Multimorbidity patterns in South Africa: A latent class analysis.

INTRODUCTION: South Africa has the largest burden of HIV worldwide and has a growing burden of non-communicable diseases; the combination of which may lead to diseases clustering in ways that are not seen in other regions. This study sought to identify common disease classes and sociodemographic and lifestyle factors associated with each disease class. METHODS: Data were analyzed from the South African Demographic and Health Survey 2016. A latent class analysis (LCA) was conducted using nine disease conditions. Sociodemographic and behavioral factors associated with each disease cluster were

Treating chronic diseases without tackling excess adiposity promotes multimorbidity.

Few people now reach old age without taking multiple drugs, often attending various clinics, and undergoing secondary or tertiary investigations. Most chronic conditions are, to differing extents, caused or exacerbated by excess adiposity, but weight management is rarely discussed or attempted for patients. Furthermore, progressive symptoms usually attributed to ageing (eg, musculoskeletal pains, fatigue, and breathlessness), and which create considerable health-care demands, can also be attributed to the accumulation of body fat over time. For many symptoms and diseases that are more frequent

Multimorbidity Analysis of 13 Systemic Diseases in Northeast China.

BACKGROUND: Multimorbidity not only affects the quality of patients' lives, but can also bring a heavy economic burden to individuals, families and society. The purpose of this study was to reveal the connections between diseases, especially the important role each disease played in the entire multimorbidity network. METHODS: A total of 1,155,734 inpatients were enrolled through multistage stratified random sampling in Jilin Province in 2017. Categorical variables were compared using the Rao-Scott-χ2 test. Weighted networks were adopted to present the complex relationships of multimorbidity.

Alcohol Use and Multimorbidity Among Individuals Living with HIV.

Alcohol is prevalent among people living with HIV and can lead to multiple comorbid conditions (multimorbidity). The purpose of this study was to examine the relationship between alcohol use history and multimorbidity among people living with HIV. A retrospective cohort study design was conducted at an urban, academic infectious disease clinic in Kentucky. Individuals seeking care between 2010 and 2014 were included. Modified Poisson regression was used to examine the relationship between alcohol use history (never, current, and former use) and multimorbidity (≥ 2 conditions). A total of 949 i

Identifying obesity-related multimorbidity combinations in the United States.

Interest in understanding the effects of multimorbidity on outcomes has increased in recent years. This paper presents the most common obesity-related groupings of multimorbidity in the United States. Using Cerner HealthFacts data, we applied the frequent pattern growth algorithm to identify prevalent multimorbidity groupings of 3 or more diseases (one being obesity) by race using a dataset of 574 172 patients with obesity from all over the United States. We set the minimum prevalence to 10% and identified groupings of ICD10-CM diagnoses that occur in our dataset at or above the minimum preval

Multimorbidity patterns in high-need, high-cost elderly patients.

INTRODUCTION: Patients with complex health care needs (PCHCN) are individuals who require numerous, costly care services and have been shown to place a heavy burden on health care resources. It has been argued that an important issue in providing value-based primary care concerns how to identify groups of patients with similar needs (who pose similar challenges) so that care teams and care delivery processes can be tailored to each patient subgroup. Our study aims to describe the most common chronic conditions and their combinations in a cohort of elderly PCHCN. METHODS: We focused on a cohor

Interferon-related inflammaging links epigenetic age acceleration to multimorbidity.

Chronic systemic inflammation and DNA methylation changes are two major hallmarks of aging, yet their interaction is poorly known. We investigated the relation between circulating inflammatory proteome and epigenetic age acceleration as assessed by DNA methylation in four independent cohorts of different ages and health conditions. Epigenetic age scores known to predict human health span (GrimAge and PhenoAge) were more strongly associated with age-associated inflammatory proteins, frailty, and multimorbidity when compared to epigenetic age scores associated with lifespan (Horvath and Hannum).

Management of brain-heart multimorbidity: a clinical practice guideline.

BACKGROUND: Although brain and heart conditions share overlapping risk factors and commonly co-occur, current cardiac and neurologic clinical guidelines are typically produced within specialty silos. The objective of this guideline from a Canadian Cardiovascular Harmonized National Guideline Endeavour (C-CHANGE) panel is to expand on current cardiovascular guidelines to include evidence from the neurologic and mental health literature, with specific recommendations for providers managing comorbid brain and heart conditions. METHODS: The guideline development panel comprised an Executive Steer

Integrated tuberculosis-multimorbidity management in India: A SWOT analysis.

