Extruded lumbar osseous endplate causing long-term radiculopathy in an adult: an endoscopic excision.

In this report, we described an adult case that had a long-term radiculopathy due to an extruded osseous endplate of the lumbar spine at the L5-S1 intervertebral disc level. The osseous material inside the extruded material was not absorbed, and it had continued compressing the nerve root for one year. Endoscopically, the bony fragment was successfully removed. After the surgery, the patient's symptom disappeared, and neurological deficits became normalized. In conclusion, we propose that surgical intervention should be taken into account for the treatment of HNP, when the extruded material co

Adrenaline Facilitates Synaptic Transmission by Synchronizing Release of Acetylcholine Quanta from Motor Nerve Endings.

The long history of studies on the effect of catecholamines on synaptic transmission does not answer the main question about the mechanism of their action on quantal release in the neuromuscular junction. Currently, interest in catecholamines has increased not only because of their widespread use in the clinic for the treatment of cardiovascular and pulmonary diseases but also because of recent data on their possible use for the treatment of certain neurodegenerative diseases, muscle weakness and amyotrophic sclerosis. Nevertheless, the effects and mechanisms of catecholamines on acetylcholine

Productive and non-productive binding of botulinum neurotoxin A to motor nerve endings are distinguished by its heavy chain.

Botulinum neurotoxin type A, a di-chain protein produced by Clostridium botulinum and responsible for botulism, blocks acetylcholine release from peripheral nerves by binding to the terminals, undergoing internalization and proteolyzing a protein essential for exocytosis. As butolinum neurotoxin is being used clinically for the treatment of dystonias and certain spasticities, deciphering the details of its specific targeting to cholinergic nerve endings has assumed great importance. Thus, interaction of butolinum neurotoxin type A with murine motor nerve terminals-a prime target in vivo-was in

Immunohistochemical study of utrophin and dystrophin at the motor end-plate in myasthenia gravis.

We studied the densities of utrophin and dystrophin at the motor end-plates of patients with myasthenia gravis (MG) using immunohistochemical analysis. The densities were compared with those found in patients with amyotrophic lateral sclerosis, Lambert-Eaton myasthenic syndrome and normal controls. Utrophin was reduced at the motor end-plates of MG patients, in association with a reduction of alpha-bungarotoxin binding sites. In contrast, the density of dystrophin at the motor end-plate of MG patients was not significantly different from that found in the controls. We conclude that, at the mot

Effects of famotidine at the mouse neuromuscular junction compared with those of cimetidine and ranitidine.

The effects of some histamine H2-receptor antagonists on the cholinergic system have been evaluated at the mouse end-plate. Previous data revealed interactions at both pre- and postsynaptic sites for cimetidine and ranitidine. The present work shows the effects of the same two drugs, and of famotidine, on some parameters related to the neuromuscular transmission. Ranitidine potentiates the amplitude of the spontaneously occurring miniature end-plate potentials. Famotidine reduces the same parameter. A common effect achieved with higher concentrations of the three drugs is the reduction of the

Development and characterisation of an experimental recurrent laryngeal nerve injury model for the study of viral gene therapy.

OBJECTIVES: To develop and characterise an experimental model of recurrent laryngeal nerve injury for the study of viral gene therapy. METHODS: Twenty rats underwent unilateral recurrent laryngeal nerve injury. After vocal fold mobility was observed, larynges were serially sectioned, and immunohistochemical techniques were employed to stain for neurofilament and motor endplates in order for a blinded investigator to determine the percentage of nerve-endplate contact, as a histological indicator of an intact neuromuscular connection. RESULTS: All animal procedures resulted in complete, ipsila

Acute late sepsis attenuates effects of a nondepolarizing neuromuscular blocker, rocuronium, by facilitation of endplate potential and enhancement of membrane excitability in vitro.

BACKGROUND: Sepsis attenuates the muscle-relaxing effects of nondepolarizing neuromuscular blockers. The authors investigated the effects of acute late sepsis on neuromuscular transmission and neuromuscular actions of rocuronium to clarify the mechanisms by which sepsis attenuates the effects of nondepolarizing neuromuscular blockers. METHODS: Sepsis was induced by cecal ligation and puncture operation. Endplate potentials, acetylcholine potentials, and electrotonic potentials were recorded from the motor endplates of isolated diaphragms from acute late septic and nonseptic rats. RESULTS: (1

Neuromuscular Junction Morphology and Gene Dysregulation in the Wobbler Model of Spinal Neurodegeneration.

Amyotrophic lateral sclerosis (ALS) is the most common adult-onset neuromuscular disease for which there is currently no effective treatment. The progression of ALS includes loss of motor neurons controlling the voluntary muscles, with much of this loss occurring at the neuromuscular junction. In an effort to better understand changes at the neuromuscular junction, we utilized the wobbler mouse model of motor neuron loss. We examined biceps and end plate morphologies and monitored selected factors involved in end plate function. Structural volumes were determined from 3D reconstructions that w

Comparison of changes in AChE activity in the brain of the laboratory rat after soman and tabun intoxication.

The aim of this study was to compare changes in activity of acetylcholinesterase (AChE) in the brain and motor endplates of rat after administration of soman and tabun. We took brain and diaphragm from laboratory rats administered a median lethal dose (LD(50)) of soman or tabun. Enzyme activity of AChE was studied in selected structures of brain and in motor endplates in the diaphragm. Histochemical detection of AChE by Karnovski and Roots with simultaneous histochemical detection of alkaline phosphatase in case of brain sections was used. The highest activity of AChE in the control group was

Maturation of fast and slow motor units during synapse elimination in the rabbit soleus muscle.

