ECTODERMAL DYSPLASIA (ED) IS A RARE DISORDER WITH DEFECTS IN TWO OR MORE OF THE FOLLOWING STRUCTURES: the teeth and the skin and its appendages including hair, nails, eccrine, and sebaceous glands. Dental manifestations include hypodontia, complete anodontia or malformed teeth. The most common form of the ED syndrome is hypohidrotic ED and is usually inherited as an X-linked recessive trait. Female carriers may have a variable degree of clinical manifestations. The condition is thought to occur in approximately 1 in every 100,000 live births. Dental treatment for these patients varies on an in

Dermatologic literature describes nail abnormalities involving nail bed as linear erythronychia or onychomatricoma. The abnormality reflects cognitive content of the nail bed. A resourceful epidermis capable of manifesting in a variety of clinical appearances depends on initiating stimulus affecting remarkably its nail bed matrix cells. These cells are stem cells (NBMSC) migrating distally to cover remarkably the underlying nail bed dermal ridges that are homologous to finger print dermal ridges. Normally, adult nail bed epidermal cells are uniform and keratinize with the stratum corneum witho

Shifts in skin microbiome are considered to be involved in the pathogenesis of psoriasis. However, data on the microbial dysbiosis of nail psoriasis are scarce. In this study, we aim to investigate and characterize the nail bacterial and fungal microbiome in patients with psoriasis. Nail samples were collected prospectively from 36 subjects with nail psoriasis, 24 psoriatic subjects without nail involvement and 32 healthy controls. Amplicon sequencing was performed to evaluate the bacterial and fungal community compositions. Significant alterations in the bacterial microbiome were found in the

BACKGROUND: Onychoheterotopia, or ectopic nail, is a rare nail condition in which nail growth occurs in areas other than the distal phalanges. The majority of cases in the literature are noted to be congenital; however, acquired cases have less commonly been described following either acute or chronic trauma.
METHODS: We present a case of acquired onychoheterotopia in a child, with a corresponding literature review of acquired pediatric onychoheterotopia cases for comparison.
RESULTS: There were a total of 10 cases of acquired onychoheterotopia in pediatric patients in the literature. Includ

For those interested in nails, a fairly good knowledge of the anatomy of the nail unit is indispensable. But nothing will replace a careful, clinical examination of the nail apparatus. The shape of the nail, surface abnormalities and coloration, with or without a magnifying glass, may well provide a medical diagnosis not only in dermatology but also for medicine in general. Pits on the surface of the nail plate may be enough to diagnose psoriasis on a limited area of scalp dandruff. Even partial detachment of the nail apparatus might lead to the suspicion of psoriatic arthritis. A triangular l

The majority of nail disorders are usually acquired; few are of congenital origin. Trauma to the nail unit commonly leads to dystrophy or deformity of the nail, but a division of the nail matrix can result in acquired polyonychia (split nail). Herein we describe a case of a 50-year-old male who had trauma in childhood after which he developed two nails on the right thumb. Cutaneous examination and X-ray revealed a well-defined split nail plate on the thumb. Surgery was advised but patient declined any surgical intervention. We describe this case for its rare occurrence.

As the so-called "non-surgical" nail procedures grow more and more aggressive and invasive in nature, the cold steel matrixectomy becomes an increasingly viable alternative. Many chemical and thermal matrixectomies are performed under poor aseptic conditions, increasing the risk of wound infection. With chemical matrixectomy, regulation of the level of tissue destruction is uncontrolled and often results in bone injury. The combination of these two elements can result in delayed diagnosis and recognition of osteomyelitis. When a practitioner is faced with a challenging ingrown, dystrophic, or

Onychotillomania, defined as self-induced trauma to the nail unit, either by picking or pulling at the nails, affects 0.9% of the population. It may lead to severe irreversible nail dystrophy, melanonychia, or infections. Although no large clinical trials have assessed the efficacy of treatments, cognitive-behavioral therapy, physical barrier methods, and pharmacological treatments have shown some benefits in case reports. The objective of this article is to review the prevalence, diagnostic criteria, etiology, historical and physical examination findings, pathological features, and current tr

BACKGROUND: Nail thickening is a poor prognostic factor in onychomycosis. Mechanical reduction by micromotor nail grinding is an alternative treatment for onychomycosis. However, this treatment introduces a large amount of infected nail dust particles into the air and can adversely affect other patients and health-care providers. The innovative recirculating airflow safety cabinet (ASC) was developed to prevent the spread of these generated infected nail dust particles. The aim of this study was to determine the efficacy of the ASC in patients with onychomycosis or traumatic onychodystrophy.

Nail lichen planus is an inflammatory disorder of the nails with potential for significant cosmetic disfigurement and functional impairment. Nail manifestations may be isolated or appear concurrently with other forms of lichen planus. Longitudinal ridging is the most common clinical finding, but progressive disease may result in irreversible scarring (dorsal pterygium) or permanent nail loss (anonychia). Data on treatment are limited to retrospective studies and case reports. The mainstays of treatment are intralesional and intramuscular corticosteroid injections and oral retinoids. There is a

The structure of the nail unit is complex and many dermatologists and dermatopathologists have an incomplete understanding of it. Familiarity with the anatomy and histology of this unit, however, is a key factor in improving the diagnostic yield of nail biopsy. Inflammatory or infectious conditions that affect the nail can have a marked impact on a patient's quality of life. A wide-ranging variety of tumors can also develop in this region and they may be life-threatening or require surgery that will result in functional defects. The author reviews the anatomy and histology of the nail unit as

Clinical symptoms attributed to the nail apparatus and observed in cosmetology include atrophic or hypertrophic lesions, pathologic nail coloration, abnormalities of the nail surface, and disorders of the nail plate and bed junction. These symptoms may reflect pathologic processes limited to the nail apparatus or may be the consequence of a dermal or systemic disease. Even though the etiology of nail lesions is variegated, diseases of the nails are simply classified as infectious or non-infectious. The aim of this work was to present the most common diseases of the nail apparatus encountered i

There are many causes for nail deformities, the most common being the result of trauma or pressure caused by infections, tumors, congenital anomalies, and irregularity of the underlying bone or unknown causes such as the pincer nail. The corrective treatment of the more common of these will be discussed in this and other articles. Examples of nail reconstructions have been described in the 1990 Hand Clinics and as a rule will not be repeated here.

A variety of individual nail dystrophies that are not categorized easily in other articles are reviewed. Onychoatrophia, anonychia, onychorrhexis, leukonychia, Beau's lines, onycholysis, onychomadesis, onychoschizia, haplonychia, longitudinal melanonychia, and ventral pterygium are included and pictured clinically. Their clinical description, etiology, associated conditions, differential diagnoses, and treatments are discussed and tabulated.

Median canaliform dystrophy (MCD) of Heller is a rare nail disorder characterized by a median split and obliquely running furrows of the nail plate, often presenting in a "fir-tree" pattern. While its pathogenesis remains poorly understood, proposed etiologies include habitual trauma, iatrogenic causes, and familial predisposition. Diagnosis is generally made by clinical evaluation. There is a paucity of data regarding the histopathology of MCD. Herein, we present two cases of MCD with novel histopathologic features, broadening the understanding of the pathophysiology and potential therapeutic
