[Intraoperative electroneurophysiological monitoring of basal cranial nerve surgery].

INTRODUCTION: Since 1980 when intraoperative monitoring was introduced or acoustic nerve surgery, the technique rapidly was outspreaded to the other basal cranial nerves, and today is the most important and indispensable piece of work first of all because of its effectiveness in the prevention of the neurologic deficits. OBJECTIVE: The aim of this work is to describe the basic requisites needed (personnel, equipment, stimulation and recording electrodes, patient handling), as well as to review the different techniques used (electromyography and electroneurography) in the motor cranial nerve m

[Intraoperative monitoring with evoked potentials in spinal interventions].

This review article delineates the physiology and methodological principles of somatosensory (SEP) and motor evoked potentials (MEP), as well as our own results in 40 patients monitored during spinal surgery. In 29 patients an intraoperative SEP and in 15 patients a MEP monitoring was performed. Both modalities were applied in 4 patients. 19 patients had an intramedullary tumor, 15 patients had an intradural extramedullary tumor, 4 patients had an extradural mass lesion, and 2 patients had a spinal arteriovenous malformation. Technical problems with SEP monitoring occurred in 3 of 29 cases, pr

[Intraoperative ABR monitoring during cerebello-pontine angle surgery].

Intraoperative auditory brainstem response (ABR) monitoring was performed in 27 patients undergoing C-P angle surgery (12: hemifacial spasm, 10: trigeminal neuralgia, 2: glossopharyngeal neuralgia, 3: C-P angle tumor, 2 epidermoid, 1 meningioma). Because of the introduction of this method, no patient suffered from postoperative hearing disturbance in this series. During C-P angle surgery, the wave V of ABR changes according to the retraction of the cerebellum and the manipulation of the eighth cranial nerve. Many authors have discussed this change, however the timing and the mechanism of disap

Routine intraoperative facial nerve monitoring during otologic surgery.

We have used intraoperative monitoring and stimulation of facial nerve function routinely in 301 consecutive otologic and neurotologic cases. The device has been safe, simple to use, and practical. Facial contraction is detected by a strain-gauge sensor in the corner of the mouth and is signalled audibly to the surgeon. Electrical stimulation of the facial nerve can be delivered through a sterile probe, which produces a constant-current-square-wave impulse. The device has several advantages: it signals unintentional mechanical stimulation of the facial nerve during surgery; it allows mapping o

Transtragal, transtympanic electrode placement for intraoperative electrocochleographic monitoring.

Transtragal, transtympanic electrode placement was performed for the purpose of intraoperative electrocochleographic (ECoG) monitoring during seven suboccipital acoustic neuroma resections. The promontory, the tragus, and two external sutures stabilized the electrode during surgery. The only noted otologic sequallae of electrode placement were small circular lesions at tympanic membrane puncture sites. Satisfactory ECoG recordings were obtained in five of seven cases. Peak-to-peak N1 amplitudes were (on average) 13.4-fold larger than the corresponding amplitudes of peak I of the surface-record

[The intraoperative monitoring of the gas pressure in the middle ear].

The objective of the present work was to elucidate the patterns and mechanisms underlying the changes of gas pressure in the middle ear during the operation of stapedoplasty with a view to the application of the data obtained for objective recording of the stapedial reflex at the time of surgical intervention. Ten subjects with the unaffected hearing function were recruited for the study. Tympanometry was carried out at one-minute intervals starting from the onset of feeding nitrogen monooxide till the completion of septoplasty. It was shown that the surgical intervention under inhalation anes

Intraoperative motor and sensory monitoring of the cauda equina.

A set of standard techniques to monitor the motor and sensory function of the cauda equina is proposed for surgery in the lumbosacral spinal canal for the release of a tethered cord or the removal of a neoplasm. Continuous loudspeaker-controlled recording of electromyographic activity in four leg muscles of both sides supplied the surgeon with immediate feedback on injury to any of the motor roots from the second lumbar to the fourth sacral segment. Continuous recording of tibial nerve somatosensory evoked potentials yielded information about the functional state of parts of the lumbosacral se

Methods and Principles of the Intraoperative Neurophysiologic Monitoring in Neurosurgery.

Intraoperative neurophysiologic monitoring (IONM) is an innovation introduced in neurosurgery in the past decades. It aims to support and guide the neurosurgeon to obtain the best surgical result possible, preventing the occurrence of neurological deficits. The somatosensory evoked potentials (SSEP) assess the integrity of the sensory pathways monitoring the dorsal column-medial lemniscus pathway during spine and cerebral surgery. Motor evoked potentials (MEPs) provide information on the integrity of the motor pathway monitoring the efferent motor pathways from the motor cortex to the muscle t

Neurophysiologic intraoperative monitoring of the glossopharyngeal and vagus nerves.

