Inhaled and nasal corticosteroids: factors affecting the risks of systemic adverse effects.

There has been increasing concern in the medical literature about the safety of inhaled and nasal corticosteroids, since many patients, both adults and children, are increasingly prescribed these drugs for the long-term prophylactic treatment of asthma and rhinitis. It is well recognised that systemic absorption occurs following inhaled and nasal administration of corticosteroids, but the dose at which clinically relevant side effects occurs is controversial. The controversy stems from the fact that the degree of systemic absorption depends not only upon the prescribed dose, but also upon the

Antibacterial polyurethane foams with quaternized-chitosan as a chain extender for nasal packing and hemostasis.

Endoscopic surgery is an effective and common clinical practice for chronic sinusitis. Nasal packing materials are applied in nasal surgery to prevent hemorrhage and promote wound healing. In this study, a degradable polyurethane foam dressing is successfully developed as a promising nasal packing material with good biocompatibility and antibacterial capability. Specifically, quaternized chitosan (QCS) serves as the crosslinker instead of polyols to offer polyurethane foam (PUF-QCS) antibacterial capability. The PUF-QCS2.0 % (with 2.0 wt% QCS) exhibits satisfactory liquid absorption capacity (

Semi-PBPK Modeling and Simulation to Evaluate the Local and Systemic Pharmacokinetics of OC-01(Varenicline) Nasal Spray.

This study aimed to build a nasal semi-physiologically based pharmacokinetic (PBPK) model to predict the intranasal pharmacokinetic (PK) of the OC-01(varenicline) nasal spray and accelerate the development of this drug. Based on the physiology of the human upper respiratory system, the semi-PBPK model was established and validated using systemic plasma PK data of varenicline previously observed in Americans and Chinese. Drug concentrations, both in respiratory tissue and plasma circulation system, were well simulated, and it was indicated that local concentration at the target site (nasal cavi

Stimuli-responsive In situ gelling system for nose-to-brain drug delivery.

The diagnosis and treatment of neurological ailments always remain an utmost challenge for research fraternity due to the presence of BBB. The intranasal route appeared as an attractive and alternative route for brain targeting of therapeutics without the intrusion of BBB and GI exposure. This route directly and effectively delivers the therapeutics to different regions of the brain via olfactory and trigeminal nerve pathways. However, shorter drug retention time and mucociliary clearance curtail the efficiency of the intranasal route. The in situ mucoadhesive gel overthrow the limitations of

A Nasal Aerodynamics Perspective of Retronasal Olfaction: Rodents vs. Humans.

INTRODUCTION: Odor perception can be achieved through ortho or retronasal routes, with the latter being an important component of flavor perception. There are significant olfactory differences that exist between rats and humans and by understanding the role of structural differences, further insight can be gained into the mechanism of odorant perception via ortho or retronasal routes. METHODS: 3D human and rat (Sprague Dawley) computational models were used to investigate nasal anatomy impact on ortho vs. retronasal odorant transport to the olfactory epithelium. The nasal pharynx region was m

Cerebrospinal fluid absorption in the rabbit: olfactory pathways.

Twenty rabbits were intrathecally perfused with horseradish peroxidase (HRP) without increasing the cerebrospinal fluid (CSF) pressure. Histologic and ultrastructural studies of the olfactory nerve and nasal mucosa showed HRP along the space between peri- and epineurium down to the smaller nerve branches in the mucosa. Within the mucosa, HRP leaked out from the epineurium and flowed into: (a) lymphatic vessels; (b) mucous glands and ducts; (c) intercellular spaces of the nasal pseudostratified nasal epithelium; and (d) venules. These findings could account for "bulk" flow CSF absorption into t

Versatile Nasal Application of Cyclodextrins: Excipients and/or Actives?

