Gastroesophageal intussusception is a rare but clinically significant condition necessitating prompt diagnosis and management. This highlights the importance of early recognition and intervention to prevent severe complications such as oesophageal perforation. Endoscopic assessment and surgical intervention remain the cornerstone of the management, tailored to the clinical severity and surgeon's expertise. The prognosis largely depends on the timeliness of diagnosis and intervention, with early treatment typically resulting in favourable outcomes.

PURPOSE OF REVIEW: To describe the emerging role of transcatheter septal myotomy in the treatment of hypertrophic obstructive cardiomyopathy.
RECENT FINDINGS: Transcatheter septal myotomy was developed to address risk of LVOT obstruction during TMVR, however it has been performed in patients with LVOT obstruction related to hypertrophic cardiomyopathy, in whom it improves symptoms, reduces LVOT gradients, and is associated with low incidence of procedural complications. Transcatheter myotomy is a novel approach to address LVOT obstruction related to hypertrophic cardiomyopathy in patients who

Achalasia, characterized by impaired lower esophageal sphincter (LES) relaxation and failed peristalsis, stands out as the most widely recognized primary esophageal motility disorder. It manifests with dysphagia to solid and liquid foods, chest pain, regurgitation, and weight loss, leading to significant morbidity and healthcare burden. Traditionally, surgical Heller myotomy and pneumatic dilation were the primary therapeutic approaches for achalasia. However, in 2009, Inoue and colleagues introduced a groundbreaking endoscopic technique called peroral endoscopic myotomy (POEM), revolutionizin

OBJECTIVE: To report the first successful full-term delivery following Per Oral Endoscopic Myotomy (POEM) performed during pregnancy.
METHODS/BACKGROUND: Achalasia is an esophageal motility disorder characterized by dysphagia, regurgitation, reflux, recurrent vomiting, and weight loss. Achalasia in pregnancy can affect nutritional status of the mother, and subsequently, the child, increasing morbidity and creating potential pregnancy complications. POEM is a novel endoscopic procedure which involves cutting the lower esophageal sphincter to allow food to pass, and is considered a safe and eff

Background and study aim: Peroral endoscopic myotomy (POEM) is a relatively novel minimally invasive technique that is used to treat achalasia and other esophageal motility disorders. We systematically reviewed the medical literature in order to evaluate the safety and efficacy of POEM. Methods: We performed a comprehensive review and meta-analysis of studies published up to March 2016 that reported on clinical outcomes of POEM. Five databases were searched: MEDLINE, EMBASE, Ovid, CINAHL, and Cochrane. Results: A total of 36 studies involving 2373 patients were included in the review. Clinical

INTRODUCTION: Esophageal achalasia is most commonly treated by laparoscopic myotomy. Transesophageal approaches using flexible endoscopy have recently been described. We hypothesized that using techniques and flexible instruments from our NOTES experience through a small cervical incision would be a safer and less traumatic route for esophageal myotomy. The purpose of this study was to evaluate the feasibility, safety, and success rate of using flexible endoscopes to perform anterior or posterior Heller myotomy via a transcervical approach.
METHODS: This animal (porcine) and human cadaver stu

Surgical myotomy is the mainstay of treatment for oesophageal achalasia. Minimally invasive surgical techniques, if feasible, reduce patient morbidity and mortality. In this study we review our experience of thoracoscopic Heller's myotomy. Thoracoscopic myotomy was undertaken in 9 patients (male = 3; female = 6, mean age = 37). All patients presented with dysphagia of 1 to 8 yr duration. Diagnosis was based on barium swallow and manometry. Two patients had previous dilatations and 1 had a transabdominal myotomy. All patients had a 5 port thoracoscopic technique. Thoracoscopic Heller's myotomy

BACKGROUND: Forty patients (18 women, 22 men) with incapacitating oropharyngeal dysphagia of neurologic origin underwent cricopharyngeal myotomy. The subjective and objective response to myotomy was analyzed retrospectively with a mean postoperative follow-up of 48 months (range 1 to 255 months).
RESULTS: Radiologic evidence of functional obstruction caused by incoordination and incomplete relaxation of the upper esophageal sphincter was significantly reduced. Manometric recordings of resting and closing pressures of the upper esophageal sphincter were also significantly altered by the myotom

