A preventive approach to oral self-mutilation in Lesch-Nyhan syndrome: a case report.

Lesch-Nyhan syndrome is a rare X-linked recessive disorder of purine metabolism, caused by complete absence of the enzyme hypoxanthine-guanine phosphoribosyl transferase. Persons affected with this incurable disease are developmentally and physically delayed, and suffer from self-injurious behavior. The most typical feature results in partial or total destruction of perioral tissues. The purpose of this paper was to discuss a case of Lesch-Nyhan syndrome with self-mutilative behavior and lip injuries, including some suggestions of a preventive approach avoiding the extraction of teeth. A soft

Non-concussive impacts, cognition, and the menstrual cycle: a proof-of-concept study of elite female footballers.

Evidence indicates females may be more susceptible to sports-related concussion with worse and prolonged symptom severity according to menstrual cycle phase. We investigated whether menstrual cycle phases influence non-concussive heading kinematics in elite female footballers, and whether these impacts affect an athlete's cognition. Five eumenorrheic elite female footballers (Mage = 23 ± 4 years) participated in a 16-week proof-of-concept study wherein menstrual cycle phases were tracked, and cognition was monitored. Participants performed a weekly controlled heading drill by heading a ball th

Head Acceleration Events Sustained During High School Wrestling Matches: Grappling With Differences Between Male and Female Adolescents.

Wrestling is a popular high school sport despite high injury and concussion rates. Instrumented mouthguards can reliably measure head acceleration events (HAEs) accrued by wrestlers and may highlight potential sex differences; important considering the higher concussion rates among female athletes. The purpose of this study was to measure HAEs accrued in high school wrestling matches and compare frequency, magnitude, and head impact location between female and male wrestlers. Forty-five (16.5 [0.8] y) wrestlers (20 females; 25 males) wore instrumented mouthguards during high school matches. A

Contact Role and Tackle Characteristics Shape Head Acceleration Exposure in Male Community Rugby: A Cohort Study Utilising Instrumented Mouthguards.

BACKGROUND: Rugby Union has attracted increased scrutiny because of concerns over head acceleration events (HAEs), particularly regarding their frequency, severity and potential long-term health implications. While substantial efforts by governing bodies have focused on reducing head impact risks through education, regulatory changes and the introduction of instrumented mouthguards, limited data exist for the community rugby context, especially across different age grades and playing positions. OBJECTIVE: We aimed to quantify HAE across playing positions, age grades and contact phases in comm

Assessing Head Acceleration Events in Female Community Rugby Union Players: A Cohort Study Using Instrumented Mouthguards.

BACKGROUND: The rapid growth of women's rugby union has underscored the need for female-specific player welfare protocols, particularly regarding the risk of head injuries. Instrumented mouthguards (iMGs) play a vital role in gathering comprehensive data on head acceleration events (HAEs), including their frequency, magnitude, and spatial distribution during games and training. By doing so, iMGs offer valuable context for circumstances in women's matches that may increase player risk. OBJECTIVES: The study aimed to contextualize HAEs in female community rugby players using instrumented mouthg

Development of a Low-Power Instrumented Mouthpiece for Directly Measuring Head Acceleration in American Football.

Instrumented mouthpieces (IM) offer a means of measuring head impacts that occur in sport. Direct measurement of angular head kinematics is preferential for accuracy; however, existing IMs measure angular velocity and differentiate the measurement to calculate angular acceleration, which can limit bandwidth and consume more power. This study presents the development and validation of an IM that uses new, low-power accelerometers for direct measurement of linear and angular acceleration over a broad range of head impact conditions in American football. IM sensor accuracy for measuring six-degre

[Evaluation of the bite block wedged between the maxillary and mandibular molars].

The purpose of this study was to compare our original bite block (T-X Block) wedged between the maxillary and mandibular molars, with the standard gum bite block, in 200 patients whose tracheas were intubated. During emergence from isoflurane anesthesia, no trouble occurred in T-X Block group (n = 100). On the other hand, lip damage and ejection of the bite block were found in 11 and 10 cases, respectively, in gum bite block group (n = 100). As another study, an opening between the maxillary and mandibular incisor edges was measured with T-X Block placed in twenty patients under general anesth

Sensitivity of brain injury criteria to anthropometric scaling assumptions in instrumented mouthguard data.

Instrumented mouthguards (iMGs) enable in-game measurement of head acceleration events (HAEs) in contact sports, forming the basis for brain injury criteria (BICs). However, most commercial systems assume a 50th-percentile male head model for center-of-mass (COMh) transformations, which may bias risk estimates for athletes of different sex and body size. To quantify how anthropometric scaling assumptions influence BICs derived from iMG data, we analyzed 15,237 video-verified HAEs from 572 community rugby players (ages 10-38 years; 34-142 kg). Three scaling approaches were compared: (i) a one-s

Head Impacts in the Top 1% by Peak Linear Acceleration and/or Work Cause Immediate Concussion Signs and 'Check Engine' Responses in Military Service Members and Civilian Athletes.

PURPOSE: Historically, head impact monitoring sensors have suffered from single impact measurement errors, leading to their data described by clinical experts as 'clinically irrelevant.' The purpose of this study was to use an accurate impact monitoring mouthguard system and (1) define head impact distributions for military service members and civilians and (2) determine if there was a dose-response relationship between accurately measured head impact magnitudes versus observations of concussion signs. METHODS: A laboratory-calibrated commercial impact monitoring mouthguard system, along with

The use of a special mouthguard in the management of oral injury self-inflicted by a 4-year-old child.

This paper describes the management of self-inflicted trauma by a 4-year-old boy who was mentally retarded as a result of encephalitis in infancy. There was severe damage to his lower lip, as well as a moderate degree of injury to the skin and nail of his right thumb. Preliminary intervention was made with a mouthguard using silicone elastomeric positioner material, which soon proved ineffective because of poor retention. The problem was overcome successfully through use of a special appliance in which a heat-cured acrylic base was combined with a superstructure made of silicone positioner mat

Lingual Pressure During Dingman-Assisted Cleft Palate Repair: An Investigatory Case Series.

BACKGROUND: A variety of palatoplasty techniques are used for cleft palate repair, almost all involving a device called the Dingman-Grabb mouth gag ("Dingman") to push the tongue and cheeks out of the way of the operating field. There have been numerous case reports of complications hypothesized to be due to the gag, such as lingual edema and airway obstruction. The purpose of this study is to introduce a technique for monitoring lingual pressure during Dingman-assisted palatoplasty and present preliminary data from a small series. METHODS: Patients with a cleft palate who underwent palatopla

Mandibular subluxation stabilized by mouthpiece for distal internal carotid artery exposure in carotid endarterectomy.

The standard approach for carotid endarterectomy cannot provide adequate exposure of the distal internal carotid artery in the presence of high cervical carotid bifurcation or high plaque. Limited accessibility of the distal internal carotid artery has resulted in the development of various operative techniques. Mandibular subluxation is the most simple and least invasive technique, but it does require invasive maneuvers, such as wiring, to stabilize the mandible. We use a mouthpiece made by the dentist to stabilize the mandible in the physiologic subluxated position. This technique provides a
