The infrahyoid myocutaneous flap (IHF) is used in reconstructive procedures for small- and medium-sized defects of the head and neck. The IHF is a versatile and technically straightforward reconstructive option; however, it has several limitations, including less reliable skin-paddle vascularity, size constraints, and a relatively short pedicle length. We report a case of lumbar hernia in a patient with carcinoma in right breast who underwent mastectomy and reconstruction with a latissimus dorsi myocutaneous flap. Although rare, the diagnosis of lumbar hernia should be kept in mind in cases where latissimus dorsi myocutaneous flap is done to differentiate it from seroma (where aspiration is a form of management) and avoid potential complications such as bowel perforation. Treatment options are individualized, and can range from fascia closure, mesh reinforcement to minimally invasive laparoscopic and endoscopic techniques.

BACKGROUND: The vertical rectus abdominis myocutaneous (VRAM) flap has emerged as the workhorse flap for perineal and pelvic reconstruction. The authors aimed to evaluate outcomes of the VRAM flap over a 20-year period and the role of mesh abdominal wall reinforcement following VRAM flap-based reconstruction.

BACKGROUND: Pelvic organ prolapse (POP) is a significant issue requiring surgical correction in 19% of the female population by age 85 years. Complications of POP, especially in women who have undergone hysterectomy, include vaginal evisceration-a serious complication that carries high morbidity and mortality rates. Rarely, vaginal evisceration occurs after colpocleisis.
CASE: A 69-year-old female with recurrent vaginal evisceration following colpocleisis underwent surgical repair using a vertical rectus abdominis myocutaneous (VRAM) flap.

Deep sternal wound infections are potentially life-threatening complications after cardiac operations because they can spread into the mediastinum and cause postoperative morbidity and mortality. Surgical treatment of hypopharyngeal cancer is indicated in advanced stages and recurrent/persistent disease after conservative regimens. To assess whether locking-screw titanium plates (UniLOCK) and pedicled pectoralis major myocutaneous flaps are a valid alternative to complex reconstruction with bony free flaps in poor prognosis or poor performance status oncological patients with mandibular defects, a retrospective evaluation has been made of outcomes in 27 consecutive cases. Post-surgical maxillary defects have recently been reconstructed by microvascularized free flaps. However, few reports have evaluated the resulting speech function. This study compared the speech intelligibility of four maxillectomy patients who underwent reconstruction by rectus abdominis myocutaneous flaps (RAMCF) with that of four non-reconstructed patients who were treated with split-skin grafts and prostheses, to explore ways of achieving better speech function. Speech function was assessed by a Japanese language speech intelligibility test. Intelligibility scores ranged from 57.3% to 75.

A patient sustained a compound fracture of the elbow resulting in a deep soft tissue defect with exposed bone, joint and nerve. The flexor carpi radialis (FCR) muscle was incorporated within the radial forearm island flap to cover the defect. Wrist flexion was not compromised by this procedure. BACKGROUND: The pedicled lower trapezius myocutaneous flap is generally transferred to the recipient site through a subcutaneous tunnel, and a portion of the flap buried in the tunnel needs to be de-epithelialized. We describe the novel use of a rectus abdominis myocutaneous flap for reconstruction of the female urethra following an oncological pelvic resection. Reconstruction of the female urethra from the bladder neck to external urethral orifice was successfully performed, avoiding the need for a urostomy. To our knowledge, the myocutaneous flap has never been described for complete urethral reconstruction.
