New concepts in median nail dystrophy, onychomycosis, and hand, foot, and mouth disease nail pathology.

Nails are underutilized as diagnostic tools, despite being involved in many dermatologic conditions. This paper explores new concepts in the treatment of median nail dystrophy (MND), onychomycosis, and the nail pathology of hand, foot, and mouth disease (HFMD). A Pubmed database literature search was conducted for MND treatment, onychomycosis treatment, and HFMD nail pathology. Only papers published after January 2008 were reviewed. The results showed that 0.1% tacrolimus ointment can be an effective treatment for MND. Early studies on laser therapy indicate that it is a safe and efficacious t

Evaluation of nail disease in psoriatic arthritis by using a modified nail psoriasis severity score index.

BACKGROUND: The Classification of Psoriatic Arthritis Study Group published new criteria for classifying psoriatic arthritis (PsA) which included nail psoriasis. Our aim was to clarify the clinical importance of nail disease in PsA patients. METHODS: We investigated the types and severity of nail disease by using the modified nail psoriasis severity score index (mNAPSI) in 23 PsA patients and 23 patients with uncomplicated psoriasis. We analyzed the relationships of mNAPSI with nail fold psoriasis, psoriasis area and severity index score, swollen and/or tender joint counts, distal interphalan

Nail abnormalities in upper extremity transplantation: Perspectives and insights from systemic diseases and organ transplantation.

Nail changes following upper extremity transplantation (UET) cannot be overlooked as they possess diagnostic and prognostic relevance in allotransplantation of upper limbs. This comprehensive review explores nail and nail bed related changes encountered in UET recipients in the literature. The differential diagnosis of nail abnormalities in UET includes a wide range of systemic, local and iatrogenic conditions other than immune responses to the allograft. It requires interdisciplinary evaluation by primary transplant surgeons, pathologists, dermatologists and immunologists. The possible underl

Vitamins for the Management of Nail Disease: A Literature Review.

BACKGROUND: Vitamins have gained popularity among physicians and patients for purported benefits to hair, skin, and nail health. Safe and efficacious therapies for nail disorders, many of which are chronic conditions, are needed. SUMMARY: We conducted a literature review of studies assessing the efficacy of oral, topical, and intralesional vitamin/vitamin derivatives for the treatment of nail disorders, including yellow nail syndrome, brittle nail syndrome, onychomycosis, habit-tic nail deformity, periungual/subungual verruca, and nail psoriasis. Forty-nine articles were reviewed. There is go

Biomarkers of disease in human nails: a comprehensive review.

Diagnostic, monitoring, response, predictive, risk, and prognostic biomarkers of disease are all widely studied, for the most part in biological fluids or tissues, but there is steadily growing interest in alternative matrices such as nails. Here we comprehensively review studies dealing with molecular or elemental biomarkers of disease, as opposed to semiological, pharmacological, toxicological, or biomonitoring studies. Nails have a long history of use in medicine as indicators of pathological processes and have also been used extensively as a matrix for monitoring exposure to environmental

Biotin for the treatment of nail disease: what is the evidence?

AIM: To describe the pharmacology, mechanism of action, and clinical reports using biotin to treat nail conditions. METHODS: A review of articles indexed for MEDLINE on PubMed using keywords 'biotin' and 'nail' was performed and applicable articles were selected for review. RESULTS: Clinical trials have shown an improvement in firmness, hardness, and thickness of brittle nails with oral biotin. There are some case reports and series demonstrating that oral biotin may improve triangular worn down nails, trachyonychia, and habit tic nail deformity. CONCLUSIONS: Oral biotin has been used to tr

Nail findings in hand-foot-and-mouth disease.

Hand-foot-and-mouth disease (HFMD) is a common self-limited viral illness seen in the United States and around the world. Its classic features are easily recognizable; however, nail changes are not well known or characterized. This case demonstrates onychomadesis and Beau lines in a child following clinical diagnosis of hand-foot-and-mouth disease. In this setting, nail dystrophies should be reassuring to pediatricians and families.

