Effect of joint mobilization on serum hyaluronate.

Serum hyaluronate was monitored in volunteer patients with rheumatoid arthritis who were mobilized on day 1 at 7 am, but stayed at rest in bed on day 2 until mobilized at 11:30 am. 125I-hyaluronate binding protein assay disclosed a normal morning peak on day 1 but not on day 2. Instead, on day 2 transiently high hyaluronate levels were seen after 11:30 am. This suggests that joint mobilization affects serum hyaluronate. The morning peak may depend on the accumulation of hyaluronate in immobilized joints at night and its mobilization in the morning. High performance liquid chromatography with a

High tibial osteotomy. Use of an osteotomy jig, rigid fixation, and early motion versus conventional surgical technique and cast immobilization.

High tibial osteotomy (HTO) using conventional surgical technique and cast immobilization was compared to HTO using an osteotomy jig, rigid internal fixation, and early motion. Fifteen patients (19 knees) had conventional HTO. The mean preoperative femoral-tibial angle was 2.5 degrees of varus, and the mean postoperative angle was 6.5 degrees of valgus. Two knees were undercorrected and eight knees (42%) had associated complications. Twenty patients (21 knees) had HTO utilizing the new surgical technique and postoperative management. The mean preoperative femoral-tibial angle was 2.3 degrees v

The early treatment of motion complications after reconstruction of the anterior cruciate ligament.

The use of active and passive knee motion in the immediate postoperative period and a treatment plan for early postoperative limitations in knee motion has proven highly effective in restoring motion after anterior cruciate ligament (ACL) reconstruction. Of 207 knees, 189 (91%) regained a full range of motion of 0 degrees-135 degrees. The remaining 18 knees (9%) did not regain motion as rapidly as the others and were placed in an early postoperative phased treatment program. Six knees had serial extension casts, nine had early gentle manipulation under anesthesia, and three had arthroscopic ly

Heterotopic ossification around the elbow following burns in children: results after excision.

BACKGROUND: Major burn injuries close to joints alter the function of the musculoskeletal system through tissue loss and limitation of joint motion. In children with involvement of the hand, wrist, and forearm, restriction of elbow motion secondary to heterotopic ossification following a burn injury severely limits the function of the upper extremity. The purpose of this study was to review elbow function following excision of heterotopic ossification around the elbow in children. METHODS: Eight children (ten elbows) from a population of 3245 consecutive patients who were admitted to our pedi

Increased ROM and high patient satisfaction after open arthrolysis: a follow-up-study of 43 patients with posttraumatic stiff elbows.

BACKGROUND: Posttraumatic stiffness of the elbow is a common finding after elbow trauma. Restoration of motion in the posttraumatic stiff elbow is difficult, time consuming, and requires high patient compliance. We have evaluated the long-term effect of an open elbow arthrolysis in the posttraumatic stiff elbow. METHODS: We evaluated 43 patients (14 women, 29 men) with a median age of 47(16-78) years operated with open arthrolysis for a posttraumatic stiff elbow. The median follow-up time was 41(12-204) months. The patients were hospitalized median 12(4-14) days, with daily physiotherapy and

The results of conservatively treated simple elbow dislocations.

OBJECTIVES: Closed reduction followed by short-time immobilization is the generally accepted treatment for simple elbow dislocations. However, the number of studies concerning the results of this method is limited. In this study, the clinical and radiographic results of conservatively treated simple elbow dislocations were retrospectively evaluated. METHODS: The study included 21 patients (16 males, 5 females) who underwent closed reduction and immobilization for simple elbow dislocations. Simple dislocations were defined as non-fracture dislocations and dislocations accompanied by minor avul

Gap formation after flexor tendon repair in zone II. Results with a new controlled motion programme.

