Multi-institutional development and testing of attention-enhanced deep learning segmentation of thyroid nodules on ultrasound.

PURPOSE: Thyroid nodules are common, and ultrasound-based risk stratification using ACR's TIRADS classification is a key step in predicting nodule pathology. Determining thyroid nodule contours is necessary for the calculation of TIRADS scores and can also be used in the development of machine learning nodule diagnosis systems. This paper presents the development, validation, and multi-institutional independent testing of a machine learning system for the automatic segmentation of thyroid nodules on ultrasound. METHODS: The datasets, containing a total of 1595 thyroid ultrasound images from 5

The pediatric solid organ transplant experience with COVID-19: An initial multi-center, multi-organ case series.

The clinical course of COVID-19 in pediatric solid organ transplant recipients remains ambiguous. Though preliminary experiences with adult transplant recipients have been published, literature centered on the pediatric population is limited. We herein report a multi-center, multi-organ cohort analysis of COVID-19-positive transplant recipients ≤ 18 years at time of transplant. Data were collected via institutions' respective electronic medical record systems. Local review boards approved this cross-institutional study. Among 5 transplant centers, 26 patients (62% male) were reviewed with a me

The perception of the impacts of climate change in communities surrounding Lake Titicaca, Peru-Bolivia.

BACKGROUND: Climate change is increasingly transforming socio-ecological systems in high-Andean regions, particularly in communities surrounding Lake Titicaca, where livelihoods depend heavily on climate-sensitive activities such as agriculture, fisheries, livestock, and tourism. Despite growing environmental pressures, limited evidence exists regarding how local populations and institutional actors perceive climate-related impacts and adaptive responses in this transboundary region. This study analyzes climate change perceptions, perceived impacts, and adaptation capacities in communities loc

'Maybe you should have a bowl of ice cream': Inequities in patient-clinician interactions among individuals with chronic low back pain.

Prior literature has shown inequities in patient-clinician interactions experienced by individuals with chronic low back pain (CLBP) with underlying pain-related stigmatization and invalidation. Yet, there is a notable gap in understanding how these inequities intersect with multiple systems of oppression, including racism and sexism. This qualitative study examined intersectional perspectives and experiences of patient-clinician interactions among individuals with CLBP. Semi-structured interviews were conducted after the participants engaged in simulated enhanced or limited patient-clinician

Exploring national support mechanisms for European reference network centres.

Integrating European Reference Networks (ERNs) into national health systems is crucial for rare disease care but remains challenging due to inconsistent support structures across countries. This study used a 2024-2025 multi-stakeholder survey of ERN clinical centre leads, hospital administrators, and national health authorities (JARDIN Joint Action, Work Package 9) to evaluate national support for ERN centres across six domains: staffing, education, formal recognition, funding, quality assurance, and strategic planning. Survey results revealed inconsistent national legal frameworks for recogni

Assessing the role of plan complexity and target geometry through multi-institutional gel based end-to-end QA in multi-focal single isocenter stereotactic radiosurgery.

BACKGROUND: Single-isocenter stereotactic radiosurgery enables efficient treatment of multiple brain metastases (SI-MBM SRS), but demands high geometric accuracy. Plan complexity metrics are increasingly used as indicators for quality assurance (QA) performance, however their applicability to SRS remains uncertain, particularly in the context of multi-institutional variability. PURPOSE: This study evaluated the relationship between plan complexity, target geometry, and end-to-end dosimetric QA outcomes for SI-MBM SRS across multiple institutions. METHODS: Forty-two SI-MBM SRS plans from diff

Bridging worlds: polycentric collaboration and the integration of traditional governance in Samoa's disaster management.

Effective multi-agency collaboration (MAC) is essential for disaster risk reduction; however, its implementation remains profoundly challenging in contexts where modern state structures intersect with traditional governance systems. Grounded in polycentric governance theory, this study investigates the barriers to MAC in Samoa, a Small Island Developing State characterized by the coexistence of formal agencies and the indigenous fa’a-Samoa governance system. Using a qualitative case study design, semi-structured interviews were conducted with 15 senior officials across Samoa’s disaster managem

Clinical quality improvement in a multihospital system: the voluntary hospitals of America/Pennsylvania experience.

Sharing of patient outcome data among hospitals hastened the evolution of continuous quality improvement at Voluntary Hospitals of American/Pennsylvania (VHA/PA) to a focus on learning from the best: clinical benchmarking. Analysis of VHA/PA MedisGroups comparative outcome data showed significant variation. In the first study, variations from 3 to 25% normal appendectomy rates were noted. A second study on primary and secondary cesarean section rates again showed variation in outcomes. A study of the benchmark hospitals showed characteristics, specific to those institutions, that resulted in e

Prognostic Factors in Primary Head and Neck Cutaneous Squamous Cell Carcinoma: A Large-Scale, Multi-Institutional Competing-Risks Analysis.

BACKGROUND: Current staging systems for head and neck cutaneous squamous cell carcinoma (HNCSCC), such as AJCC8 and BWH, often fail to adequately stratify high-risk patients, particularly due to the exclusion of important biological and host factors. This study aimed to identify independent prognostic factors for HNCSCC outcomes in the largest multi-institutional retrospective series to date. METHODS: De-identified data from 8,610 primary HNCSCC tumors, treated with curative intent surgery across multiple international centers, were analyzed. Outcomes included local recurrence (LR), locoregio

Structured review of neonatal deaths in a managed care organisation.

We sought to quantify neonatal mortality (< 28 days) in a 10-hospital system, determine what proportion was associated with suboptimal neonatal care and make recommendations on how neonatal mortality rates (NMRs) could be used in quality improvement efforts. Deaths were identified using electronic linkage to the State of California Death Certificate Tapes. Individual fatalities were reviewed by a minimum of two physicians who did not care for the infant. Deaths were classified as either being associated with suboptimal care or not. For deaths where suboptimal care was an issue, emphasis was on

How do we leverage data through replication and warehousing to meet blood collection and transfusion service needs.

BACKGROUND: With the widespread adoption of Blood Establishment Computer Systems and other Blood Collection and Transfusion Service (BCTS) clinical information systems (CIS), electronic blood donor, product, and patient data are now routinely required for clinical, regulatory, operational, and quality needs. That data are often not readily accessible for such secondary use within CIS databases, particularly for applications with significant data availability requirements such as machine learning and artificial intelligence. Data replication provides one avenue by which CIS data can be made mor

YOLO26x-based automated fracture detection on radiographs and its impact on radiologist performance: A multi-reader multi-case study.

BACKGROUND AND AIMS: Missed fractures are among the most common diagnostic errors in emergency radiology and may lead to delayed treatment and adverse outcomes. Deep learning-based approaches have shown promise for automated fracture detection; however, many existing models are restricted to specific anatomical regions, operate on downsampled images, or focus primarily on image classification rather than robust localization. This study aimed to develop and validate a YOLO26x-based deep learning model trained using high-resolution input (1280 × 1280 pixels) for detection of appendicular fractur