Neurophysiologic intraoperative monitoring (NIOM) of the glossopharyngeal and vagus nerves (CN IX and X) is often used during surgeries involving the lower brain stem. Although both of these nerves contain sensory, autonomic, and motor fibers, it is the motor fibers that are most amenable to NIOM. CN IX supplies the stylopharyngeus muscle, and CN X supplies striated muscles in the soft palate, pharynx, and larynx. Monitoring of these CN can be performed by monitoring free running and stimulated electromyography (EMG) from the stylopharyngeus muscle (CN IX) and the vocal cords (CN X). Various s

Targeted parathyroidectomy in the era of intraoperative parathormone monitoring.

A combination of preoperative localization and intraoperative parathormone (PTH) monitoring permits targeted parathyroidectomy. Multiple approaches have been developed, ranging from unilateral neck exploration (UE) to radio-guided parathyroidectomy (RP) to endoscopic parathyroidectomy (EP). The purpose of this study was to evaluate the efficacy of these approaches in the management of primary hyperparathyroidism. From June 1998 to November 2000 a total of 110 targeted parathyroid operations were performed at a university medical center. All patients underwent technetium-99m-sestamibi scanning,

Intraoperative parathyroid hormone monitoring as an adjunct to parathyroidectomy.

BACKGROUND: Parathyroidectomy has a success rate of greater than 95% in the hands of experienced surgeons. To maintain this result in a more cost-effective way, intraoperative monitoring of intact parathyroid hormone (iPTH) has been used to decrease operative times. This technique signals when all hyperfunctioning tissue has been excised or when further dissection is necessary. METHODS: Eighty-nine consecutive patients with hyperparathyroidism had plasma samples measured for iPTH levels during parathyroidectomy. Nine patients had previous neck explorations. Perioperative iPTH measurements usi

Intraoperative Intracranial Pressure Monitoring as an Intraoperative Guide During Operations for Relieving Elevated Intracranial Pressure.

BACKGROUND: Intracranial pressure (ICP) is a well-established measure in managing not only traumatic brain injury but also nontraumatic intracranial bleeding or edema. When ICP increases despite nursing or medical management, ICP may be reduced via surgical measures. Deciding whether to perform a craniotomy vs. craniectomy (whether the bone flap is replaced or not, respectively) is commonly made intraoperatively following preoperative planning. While ICP monitoring (ICPm) is standard pre- and postoperatively, its intraoperative utility remains understudied. METHODS: We conducted a study utili

Superior laryngeal nerve monitoring using laryngeal surface electrodes and intraoperative neurophysiological monitoring during thyroidectomy.

The objective of this study is to establish normative waveform data for the external branch of the superior laryngeal nerve (SLN) utilizing laryngeal surface electrodes and intraoperative neurophysiological monitoring (IONM) in conjunction with a clinical neurophysiologist. A retrospective chart review of 91 consecutive at-risk SLN were identified in 51 patients in whom IONM using laryngeal surface electrodes was performed by a clinical neurophysiologist using Dragonfly (Neurovision Medical Products, Ventura, CA) recording electrodes and a Protektor (Natus Medical Inc., San Carlos, CA)16 chann

Intraoperative cerebral perfusion monitoring with ultrafast power doppler imaging.

Brain perfusion monitoring during cerebrovascular surgery is highly desirable; however, this technology is not yet available to surgeons. Our center has developed and implemented a dedicated ultrafast power Doppler imaging (UPDI) sequence along with a novel postprocessing analysis. This method enables intraoperative, real-time (1 Hz) detection of changes in cerebral blood volume (CBV) at high spatial resolution (100 μm). We performed continuous UPDI acquisitions in 10 patients undergoing surgery for intracranial aneurysm clipping, AVM resection, or cerebrovascular bypass surgery. When temporar

Management of Postthyroidectomy Hypocalcemia Using Intraoperative Parathyroid Hormone Monitoring.

IMPORTANCE: Postthyroidectomy hypoparathyroidism is typically diagnosed with low serum calcium levels, often requiring patients to remain in the hospital for appropriate treatment. Given the short half-life of parathyroid hormone (PTH), can hypoparathyroidism be diagnosed intraoperatively when re-exploration and autotransplant are still possible? OBJECTIVE: To determine whether intraoperative parathyroid hormone monitoring (ioPTH) permits the stratification of patients into appropriate tiers for postoperative supplementation. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study

Progress and Outcomes of Intraoperative Nerve Monitoring During Thyroidectomy.

IMPORTANCE: Intraoperative nerve monitoring (IONM) is not considered standard of care during thyroidectomy, and guidelines are vague about its use in the absence of strong evidence of superiority over visualization of the recurrent laryngeal nerve (RLN) alone. OBJECTIVE: To characterize patterns of IONM use during thyroidectomy in the US and evaluate the association of IONM with postoperative outcomes. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used the National Surgical Quality Improvement Program (NSQIP) thyroidectomy data from January 1, 2016, to December 31, 2022. A nationally