Intratendinous metal markers were used to study gap formation in 36 flexor digitorum profundus repairs during and after early controlled motion with a programme combining dynamic traction and passive flexion to all four digits. The mean gap 3 weeks after operation was 2.5 mm. The mean final gap was 2.6 mm. The results of linear regression analysis showed that gap formation within the observed range from 0 to 8.5 mm did not negatively affect tendon excursions or clinical results in terms of active interphalangeal joint range of motion. These findings are in accordance with the results of a prev

[Efficiency of a postoperative treatment after rotator cuff repair with a continuous passive motion device (CPM)].

AIM: The main objective of this study was to prove that a postoperative combined continuous passive motion (CPM) and physiotherapy treatment protocol (CPM group) can achieve 90 degrees active abduction in the shoulder joint earlier than physiotherapy alone (PT group). The indication was a complete tear of the rotator cuff. METHOD: The study was conducted under in-patient and out-patient conditions. 55 patients were included in this study. The prospective, randomized multicenter study design complies with DIN EN 540. The primary endpoint was the time span until 90 degrees active abduction was

The important role of motion in the rehabilitation of patients with mandibular hypomobility: a review of the literature.

The orthopedic literature has clearly demonstrated that the physical stimulus of motion is essential for the maintenance of the structural and functional integrity of synovial joints. As a result of this information, rehabilitation with passive motion has become an extremely important part of the rehabilitation of synovial joints throughout the body, especially when there is limited range of motion. This same biological principle must be applied to the temporomandibular joint, especially when there is limitation in mandibular mobility. The effects of limited joint mobility, as well as passive

Effectiveness of continuous passive motion in alleviating neuropathic pain in people with spinal cord injury: a pilot study.

STUDY DESIGN: Single-arm prospective pilot study. OBJECTIVES: Chronic neuropathic pain (NP) following spinal cord injury (SCI) is often refractory to standard treatments. Sensorimotor activity has been proposed to counteract maladaptive plasticity associated with NP. This pilot study explored whether continuous passive motion (CPM) may have analgesic effects on chronic below-level NP in people with SCI. SETTING: Incheon Workers' Compensation Hospital, South Korea. METHODS: A total of 15 inpatients with chronic SCI and below-level NP underwent 10 sessions of knee CPM (60 minutes/day) over 2

Intensive seated robotic training of the ankle in patients with chronic stroke differentially improves gait.

BACKGROUND: Robotic driven treatment plans targeting isolated joints of the upper limb have improved the sensorimotor condition of patients with stroke. Similar intensive efforts to allay lower limb gait impairment have not been so successful. In patients with stroke, targeted robot assisted training of the ankle joint, in a seated position, has demonstrated significant alterations in ankle stiffness and foot-ankle orientation at foot strike which may provide a new treatment option for gait impairment. OBJECTIVE: To determine if isolated robot-assisted training of the ankle joint improves chr

Active prospective control is required for effective sensorimotor learning.

Passive modeling of movements is often used in movement therapy to overcome disabilities caused by stroke or other disorders (e.g. Developmental Coordination Disorder or Cerebral Palsy). Either a therapist or, recently, a specially designed robot moves or guides the limb passively through the movement to be trained. In contrast, action theory has long suggested that effective skill acquisition requires movements to be actively generated. Is this true? In view of the former, we explicitly tested the latter. Previously, a method was developed that allows children with Developmental Coordination

Gait analysis as a reliable tool for rehabilitation of chronic hemiplegic patients.

Gait analysis system was used for 3 outpatients who suffered from hemorrhagic stroke 1 year before. They presented with hemiplegia and spasticity. A new treatment was applied to paralyzed muscles of lower limb to improve both static and dynamic balance. The refined system showed that asymmetric load distribution improved in all patients. By contrast, kinematical data improved in 1 patient only. Analysis of the vertical force showed a partial improvement after treatment. More controlled studies are necessary to verify the results of such an approach. A standard therapeutical protocol for spasti

Fresh osteochondral allografts for treatment of articular defects in osteochondritis dissecans of the lateral femoral condyle in adults.