In adult mammalian skeletal muscles, fast and slow muscle fibers are selectively innervated by single inputs from corresponding motor neuron types, giving rise to fast and slow motor units. At birth, however, muscle fibers are polyinnervated, and connections between motor neurons and muscle fibers are not as specific as those found in adults. Excess synapses are removed during the first few postnatal weeks. In addition to changes in the degree of polyinnervation, motor unit types undergo maturation in their contractile properties. In this study, we have investigated the maturation of motor uni

IgM monoclonal antibody against terminal moiety of GM2, GalNAc-GD1a and GalNAc-GM1b from a pure motor chronic demyelinating polyneuropathy patient: effects on neurotransmitter release.

We describe a patient with a pure motor chronic demyelinating polyneuropathy with an IgM monoclonal component showing anti-GM2, GalNAc-GD1a and GalNAc-GM1b reactivity whose common epitope appears to be -[GalNAcbeta1-4Gal(3-2alphaNeuAc)beta1]. We used intracellular recording to study how IgM from this patient affected neurotransmitter release in the mouse diaphragm in vitro. Adding serum (and specifically, the purified monoclonal IgM component) blocked the nerve-evoked response in both quantal content and evoked endplate potential (EPP) amplitude in a complement-independent and reversible manne

The ultrastructure of cat fusimotor endings and their relationship to foci of sarcomere convergence in intrafusal fibres.

1. Six muscle spindle poles, five from experiments in which foci of sarcomere convergence had been observed during stimulation of fusimotor axons, were serially sectioned for light and electron microscopy. Every somatic motor terminal was studied in ultrathin sections at several levels.2. In all six poles static gamma axons, or presumed static gamma axons, supplying the static bag(2) fibre and/or chain fibres had no terminations on the dynamic bag(1) fibre. In five poles, the dynamic bag(1) fibre was selectively innervated by dynamic gamma or beta axons save in one case where a dynamic gamma a

Segmentation of the mouse fourth deep lumbrical muscle connectome reveals concentric organisation of motor units.

Connectomic analysis of the nervous system aims to discover and establish principles that underpin normal and abnormal neural connectivity and function. Here we performed image analysis of motor unit connectivity in the fourth deep lumbrical muscle (4DL) of mice, using transgenic expression of fluorescent protein in motor neurones as a morphological reporter. We developed a method that accelerated segmentation of confocal image projections of 4DL motor units, by applying high resolution (63×, 1.4 NA objective) imaging or deconvolution only where either proved necessary, in order to resolve axo

Acceleration by stannous ion of the evoked release of transmitter from motor nerve endings in the frog.

Stannous ion (Sn2+, 30 microM) increased the amplitude of endplate potential (EEP) in the frog sartorius muscle, although the amplitude of miniature endplate potential (MEPP) or acetylcholine potential evoked by iontophoretic application of acetylcholine was unchanged. Sn2+ (10-100 microM) dose-dependently increased the quantal content of the EPP. MEPP frequency was not altered by 30 microM Sn2+. These findings indicate that Sn2+ may increase the EPP as a result of acceleration of the transmitter release evoked by nerve impulses.

Neostigmine myopathy is a calcium ion-mediated myopathy initially affecting the motor end-plate.

Morphological techniques were used to determine the acute and chronic effects of neostigmine on rat muscles. Transient calcium deposits, eliminated by prior treatment of sections with ethyleneglycol bis (aminoethylether) tetracetate (EGTA), were independent of fiber type and found at sites corresponding to neostigmine-induced focal lesions. The dimension and number of focal lesions and calcium deposits gradually decreased with chronic drug treatment. Size, shape, and density of the calcium deposits varied. Alterations in the motor nerve terminal, synaptic space, and junctional fold persisted e

[Histochemical study of the effect of phosphoroorganic esters Z-50 (fenchlorfos) and Z-51 (bromophos) on cholinesteras activity in the course of the muscle phase in experimental trichinellosis in mice].

The studies were carried out on mice between the 14th and 19th days, i.e., in the period of maximum penetration of T. spiralis larvae in the host's muscles. Phosphoro-organic esters were given orally to experimental animals in oil solution, at the following doses: Z-50 -- 150 mg per kg of body weight and Z-51 -- 100 mg per kg of body weight. The influence of esters on the activity of cholinesterases was investigated with the Koelle-Friedenwald method, modified by Gomori. The aim of the study was to establish the joint activity of the migrating Trichinella larvae and phosphoro-organic esters on

Assessment of Lumbar Muscles Activation in Patients with Unilateral Myofascial Pain Syndrome Based on High-density Surface Electromyography: A Preliminary Study.

Myofascial pain syndrome (MPS) is a chronic musculoskeletal pain characterized by myofascial trigger points (MTrPs), which are related to dysfunctional motor endplate. However, the changes in neuromuscular functions incurred by the MTrPs are still not well understood. High-density surface electromyography (HD-sEMG) provides a non-invasive method to identify motor endplates and quantify the muscle activity distribution. This study aimed to analyze the differences in lumbar muscle activity distribution between patients with MPS and healthy individuals during an isometric trunk extension task and
