A human ciliopathy reveals essential functions for NEK10 in airway mucociliary clearance.

Mucociliary clearance, the physiological process by which mammalian conducting airways expel pathogens and unwanted surface materials from the respiratory tract, depends on the coordinated function of multiple specialized cell types, including basal stem cells, mucus-secreting goblet cells, motile ciliated cells, cystic fibrosis transmembrane conductance regulator (CFTR)-rich ionocytes, and immune cells1,2. Bronchiectasis, a syndrome of pathological airway dilation associated with impaired mucociliary clearance, may occur sporadically or as a consequence of Mendelian inheritance, for example i

Mucociliary clearance and submucosal gland secretion in the ex vivo ferret trachea.

In many species submucosal glands are an important source of tracheal mucus, but the extent to which mucociliary clearance (MCC) depends on gland secretion is unknown. To explore this relationship, we measured basal and agonist-stimulated MCC velocities in ex vivo tracheas from adult ferrets and compared the velocities with previously measured rates of ferret glandular mucus secretion (Cho HJ, Joo NS, Wine JJ. Am J Physiol Lung Cell Mol Physiol 299: L124-L136, 2010). Stimulated MCC velocities (mm/min, means ± SE for 10- to 35-min period poststimulation) were as follows: 1 μM carbachol: 19.1 ±

[The effect of theophylline on the mucociliary clearance function in ventilated intensive care patients].

Mucociliary clearance represents an important protective mechanism of the upper and lower respiratory tracts whereby inhaled particles and micro-organisms are removed from the tracheobronchial system. In incubated intensive care unit (ICU) patients, impaired ciliary function and mucus transport are associated with pulmonary complications [9]. Some authors have shown that theophylline increases mucus transport in healthy subjects and patients with chronic bronchitis [8, 16, 31, 36]. However, other investigators have found unconvincing or conflicting results [4, 20, 25]. Therefore, we studied th

Influence of tasuldine and acetylcysteine on mucociliary clearance in patients with chronic airways obstruction.

In a single-blind intraindividual cross-over comparative study the influence of two systemic-mucolytic agents, 2-(pyridyl-3-methylthio)pyrimidine (tasuldine, HE 10004-succinate) vs. N-acetyl-L-cysteine (acetylcysteine) on the mucociliary clearance and lung function was investigated in 15 patients with chronic obstructive respiratory disease. The overall treatment period was 14 days with the drug being changed after 7 days in each group. Under tasuldine and acetylcysteine treatment a tendency to an improvement of mucociliary clearance was observed compared with the control pointing to an increa

Selected methods for the evaluation of the effects of drugs on mucus viscoelasticity and mucociliary clearance.

In order to rationally test the efficacy of mucolytic and mucospissic compounds, their effects upon mucus viscoelasticity, ciliary beat frequency (c.b.f.) and total mucociliary transport rate (t.t.r.) must all be determined. Methods are described which allow the measurement of the viscoelastic modulii of small (10 microL) quantities of mucus, while c.b.f. and t.t.r. are determined using the frog palate model.

Repeated stress reduces mucociliary clearance in animals with chronic allergic airway inflammation.

We evaluated if repeated stress modulates mucociliary clearance and inflammatory responses in airways of guinea pigs (GP) with chronic inflammation. The GP received seven exposures of ovalbumin or saline 0.9%. After 4th inhalation, animals were submitted to repeated forced swim stressor protocol (5x/week/2 weeks). After 7th inhalation, GP were anesthetized. We measured transepithelial potential difference, ciliary beat frequency, mucociliary transport, contact angle, cough transportability and serum cortisol levels. Lungs and adrenals were removed, weighed and analyzed by morphometry. Ovalbumi

Respiratory drugs influence lung mucociliary clearance in central and peripheral ciliated airways.

Radiolabelling of mucus and gamma-camera imaging techniques were utilized to differentiate mucociliary function in large central and small peripheral bronchi of man. Lung mucus clearance was analyzed for the entire right lung field and a peripheral region, which was representative of mucus clearance from airways distal to lobar bronchi. On control days, healthy subjects breathed monodisperse particles, average size 4.19 micrometers, and achieved central patterns of deposition with mouth breathing at rest. Matched deposition patterns were achieved on treatment days when isoetharine or isoproter

Impaired mucociliary clearance in bronchiectasis: the result of a differentiation-delayed airway epithelium?