Branches of the nerve of Latarjet run with the serosa of the stomach before penetrating the muscularis. Myotomy of the pylorus and esophagus are safe and well established procedures. Myotomy of the lesser curve of the stomach with preservation of the major vessels should denervate the parietal cell mass; this procedure has been investigated in the dog. Pre- and postoperative secretory stimulation was carried out with pentagastrin (0.375--6 micrograms/kg/hr i.v.) and insulin (0.2 units/kg). After myotomy, in response to pentagastrin, there was a marked shift to the right of the dose response cu

We aimed to assess the short-term outcomes of per-oral endoscopic myotomy (POEM) compared with laparoscopic Heller myotomy (LHM) for achalasia through a meta-analysis of nonrandomized comparative studies. We searched PubMed, Embase, Medline, Cochrane Library, and Google Scholar for studies that compared POEM and LHM for achalasia and were published between January 1, 2008 and December 31, 2014. The Methodological Index for Nonrandomized Studies (MINORS) was used to evaluate the quality of the studies. Random- and fixed-effects meta-analytical models were used, and between-study heterogeneity wa

BACKGROUND & AIMS: Peroral endoscopic myotomy (POEM) demonstrated higher efficacy at 1-year follow-up compared with pneumatic dilation (PD) in patients after failed laparoscopic Heller myotomy (LHM). The aim of this study was to evaluate the 5-year efficacy and safety of POEM compared with PD in patients with achalasia after failed LHM.
METHODS: This study reports the long-term follow-up results of a previously published multicenter randomized controlled trial performed in the Netherlands, Belgium, and Italy. Adult patients with achalasia confirmed by esophageal manometry were included when t

BACKGROUND: Peroral endoscopic myotomy (POEM) is an effective treatment for achalasia; however, accurately identifying the distal extent of myotomy remains a technical challenge. Indocyanine green with infrared imaging (ICG-IRI) may facilitate the intraoperative visualization of the esophagogastric junction.
METHODS: We evaluated 39 patients who underwent POEM using ICG-IRI. The primary outcome was the success rate of ICG-IRI, defined as a clear endoscopic visualization of fluorescence at the distal myotomy endpoint. The secondary outcomes included technical success, changes in integrated rel

INTRODUCTION: The surgical gold standard for esophageal motility disorders is laparoscopic Heller myotomy (LHM). Per-oral endoscopic myotomy (POEM) is a less invasive flexible endoscopic alternative. We compare their intermediate-term outcomes.
METHODS: Data were collected on consecutive LHM and POEM patients treated for esophageal motility disorders from January 2015 to December 2019. All patients were invited for a comprehensive workup between 6 and 12 months post-myotomy including symptom evaluation, pH testing off medications, manometry, and esophagogastroduodenoscopy (EGD). Esophageal achalasia is a rare disorder characterized by a failure of the lower esophageal sphincter to relax during swallowing, combined with aperistalsis of the esophageal body. Treatment is not curative, but aims to eliminate the outflow resistance caused by the nonrelaxing lower esophageal sphincter. Current evidence suggests that both laparoscopic Heller myotomy and per oral endoscopic myotomy (POEM) are very effective in the relief of symptoms in patients with achalasia. Specifically, for type III achalasia, POEM may achieve higher success rates. However, POEM is associated to a very hig

BACKGROUND AND AIMS: Peroral endoscopic myotomy (POEM) necessitates proper orientation of the anatomical landmarks throughout the whole procedure to assess the myotomy, which is the main target of POEM. Insufficient myotomy renders the procedure ineffective and may lead to nonresponse or recurrence, although longer myotomy may increase the risk of reflux. The current conventional methods for esophagogastric junction (EGJ) detection are often operator-dependent, time-consuming, and have questionable accuracy. We aimed to provide a novel anatomically based approach for more precise tailoring of

Peroral endoscopic myotomy (POEM) is an intervention for the treatment of achalasia which has gained popularity over the last decade. It's efficacy and invasiveness are comparable to laparoscopic Heller myotomy (LHM). The purpose of this systematic review is to compare POEM to existing therapies. The systematic review was performed following the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines. MEDLINE, EMBASE, Web of Science and Cochrane Libraries were searched using keywords: esophageal achalasia, POEM, LHM, pneumatic dilation (PD), and related terms. Th
