Pharmacokinetics and pharmacodynamics of subcutaneous naltrexone pellets in the rat.

Subcutaneous implantation of naltrexone pellets is a standard method of producing chronic blockade of opioid receptors. In the present experiments, rats were treated with two, 30-mg naltrexone pellets and the pharmacokinetics and pharmacodynamics examined. This dosing method produced high plasma naltrexone levels (350 ng/ml) by 1 hr which declined over an implant period of 192 hr (24 ng/ml). Approximately 40% of the systemically available naltrexone (15.6 mg) was released in the first 24 hr. A total of 39.8 mg was released over the 192-hr implantation period. At 192 hr after implantation, nalt

Effects of chronic naltrexone on amphetamine locomotor activity.

Animals from two different populations of Wistar rats were chronically pretreated with naltrexone for 8 days. After a 2-day rest period, animals were tested with amphetamine for locomotor activity in the open-field with or without white noise (Day 2). The rats were similarly retested on Day 7 and Day 14. New colony animals showed a significant attenuation in amphetamine locomotor activity in the absence of noise only. In contrast, chronic naltrexone significantly decreased amphetamine activity in old colony animals only under noise conditions. The possibility of an inhibitory role opiate recep

Naltrexone: physiological and psychological effects of single doses.

An ascending series of single doses of the narcotic antagonist naltrexone, ranging from 20 to 160 mg, was administered to 8 abstinent former addicts in order to assess agonistic activity and any toxic side effects. There was little alteration of normal body function. Significant, but small, changes in sublingual temperature (0.4 degrees F decrease), and diastolic blood pressure (1.7 mm Hg increase) were induced. Among the battery of tests assessing behavioral or mood-feeling variables, only 2 showed significant between-condition effects: facilitated performance on the Cross-out Test (attention

Naltrexone-induced Nicolau syndrome masquerading as cutaneous abscess.

Nicolau syndrome-also known as Embolia Cutis Medicamentosa-is a rare complication of intramuscular and subcutaneous injections manifesting as necrosis of skin and the underlying tissues. The exact pathogenesis is uncertain. There are several hypotheses including direct damage to the end artery, cytotoxic effects of the implicated drug or additives in the injectable preparations. Naltrexone is a long-acting opioid antagonist used primarily in the management of alcohol and opioid dependence. The patient received intramuscular naltrexone for treatment of alcoholism. A week later, she presented wi

Naltrexone prevents ethanol-induced changes in rat thymus.

Ethanol is known to suppress the immune response, but the underlying mechanism accounting for the immunosuppression is not clearly elucidated yet. The purpose of this study was to investigate the effect of a single dose of ethanol on relative proportion of the four major rat thymocyte subsets and possible mechanism of its action. To this end, adult female AO rats were treated with: a) ethanol (2 or 4 g/kg, i.p.), b) naltrexone (5 mg/kg, i.p.) followed 45 min later by ethanol (2 or 4 g/kg, i.p.), c) naltrexone (5 mg/kg, i.p.), or d) only saline. Twenty hours later the rats were sacrificed and t

The Utilization of Low Dose Naltrexone for Chronic Pain.

Naltrexone is a mu-opioid receptor antagonist with a long half-life compared with naloxone. Both of these drugs, along with others, were developed with the intention of reversing the effects of opioid abuse or toxicity. Evidence has also shown that naltrexone has a benefit in preventing relapse by reducing opioid cravings and reducing symptoms of opioid withdrawal. The benefits of this drug were not only shown with opioid abuse. In 1984 this drug was also approved for alcohol abuse. Naltrexone has been proven to decrease alcohol relapse by decreasing the craving. Apart from these approved indi

Developmental Considerations for the Use of Naltrexone in Children and Adolescents.

Naltrexone (NTX) is a well-tolerated drug with a wide safety margin and mechanism of action that affords use across a wide variety of indications in adults and children. By antagonizing the opioid reward system, NTX can modulate behaviors that involve compulsivity or impulsivity, such as substance use, obesity, and eating disorders. Evidence regarding the disposition and efficacy of NTX is mainly derived from adult studies of substance use disorders and considerable variability exists. Developmental changes, plausible disease-specific alterations and genetic polymorphisms in NTX disposition, a

Naltrexone a potential therapeutic candidate for COVID-19.

Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) is the cause of Coronavirus Disease (COVID-19) that has resulted in a global pandemic. At the time of writing, approximately 16.06 million cases have been reported worldwide. Like other coronaviruses, SARS-CoV-2 relies on the surface Spike glycoprotein to access the host cells, mainly through the interaction of its Receptor Binding Domain (RBD) with the host receptor Angiotensin-Converting Enzyme2 (ACE2). SARS-CoV-2 infection induces a profound downstream pro-inflammatory cytokine storm. This release of the pro-inflammatory cytokines

Retention in naltrexone implant treatment for opioid dependence.

BACKGROUND: Naltrexone's usefulness in the treatment of opioid dependence stems from its ability to block the action of heroin and other opioids. However, many patients are ambivalent towards naltrexone and often drop out of treatment with orally administered naltrexone. Sustained release naltrexone seems promising in reducing opioid use, but the extent to which patients remain in treatment beyond the first dosage of naltrexone is not clear. METHODS: Patients (n=61) receving treatment with sustained release naltrexone implants were offered a second naltrexone implant after 6 months. Patients

An open trial of naltrexone in the treatment of depersonalization disorder.

Depersonalization disorder (DPD) remains one of the few disorders in modern psychiatry for which no treatments are established that are even partially effective, whether pharmacological or psychotherapeutic. Depersonalization disorder is a Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition dissociative disorder characterized by a pervasive subjective sense of unreality and detachment with intact reality testing. Two recent controlled medication trials, one with lamotrigine and one with fluoxetine, failed to show efficacy. There is some evidence for dysregulation of endogenou

Successful treatment of refractory aquagenic pruritus with naltrexone.

BACKGROUND: Aquagenic pruritus is an intense prickling sensation that develops in affected individuals immediately after contact with water at any temperature. It is most commonly associated with polycythemia rubra vera. Common but often ineffective treatments include anticholinergics and antihistamines. Other moderately successful treatments include capsaicin cream, UVB phototherapy, and sodium bicarbonate bath water. OBJECTIVE: In this case report we describe a 55-year-old female with severe itching following showers. Underlying causes were ruled out with a series of blood tests, a chest X-

Pharmacokinetics and dialysability of naltrexone in patients undergoing hemodialysis.

The disposition of naltrexone (NLT) (REVIA), an opioid antagonist intended for patients with impaired renal function and with severe intractable itching refractory to regular antipruritic therapy, was characterized. Hemodialysis effects on both efficacy and elimination of NLT also were assessed. We developed a simple, sensitive and selective reverse-phase high-performance liquid chromatographic (HPLC) method for measuring NLT plasma concentration in hemodialysis patients treated to relieve pruritus. NLT and the internal standard, naloxone (NLX) were extracted from plasma using a solid-phase ex

Naltrexone in the treatment of alcohol dependence: a Canadian trial.

OBJECTIVES: Alcohol dependence is a prevalent psychiatric disorder affecting approximately 12% of the adult population at some point in their lifetime. Psychosocial treatments are associated with only modest success rates. The first Canadian clinical trial with naltrexone, an opiate antagonist, was conducted to evaluate its safety and usefulness as an adjunctive treatment in the management of alcohol dependence. METHODS: One hundred twenty alcohol-dependent individuals were assessed to receive treatment with 50 mg of naltrexone orally for 12 weeks in an open-label trial. Patients were seen bi

Naltrexone improves outcome of a controlled drinking program.

Naltrexone is widely used in therapeutic programs with abstinence as a goal. However, it has been used in only a few studies aimed at reducing alcohol consumption. The purpose of this study was to evaluate the efficacy of naltrexone as an adjunct in controlled drinking programs. This was an open randomized study of 12 weeks duration that compared two therapeutic strategies: use of naltrexone in a controlled drinking program (NTX+CD) and the controlled drinking program alone (CD), without NTX. Each group comprised 30 male patients with mild alcohol dependence. During treatment, there were no di