Impaired mucociliary clearance (MCC) is a potential targetable feature of bronchiectasis; however, it is difficult to model outside of the patient, and thus, there remains little mechanistic evidence to suggest what drives it. To unravel the complexities associated with assessing mucociliary clearance we have established a four-parameter assessment, the Newcastle Cilia and Mucus (NewCAM) score, which quantifies active ciliated area, cilia beat frequency, apical particle velocity, and mucus viscosity in vitro. In addition, single-cell RNA (scRNA) sequencing was used to assess the breakdown of e

Development of a computational fluid dynamics model for mucociliary clearance in the nasal cavity.

Intranasal drug delivery has attracted significant attention because of the opportunity to deliver systemic drugs directly to the blood stream. However, the mucociliary clearance poses a challenge in gaining high efficacy of intranasal drug delivery because cilia continuously carry the mucus blanket towards the laryngeal region. To better understand mucus flow behaviour on the human nasal cavity wall, we present computational model development, and evaluation of mucus motion on a realistic nasal cavity model reconstructed from CT-scans. The model development involved two approaches based on th

Mucociliary clearance techniques for treating non-cystic fibrosis bronchiectasis: Is there evidence?

Non-cystic fibrosis bronchiectasis (nCFb) is an acquired condition of variable etiology. An impaired mucociliary clearance seems to be one of the mechanisms behind nCFb, and treatment involves antibiotics, mucoactive agents, and airway clearance techniques (ACTs). Traditional ACTs have four components: postural drainage, percussion, vibration of the chest wall, and coughing. Reviewing the international medical literature on the use of ACTs for patients with nCFb from 1989 to the present day, we retrieved 93 articles, of which 35 met our selection criteria for this analysis. We reviewed active

Impaired mucociliary clearance in allergic rhinitis patients is related to a predisposition to rhinosinusitis.

Although mucociliary clearance has been shown to be impaired in patients with allergic rhinitis and chronic rhinosinusitis, its exact role in relation to a predisposition to rhinosinusitis is unknown. To investigate this possible association, we conducted a prospective study of 125 patients with allergic rhinitis. Of this group, 23 patients were classified as being sinusitis-prone based on their history of antibiotic consumption for the treatment of rhinosinusitis; the remaining 102 patients were deemed to be not sinusitis-prone. The saccharine test was used to evaluate mucociliary clearance i

The effect of endoscopic sinus surgery on mucociliary clearance in patients with chronic sinusitis.

The saccharin test was used to measure mucociliary clearance in 50 patients with symptoms of chronic sinusitis. Samples of the nasal mucosa were also examined under transmission electron microscopy before and after functional endoscopic sinus surgery (FESS). Before surgery, the mean saccharin clearance (ST) was 37.0 +/- 15.7 min, with nasal mucosa exhibiting ciliary loss as well as other ultrastructural changes. Three months after surgery, the mean ST had improved to 20.3 +/- 7.5 min and significant regeneration of cilia was observed. It was therefore concluded that FESS had successfully corre

Tomographic Imaging of Mucociliary Clearance Following Maxillary Sinus Augmentation: A Case Series.

Mucociliary clearance (MCC) allows ventilation of graft particles that are displaced through a perforated Schneiderian membrane during maxillary sinus augmentation (MSA). However, it is very rarely confirmed by cone-beam computed tomographic (CBCT) images. It is not yet known how long the dislodged bone graft particles remain in the maxillary sinus or how quickly they are ventilated after MSA. The purpose of these case reports is to introduce tomographic imaging of ventilation of bone graft particles displaced through a perforated Schneiderian membrane after MSA. Four patients, who needed impl

[Respiratory mucus: Exploring its viscoelastic properties and their relationship with mucociliary clearance abnormalities].

Following an introduction on the structure and function of the respiratory mucus, we describe the viscoelastic properties of mucus, a key determinant of mucociliary clearance, along with the types of measurement devices most commonly used in laboratories. More specifically, this review presents macro- and micro-rheological methods for characterization of the respiratory mucus by means of small sample volumes. We discuss how study of the viscoelastic properties of mucus facilitates understanding of mucus abnormalities in respiratory muco-obstructive diseases and paves the way for the developmen