Psoriatic nail disease: quality of life and treatment.

Nail psoriasis is common among patients with plaque psoriasis or psoriatic arthritis and has a detrimental effect on quality of life. However, there are currently no standardized therapeutic regimens for nail psoriasis. Traditional treatments for nail psoriasis, which include topical, intralesional, and oral therapies, may be time-consuming, painful, or unsafe when administered long term. Biologic therapies have demonstrated efficacy for plaque psoriasis and psoriatic arthritis; these therapies may be particularly promising for the treatment of nail psoriasis as both groups of patients have an

Vitamins and minerals: their role in nail health and disease.

Nail health and appearance are global concerns. We investigated the use of biotin vitamin E, alpha-tocopherol, vitamin C (ascorbic acid), vitamin A, retinoids, retinol, retinal, silicon, zinc, iron, copper, selenium, and vitamin B12 (Cyanocobalamin) in nail health and disease. The evidence that we adduce in this paper suggests that: 1) proper nail care seems to help maintain nail health; 2) no evidence supports the use of vitamin supplementation with vitamin E, vitamin C (ascorbic acid), vitamin A, retinoids, retinol, retinal, silicon, zinc, iron, copper, selenium, or vitamin B12 (Cyanocobalam

[Suspicion of systemic diseases through nails abnormalities].

Nails are protective, thin, horny growth at the end of fingers and toes. Abnormalities of the fingernails and toenails can provide both subtle and obvious clues to common medical problems or severe systemic diseases. Inheritance, minor injuries, common habits, the use of cosmetics and a variety of infections account for many changes in the appearance of nails. Awareness of normal nail variants, abnormalities and their disease associations will be beneficial to detect systemic diseases.

Molecular genetics of hereditary hair and nail disease.

Hair and nail development have many similarities and are likely to share many developmental pathways. This is evident from the ectodermal dysplasias that affect both appendages. Of special interest are syndromic disorders as they show the effects of disturbances in molecular pathways that are essential for normal embryonic development. In this review, recent results from studies of syndrome affecting hair and nail development will be discussed.

Beyond the Skin: Nail Clues to a Bone Disease.

Chronic recurrent multifocal osteomyelitis (CRMO) is a rare autoinflammatory bone disorder that predominantly affects children, and approximately 25% patients also present with an associated inflammatory condition. Most patients respond to nonsteroidal anti-inflammatory drugs, but refractory cases may need corticosteroids, bisphosphonate therapy, or disease-modifying antirheumatic drugs (DMARDs) like methotrexate, sulfasalazine, tumor necrosis factor-alpha (TNF-α) inhibitors, or even anti-interleukin-1 (anti-IL-1) blockers. We report the case of a previously healthy nine-year-old girl presenti

[Translated article] Congenital and Hereditary Nail Disease.

Nail disorders in newborns can show independently or as components of systemic illnesses or genodermatoses. The examination of these abnormalities is complex and sometimes challenging. However, familiarity with these disorders can significantly contribute to uncovering potential underlying conditions. This review includes the physiological nail changes seen within the first few months of life, such as Beau's lines, onychoschizia, koilonychia, congenital nail fold hypertrophy of the first digit, and onychocryptosis. This review also focuses on the most relevant congenital disorders reported and

Optimal management of nail disease in patients with psoriasis.

Psoriasis is a common skin disease, with nail involvement in approximately 80% of patients. Nail psoriasis is often associated with psoriatic arthropathy. Involvement of the nails does not always have relationship with the type, gravity, extension, or duration of skin psoriasis. Nail psoriasis can occur at any age and all parts of the nails and the surrounding structures can be affected. Two clinical patterns of nail manifestations have been seen due to psoriasis: nail matrix involvement or nail bed involvement. In the first case, irregular and deep pitting, red spots of the lunula, crumbling,