Cyclodextrins (CDs) are oligosaccharides widely used in the pharmaceutical field. In this review, a detailed examination of the literature of the last two decades has been made to understand the role of CDs in nasal drug delivery systems. In nasal formulations, CDs are used as pharmaceutical excipients, as solubilizers and absorption promoters, and as active ingredients due to their several biological activities (antiviral, antiparasitic, anti-atherosclerotic, and neuroprotective). The use of CDs in nasal formulations allowed obtaining versatile drug delivery systems intended for local and sys

Nasal formulations for drug administration and characterization of nasal preparations in drug delivery.

This special report gives an insight in the rationale of utilizing the nasal cavity for drug administration and the formulation as well as characterization of nasal preparations. As the nose is an easy-to-access, noninvasive and versatile location for absorption, this route of delivery will play an increasingly important role in future drug product development both for new and repurposed drugs. The nose can be utilized for local and systemic delivery including drug delivery to the central nervous system and the immune system. Typical formulation strategies and future developments are reviewed,

Pharmacokinetics of concentrated naloxone nasal spray over first 30 minutes post-dosing: analysis of suitability for opioid overdose reversal.

BACKGROUND AND AIMS: Lack of non-injectable naloxone formulations has impeded widespread take-home provision for the prevention of heroin/opioid overdose deaths. For non-injectable formulations that are finally being investigated, rapid onset of action and sufficient bioavailability will be vital. We present analysis of data from a study of concentrated naloxone nasal spray formulations. Our aims are: to assess (1) pharmacokinetic properties and (2) suitability for overdose reversal in terms of naloxone absorption within 30 minutes post-dosing. DESIGN AND INTERVENTIONS/COMPARATOR: Open-label,

In Vitro Assessment of Spray Deposition Patterns in a Pediatric (12 Year-Old) Nasal Cavity Model.

PURPOSE: Nasal sprays available for the treatment of cold and allergy symptoms currently use identical formulations and devices for adults as well as for children. Due to the obvious differences between the nasal airway dimensions of a child and those of an adult, the performance of nasal sprays in children was evaluated. METHODS: Deposition patterns of nasal sprays administered to children were tested using a nasal cast based on MRI images obtained from a 12 year old child's nasal cavity. Test formulations emitting a range of spray patterns were investigated by actuating the device into the

Tailored Borneol-Modified Lipid Nanoparticles Nasal Spray for Enhanced Nose-to-Brain Delivery to Central Nervous System Diseases.

The nanodrug delivery system-based nasal spray (NDDS-NS) can bypass the blood-brain barrier and deliver drugs directly to the brain, offering unparalleled advantages in the treatment of central nervous system (CNS) diseases. However, the current design of NNDS-NS is excessively focused on mucosal absorption while neglecting the impact of nasal deposition on nose-to-brain drug delivery, resulting in an unsatisfactory nose-to-brain delivery efficiency. In this study, the effect of the dispersion medium viscosity on nasal drug deposition and nose-to-brain delivery in NDDS-NS was elucidated. The o

A randomized, open-label, two-treatment crossover study to evaluate the effect of food on the pharmacokinetics of diazepam nasal spray in healthy adults.

OBJECTIVE: The pharmacokinetics of oral diazepam are affected by food, but food-effect studies have not been conducted for diazepam nasal spray because it is believed that most absorption occurs via the nasal mucosa. However, gastrointestinal side effects reported with nasal diazepam suggest that at least a portion of the drug may be absorbed enterally and thus subject to food effects. The objective of this study was to evaluate the possible effects of food on the pharmacokinetics of diazepam nasal spray in healthy adults. METHODS: This randomized, open-label crossover study compared equal do

Nasal Delivery to the Brain: Harnessing Nanoparticles for Effective Drug Transport.