Fresh osteochondral allografts were used to patch defects in the lateral femoral condyle in 17 patients with osteochondritis dissecans. The patients included 12 males and five females, ages 16 to 46 years. All had previously undergone other procedures including pinning (4), primary removal of osteochondral fragments (16), and abrasion arthroplasty (14). Defects up to 3 cm in diameter were treated with isotopic grafts and fixed with Herbert screws. Larger defects were treated with crescent-shaped grafts fixed with multiple Herbert screws. Follow-up time ranged from two to nine years. Pain, stif

A randomized controlled trial comparing controlled active motion and early passive mobilization protocols for rehabilitation of repaired flexor tendons in zone II.

BACKGROUND: Flexor tendon injuries in zone II of the hand pose serious clinical complications due to the high risk of adhesion formation and suboptimal clinical outcomes, although controlled active motion (CAM) and early passive mobilization (EPM) are standard protocols used during rehabilitation. OBJECTIVE: This randomized trial compared functional outcomes between CAM and EPM systems after zone II flexor tendon repair. METHODS: Forty patients with entire zone II flexor digitorum profundus and superficialis tears were randomly assigned to either the CAM or EPM protocol for 12 weeks' rehabil

The Impact of Continuous Passive Motion on Heterotopic Ossification Maturation.

Heterotopic ossification is the formation of pathological bone in soft tissues. It is postulated that continuous passive motion is a helpful adjuvant in the halt of the heterotopic ossification progression and the maintenance or increase of the joint mobility. The purpose of this clinical case study is to present the effectiveness of continuous passive motion. A 46-yr-old male patient experiencing right hemiparesis arrived in our rehabilitation department with limitation on passive flexion of the right hip. On x-ray of the pelvis, immature heterotopic bone formation was found. To halt this ong

Passive intervertebral restraint is different in patients with treatment-resistant chronic nonspecific low back pain: a retrospective cohort study and control comparison.

PURPOSE: In vivo studies of continuous lumbar sagittal plane motion have found passive intervertebral motion to be more uneven in patients with chronic nonspecific low back pain (CNSLBP) than healthy controls, but the mechanisms are unclear. This study aimed to compare patients with CNSLBP with a matched group of pain-free controls for intervertebral restraint during passive recumbent bending. METHODS: Seventeen patients with CNSLBP and minimal disc degeneration who had quantitative fluoroscopy investigations were matched to 17 healthy controls from a database acquired using the same imaging

Continuous passive motion as an alternative treatment for iatrogenic hallux limitus.

The use and effect of continuous passive motion (CPM) was evaluated for 10 patients suffering from iatrogenic hallux limitus. All patients had previous hallux valgus corrective surgery. After their initial evaluation, patients were instructed to begin CPM therapy at home for a minimum of 4 hr. a day and were instructed to increase range of motion (ROM) as tolerated. CPM was used for 4 weeks. ROM was measured on day 0, 28, 48, 90. There were significant increases in mean extension (p < 0.025) and mean flexion (p < 0.05). All patients reported a decrease in pain and stiffness of the first metata

Passive exercise to improve quality of life, activities of daily living, care burden and cognitive functioning in institutionalized older adults with dementia - a randomized controlled trial study protocol.

BACKGROUND: Dementia affects cognitive functioning, physical functioning, activities of daily living (ADLs), and quality of life (QOL). Pharmacological treatments to manage, cure or prevent dementia remain controversial. Therefore development of non-pharmacological approaches to prevent, or at least delay the onset and progression of dementia is urgently needed. Passive exercise is proposed to be such a non-pharmacological alternative. This study primarily aims to investigate the effects of three different forms of passive exercise on QOL and ADLs of institutionalized patients with dementia. T

Post-operative rehabilitation after surgical repair of the rotator cuff.

Today advances in techniques and materials for rotator cuff surgery allow the repair of a large variety of types or extensions of cuff lesions in patients from a wide range of age groups who have different kinds of jobs and participate in different kinds of sports, and who have widely different expectations in terms of recovery of functions and pain relief. A large number of factors must be taken into account before implementing a rehabilitation protocol after rotator cuff surgery. These mainly include the technique (materials and procedure) used by the surgeon. Moreover, tissue quality, retra