Tuberculosis multimorbidity is an emerging burden to the healthcare system in India, but the national programmes like the National TB Elimination Programme (NTEP), the National Programme for Prevention and Control of NCDs (NP-NCD), and the National Mental Health Programme (NMHP) are still operating in silos, leading to fragmented and inefficient care. This paper employs a SWOT (strengths, weaknesses, opportunities, threats) analysis to evaluate the integration potential of these programmes within the platform of Ayushman Arogya Mandirs (AAMs). The analysis identifies key strengths, such as NTE

Mapping multimorbidity in undergraduate medical education: a scoping review.

PURPOSE: Multimorbidity is an increasingly common clinical and public health challenge, yet undergraduate medical education (UME) frequently emphasizes single-disease models. Using Cultural Historical Activity Theory (CHAT), this review maps how multimorbidity is conceptualized within UME to inform more deliberate teaching and curriculum design. METHOD: The authors conducted a scoping review to map the existing medical education literature on multimorbidity in the undergraduate setting. Search terms were developed using a Cochrane Multimorbidity Review as a foundation. Five databases were sea

Patient-Centered Prioritization of Health Care Processes for Multimorbidity.

IMPORTANCE: Primary care provides a broad service range, including preventive screenings and chronic disease care. Busy clinicians frequently select which health care processes to address per visit based on time or perceived disease impact, especially for patients with multimorbidity. Little evidence exists for how to prioritize processes by patient-centered health goals. OBJECTIVE: To identify criteria for prioritizing health care processes for patients with multimorbidity and using these criteria, develop recommended priority rankings of health care processes aligned with patient-centered h

Correlational analysis of sarcopenia and multimorbidity among older inpatients.

BACKGROUND: Sarcopenia and multimorbidity are common in older adults, and most of the available clinical studies have focused on the relationship between specialist disorders and sarcopenia, whereas fewer studies have been conducted on the relationship between sarcopenia and multimorbidity. We therefore wished to explore the relationship between the two. METHODS: The study subjects were older patients (aged ≥ 65 years) who were hospitalized at the Department of Geriatrics of the First Affiliated Hospital of Chongqing Medical University between March 2016 and September 2021. Associations between multimorbidity and frailty transitions among older Americans.

BACKGROUND: The associations of multimorbidity patterns with transitions between frailty states remain unclear in older individuals. METHODS: We used data from the National Health and Aging Trends Study 2011-2019. Frailty was measured annually using the Fried frailty phenotype. Multimorbidity patterns at baseline were identified using latent class analysis based on 14 chronic conditions. We used the semi-Markov multi-state model to investigate the influences of multimorbidity characterized by condition counts and patterns on subsequent frailty transitions over follow-ups. RESULTS: Among 9450

[Intestinal motility disorders in multimorbidity: between constipation and diarrhea].

DIAGNOSTIC STRATEGY: The Rome IV criteria for primary chronic obstipation help to distinguish between short-term ailments and a chronic pattern of complaints 1. In general, there are several causes of defecation disorder in the age group of the elderly. Anamnesis and observance of red flags in addition to a geriatric basic assessment make it possible to narrow down the causes. The distinction between low-transit constipation and normal intestinal function can decisively influence the therapeutic consequence. THERAPEUTIC POSSIBILITIES: In the drug intervention, osmotic laxatives, stimulating l

Comparison of count-based and clustering definitions of multimorbidity and their association with prevalence of multimorbidity, health profiles, and mortality: A cohort study of UK Biobank participants.

BACKGROUND: Multimorbidity, the presence of several chronic conditions, is linked to higher mortality and healthcare use and thus poses a major challenge for aging populations. While most studies rely on simple counts of conditions, clustering approaches have been proposed to describe patterns of co-occurring diseases. We aimed to evaluate the extent to which these methodological choices influence prevalence and association with health profiles and mortality. METHODS AND FINDINGS: Using UK Biobank baseline data (n = 474,397), collected between 2006 and 2010, we compared six count-based defini
