Herpes simplex virus vector-mediated gene delivery for the treatment of lower urinary tract pain.

Interstitial cystitis (IC)/painful bladder syndrome (PBS) is a painful debilitating chronic visceral pain disorder of unknown etiology that affects an estimated 1 million people in the United States alone. It is characterized by inflammation of the bladder that results in chronic pelvic pain associated with bladder symptoms of urinary frequency and urgency. Regardless of the etiology, IC/PBS involves either increased and/or abnormal activity in afferent nociceptive sensory neurons. Pain-related symptoms in patients with IC/PBS are often very difficult to treat. Both medical and surgical therap

Opioids and Adolescents: How to Prevent and Treat Addictions Early.

Addiction is a treatable, chronic condition of pediatric onset. Providers will learn how to use evidence-based methods for prevention , screening, and treatment of substance use among adolescents. Treating adolescents who use substances can be an opportunity to provide life-saving harm reduction resources including naloxone and start medications for opioid use disorder such as buprenoprhine. Identifying early, at-risk substance use is increasingly critical given the risk of lethal overdose due to fentanyl contamination.

Continuity and change in drug treatment: Regimes of control in Japan.

BACKGROUND: This article examines the development of illegal drug control in Japan. Focusing on the area of drug treatment, a theoretical explanation is offered for how and why a previously punitive configuration has shifted towards a more complex one including both inclusionary and exclusionary approaches. In doing so, it argues for theoretical engagement with the relations of power which structure political competition in the governance of illegal drug control. METHODS: Using conceptual tools adopted from urban regime analysis, the article discusses the dispositions, resources, and schemes

Methamphetamine use and illicit opioid use during buprenorphine treatment.

INTRODUCTION: Although methamphetamine use is rising in the United States, its impacts on patient outcomes among persons undergoing treatment for opioid use disorder (OUD) remain unclear. This study aims to assess the association between baseline methamphetamine/amphetamine (MA/A) use and subsequent illicit opioid use among patients with OUD initiating buprenorphine in an office-based setting. METHODS: We conducted a secondary analysis of a pilot randomized controlled trial of a behavioral mobile health intervention for buprenorphine adherence conducted over a 12-week study period at two clin

Pharmacokinetic drug interactions and adverse consequences between psychotropic medications and pharmacotherapy for the treatment of opioid dependence.

BACKGROUND: Psychiatric comorbidities among opioid-dependent patients are common. Many medications used to treat both conditions are metabolized through complimentary cytochrome P450 isoenzymes. When medication-assisted treatment for opioid dependence is concurrently used with psychotropic medications, problematic pharmacokinetic drug interactions may occur. METHODS: We reviewed relevant English language articles identified through the MedLine, Scopus, and Embase databases from 1950 to December 2009 using the specific generic names of medications and keywords such as pharmacokinetics and drug

[Role of dopamine in drug dependence processes].

Recent neurobiological data have indicated that almost all drugs of abuse (amphetamine, cocaine, morphine, heroin, cannabis...) increase dopamine release in a sub-cortical structure, the nucleus accumbens. This nucleus is part of different cerebral structures, named "reward system" which constantly defines physical and mental states of an individual. Development of addiction would be due to the modification, by drugs of abuse, of the kinetic and the amplitude of dopamine release. Such a dysregulation would induce the addict to artificially memorize any event linked to the product consumption.

Psychostimulant abuse: the case for combined behavioral and pharmacological treatments.

Behavioral and pharmacological therapies have been used alone and in combination for the treatment of substance abuse; however, to date, no single treatment approach for psychostimulant abuse has demonstrated widespread efficacy. This paper describes the various functions that are served by both behavioral therapies and pharmacotherapies and their respective mechanisms of action. It is argued that combined treatments can be expected to produce additive effects because the two approaches operate through different and potentially complementary mechanisms. Illustrations of these underlying princi

Measurement of strong and weak drug habits: associations to stages of addiction, steps in drug-taking, and drug effects in response to opiate names and nonopiate names.

Confirmed opiate addicts (N = 78) were asked to associate three opiate names (heroin, morphine, and methadone) and six nonopiate names [pentobarbital, alcohol, Benzedrine, "goofballs" (barbiturates), cocaine, and "reefers" (marijuana)] to stages of addiction, steps in drug taking, and drug effects. It was believed that associations to opiates would represent a strong habit since most of the subjects had been addicted ("hooked") on one or more opiates and that associations to nonopiates would indicate a weak habit as most of the subjects had never been "hooked" on nonopiates. Weak and strong ha

How to spot illicit drug abuse in your patients.

Illicit drug abuse continues to become more widespread, especially in teenagers. Therefore, it is important for physicians to recognize the signs and symptoms of abuse in their patients. Drug abuse should be considered in differential diagnosis of many physical and nearly all psychiatric complaints. An understanding of the pharmacologic mechanisms and adverse effects of illicit drugs can enhance overall care of patients who abuse drugs. The primary classes of drugs of abuse--cannabinoids, opiates, stimulants, hallucinogens, and inhalants--produce clinically diverse presentations. By recognizin

Increasing Short- and Long-Term Buprenorphine Treatment Capacity: Providing Waiver Training for Medical Students.

In the face of an ongoing opioid crisis in the United States, persistent treatment gaps exist for vulnerable populations. Among the 3 Food and Drug Administration-approved medications used to treat opioid use disorder, many patients prefer buprenorphine. But physicians are currently required to register with the Drug Enforcement Administration and complete 8 hours of qualifying training before they can receive a waiver to prescribe buprenorphine to their patients. In this article, the authors summarize the evolution of buprenorphine waiver training in undergraduate medical education and outlin

Caring for patients with opioid dependence.

Australian general practitioners have a pivotal role in helping their patients with problems of alcohol and tobacco dependence, but have been less likely than their peers in the United Kingdom to be involved in the management of opioid dependence. This is despite the fact that most GPs are seeing opioid dependent patients whether they know it or not, and that GPs are ideally placed to manage this problem given the other advantages of the general practice context--continuity, accessibility, and whole patient care, including the ability to address other health issues.

Buprenorphine: a primer for emergency physicians.

The recent approval of office-based treatment for opioid addiction and US Food and Drug Administration approval of buprenorphine will expand treatment options for opioid addiction. Buprenorphine is classified as a partial micro opioid agonist and a weak kappa antagonist. It has a high affinity for the micro receptor, with slow dissociation resulting in a long duration of action and an analgesic potency 25 to 40 times more potent than morphine. At higher doses, its agonist effects plateau and it begins to behave more like an antagonist, limiting the maximal analgesic effect and respiratory depr

Rational treatment of addiction.

Treatment of alcohol and drug addictions, which has been neglected medically for a long time, is currently sparked with optimism. Craving for alcohol can be treated with two newly registered drugs: naltrexone and acamprosate. New approaches to symptom relief during detoxification or during maintenance therapies are rationally based on experimental and clinical work. It is now clear that addictive drugs are surrogates of natural substances involved in the 'reward system'.

The solution to narcotic addiction in the People's Republic of China.

The People's Republic of China dealt with addiction as a political problem, offering the new society hope, food, shelter, work, and land instead of opium. Addiction no longer had its appeal. Opium producing poppies were replaced by food corps. Large opium distributors were imprisoned. Addicts were "clean". A mass campaign against addiction mobilized the entire nation. Before Liberation in 1949, the Chinese Communist Party had kept opium out of their areas. However, it took until 1953 to rid China completely of opium. Twenty million Chinese outside the People's Republic of China continue to hav
