therapy such as renin-angiotensin-aldosterone inhibitors (RAASi) and high-potassium diet including fruits and vegetables. To date, limited evidence is available for personalized risk evaluation in this heterogeneous and multifactorial pathophysiological condition. We developed risk prediction models using extreme gradient boosting (XGB), multiple logistic regression (LR), and deep neura

Combined exposure to multiple metals on cardiovascular disease in NHANES under five statistical models.

BACKGROUND: It is well-documented that heavy metals are associated with cardiovascular disease (CVD). However, there is few studies exploring effect of metal mixture on CVD. Therefore, the primary objective of present study was to investigate the joint effect of heavy metals on CVD and to identify the most influential metals in the mixture. METHODS: Original data for study subjects were obtained from the National Health and Nutrition Examination Survey. In this study, adults with complete data on 12 kinds of urinary metals (antimony, arsenic, barium, cadmium, cobalt, cesium, molybdenum, mercu

Scalability of cardiovascular intrinsic frequencies: Validations in preclinical models and non-invasive clinical studies.

AIMS: Cardiovascular intrinsic frequencies (IFs) are associated with cardiovascular health and disease, separately capturing the systolic and diastolic information contained in a single (uncalibrated) arterial waveform. Previous clinical investigations related to IF have been restricted to studying chronic conditions, and hence its applicability for acute cardiovascular diseases has not been explored. Studies of cardiovascular complications such as acute myocardial infarction are difficult to perform in humans due to the high-risk and invasive nature of such procedures. Although they can be pe

Gene expression profiles of CLC chloride channels in animal models with different cardiovascular diseases.

Here for the first time we investigated the potential involvement of the CLC chloride channel family at the transcriptional level in different cardiovascular diseases. Northern blot and semiquantitative RT-PCR analyses were used to study the gene expression profiles of all CLC genes present in the heart and kidney; namely, CLC-2, CLC-3, CLC-4, CLC-5, CLC-6, CLC-7, CLC-K1, and CLC-K2. Rat models with distinctive cardiovascular diseases were studied: These included spontaneously hypertensive rats, nutritionally- and surgically-induced hypertensive rats with cardiac hypertrophy, as well as rats s

Anger inhibition, cardiovascular recovery, and vagal function: a model of the link between hostility and cardiovascular disease.

A model of the association between hostility and cardiovascular disease (CVD) is proposed based upon anger inhibition, slow cardiovascular recovery, and low parasympathetic activity (vagal tone). This model is opposed to the more conventional model that emphasizes anger expression, cardiovascular reactivity, and high sympathetic tone. We argue that in social reality, incidences of anger inhibition outnumber incidences of anger expression to a great extent, irrespective of preferred expression style. Moreover, slow cardiovascular recovery, rather than high reactivity, may be the mechanism under

Zebrafish in Cardiovascular Disease Research: from Model to Application.

Cardiovascular diseases (CVDs) remain a major cause of global morbidity and mortality, yet their complex pathophysiology is difficult to recapitulate in conventional mammalian models fully. Compared with traditional mammalian and in vitro systems, zebrafish offer several distinct advantages for CVD research. Their small size, high fecundity, and rapid development make them particularly suitable for high-throughput screening, while embryonic transparency enables real-time, noninvasive imaging of dynamic cardiac processes. High genetic homology with humans, together with facile genetic manipulat

Automated Blood Sampling in a Canine Telemetry Cardiovascular Model.

Successful implementation of automated blood sampling (ABS) into a telemetry instrumented canine cardiovascular model provides simultaneous cardiovascular assessment of novel compounds while collecting multiple blood samples for analysis of drug level, cytokines, and biomarkers. Purpose-bred male Beagle dogs (n = 36) were instrumented with a dual-pressure telemetry transmitter and vascular access port. Modifications to acclimation practices, surgical procedures, and housing were required for implementation of ABS in our established cardiovascular canine telemetry colony. These modifications ha

Cardiac Tissue Chips (CTCs) for Modeling Cardiovascular Disease.

OBJECTIVE: Cardiovascular research and regenerative strategies have been significantly limited by the lack of relevant cell culture models that can recreate complex hemodynamic stresses associated with pressure-volume changes in the heart. METHODS: To address this issue, we designed a biomimetic cardiac tissue chip (CTC) model where encapsulated cardiac cells can be cultured in three-dimensional (3-D) fibres and subjected to hemodynamic loading to mimic pressure-volume changes seen in the left ventricle. These 3-D fibres are suspended within a microfluidic chamber between two posts and integr

Absence of cardiovascular manifestations in a haploinsufficient Tgfbr1 mouse model.

Loeys-Dietz syndrome (LDS) is an autosomal dominant arterial aneurysm disease belonging to the spectrum of transforming growth factor β (TGFβ)-associated vasculopathies. In its most typical form it is characterized by the presence of hypertelorism, bifid uvula/cleft palate and aortic aneurysm and/or arterial tortuosity. LDS is caused by heterozygous loss of function mutations in the genes encoding TGFβ receptor 1 and 2 (TGFBR1 and -2), which lead to a paradoxical increase in TGFβ signaling. To address this apparent paradox and to gain more insight into the pathophysiology of aneurysmal disease