The nose-to-brain drug-delivery system has emerged as a promising strategy to overcome the challenges associated with conventional drug administration for central nervous system disorders. This emerging field is driven by the anatomical advantages of the nasal route, enabling the direct transport of drugs from the nasal cavity to the brain, thereby circumventing the blood-brain barrier. This review highlights the significance of the anatomical features of the nasal cavity, emphasizing its high permeability and rich blood supply that facilitate rapid drug absorption and onset of action, renderi

Current Therapeutic Strategy in the Nasal Delivery of Insulin: Recent Advances and Future Directions.

BACKGROUND: Insulin (INS) has been used in the treatment of diabetes mellitus. Due to its large molecular weight and short half-life, it has been usually administered subcutaneously accompanied with side effects such as the possibility of hypoglycemia episodes, weight gain and inadequate post-meal glucose control. OBJECTIVE: In order to overcome these limitations, alternative delivery routes of insulin are expected to provide better safety and compliance for the patient. Non-invasive insulin delivery system represents one of the most challenging goals for pharmaceutical industry. Nasal insuli

In Vitro Comparative Study on Oppositely Charged Donepezil-Loaded Intranasal Liposomes.

Background/Objectives: Intranasal delivery is a promising approach for targeting the central nervous system (CNS); however, most of the drugs show poor permeability through the nasal mucosa. Nanocarriers such as liposomes can improve nasal drug absorption; however, the surface charge of liposomes has a key role in the nasal mucosal uptake process. Therefore, the present study aimed to formulate and compare the intranasal applicability of oppositely charged liposomes loaded with donepezil hydrochloride (DPZ) as CNS-active model compound using two different charge inducers, the negatively charge

Intranasal Coadministration of a Diazepam Prodrug with a Converting Enzyme Results in Rapid Absorption of Diazepam in Rats.

Intranasal administration is an attractive route for systemic delivery of small, lipophilic drugs because they are rapidly absorbed through the nasal mucosa into systemic circulation. However, the low solubility of lipophilic drugs often precludes aqueous nasal spray formulations. A unique approach to circumvent solubility issues involves coadministration of a hydrophilic prodrug with an exogenous converting enzyme. This strategy not only addresses poor solubility but also leads to an increase in the chemical activity gradient driving drug absorption. Herein, we report plasma and brain concent

Optimizing Absorption for Intranasal Delivery of Drugs Targeting the Central Nervous System Using Alkylsaccharide Permeation Enhancers.

Intranasal delivery of drugs offers several potential benefits related to ease of delivery, rapid onset, and patient experience, which may be of particular relevance to patients with central nervous system (CNS) conditions who experience acute events. Intranasal formulations must be adapted to address anatomical and physiological characteristics of the nasal cavity, including restricted dose volume, limited surface area, and barriers to mucosal absorption, in addition to constraints on the absorption window due to mucociliary clearance. Development of an effective formulation may utilize strat

Serum concentration patterns of an LHRH agonist, gonadotrophins and sex steroids after subcutaneous, vaginal, rectal and nasal administration of the agonist to pregnant rats.

A highly potent LHRH agonist, des-Gly10-[D-Leu6]-LHRH-ethylamide (TAP-144), was administered by subcutaneous, vaginal, rectal and nasal routes to pregnant rats on Day 10. After subcutaneous administration, almost all the TAP-144 was rapidly absorbed and eliminated. After vaginal administration, the fraction of the dose that reached the systemic circulation, the F value, was very high but the rate of absorption was very low; consequently there was a low peak level and persistence of serum level. After rectal administration, the F value was very low and the rate of absorption was lower than that

Gore-Tex for augmentation of the nasal dorsum: a preliminary report.

Many different materials have been used to provide augmentation of the nasal dorsum. This includes both autogenous and homogenous materials, as well as allografts. Many times autogenous material is not readily available and homogenous materials have recently demonstrated significant absorption. Several artificial materials have been used in the past, including Supramid, silicone, and Proplast, each with its own limitations or problems. Two years ago we began using Gore-Tex Soft Tissue Patch as a material to augment the nasal dorsum. Although our experience is preliminary, certainly, experience
