[Pleiomorphism of the cytokine profile in nasal polyp tissue depending on the phenotype of chronic rhinosinusitis with nasal polyps].

UNLABELLED: Clinical phenotypes of chronic rhinosinusitis with nasal polyps (CRSwNP) are characterized with different inflammation patterns of mRNA expression of cytokines and depend on presence of allergic rhinitis (AR), atopic bronchial asthma (aBA) or nonatopic bronchial asthma (nBA). OBJECTIVE: To compare inflammation response in patients with different phenotypes of CRSwNP according to level secretion of the key cytokines in nasal polyp tissue. MATERIAL AND METHODS: 292 patients with CRSwNP were divided into four phenotypes: group 1 - CRSwNP without respiratory allergy (RA) and without

Prognostic factors for polyp recurrence in chronic rhinosinusitis with nasal polyps.

BACKGROUND: Chronic rhinosinusitis with nasal polyps is frequently managed with endoscopic sinus surgery (ESS). Prior studies describe individual clinical variables and eosinophil density measures as prognostic for polyp recurrence (PR). However, the relative prognostic significance of these have not been extensively investigated. OBJECTIVES: We sought to evaluate the impact of PR on measures of disease severity post-ESS and quantify the prognostic value of various clinical variables and biomarkers. METHODS: Ninety-four patients with chronic rhinosinusitis with nasal polyps and prospectively

Choanal polyp originating from the nasal septum: septochoanal polyp.

Most of the nasal polyps arise from the lateral walls of the nasal cavity. Nasal polyps originating from the nasal septum with choanal extension are extremely rare. We report a case of large choanal polyp that arised from the posterosuperior aspect of the nasal septum, and extended down to the oropharynx. A 52-year-old woman presented with a two-year history of progressive nasal obstruction and snoring. Findings of anterior rhinoscopy were in normal limits. We think that the term "septochoanal polyp" which, as far as we know, has not been mentioned in the literature before, can be used for thi

Bilateral nasal polyps originating from the nasal septum: a case report.

INTRODUCTION: All nasal polyps originate from the mucosa of the nasal cavity or the paranasal sinuses. Most polyps originate from contact areas of the uncinate process, the middle turbinate or the ethmoid infundibulum. Polyps originating from the mucosa of the nasal septum are rare. CASE REPORT: We present a 77-year-old man with nasal polyps originating from the superior aspect of the nasal septum. These lesions had the typical histological appearance of nasal polyps. DISCUSSION: Nasal polyps are inflammatory growths of the nasal or paranasal mucosa. Nasal polyps commonly present clinically

Nasal disorders in children. Allergic rhinitis, vasomotor rhinitis, and nasal polyps.

Although the symptoms of allergic rhinitis, vasomotor rhinitis, and nasal polyps are similar, these conditions usually require different treatment. Allergic rhinitis responds best to environmental control, avoidance of allergens and irritants, and intermittent use of medication. Hyposensitization therapy is useful in some patients. Vasomotor rhinitis is usually helped by avoidance of environmental factors, but medical or surgical management is sometimes appropriate. Nasal polyps can be removed surgically.

Eosinophilic allergic polyp: a clinically oriented concept of nasal polyp.

OBJECTIVE: Tissue eosinophilia is a hallmark of nasal polyposis. However, there is no universally accepted definition for eosinophilic nasal polyp. The aims of this study were to histologically analyze the inflammatory cell population in the nasal polyp and propose the concept of eosinophilic allergic polyp (EAP), a simple and objective concept that would be clinically and practically valuable. STUDY DESIGN: A cohort study with histological analysis of tissue sections and chart review. SETTING: A tertiary university hospital. SUBJECTS AND METHODS: Nasal polyp tissues were harvested during r

Nasal conditioning after endonasal surgery in chronic rhinosinusitis with nasal polyps.

BACKGROUND: Endoscopic sinus surgery (ESS) is a frequently performed operation for chronic rhinosinusitis (CRS). The aim of this study was to investigate nasal conditioning after ESS with/without septoplasty and turbinoplasty in patients with CRS with nasal polyps and to determine its relation to nasal patency and geometry. METHODS: Twenty-one patients were enrolled into this study. Nasal conditioning, AAR, and acoustic rhinometry were measured before and 4-8 months after endonasal surgery. RESULTS: The temperature and humidity gradient across the nose increased significantly after operation

Biologics for chronic rhinosinusitis with nasal polyps.

With the increasing recognition of the role of type 2 immune responses in chronic rhinosinusitis, its severity, recurrence, and comorbidities, several biologics targeting IL-4, IL-5, and IL-13 as well as IgE have been administered in small proof-of-concept studies. Recently, the first phase 3 trials have been reported with dupilumab, an IL-4 receptor antagonist, demonstrating a significant and clinically relevant reduction of the disease burden from polyp size and sinus involvement to symptoms and smell; these changes consecutively led to an important increase in quality of life. Finally, the

Biomarkers in Chronic Rhinosinusitis with Nasal Polyps.

Chronic rhinosinusitis is a complex disease that exists along the inflammatory spectrum between types 1 and 2 inflammation. The classic phenotypic differentiation of chronic rhinosinusitis based on the presence or absence of inflammatory polyps remains one of the best differentiators of response to therapy. Development of biologics for the treatment of atopic disease and asthma and topical therapies for sinusitis have placed renewed emphasis on understanding the pathophysiology of polyp disease. Identification of key markers of polyposis will allow for better stratification of inflammatory pol

Detection of Helicobacter pylori in Nasal Polyps.

To detect the presence of Helicobacter pylori in nasal polyps. A case-control study was conducted enrolling 35 patients with nasal polyps (cases) and patients undergoing septoplasty (controls). Fresh tissue samples were used for urea broth test and imprint cytology, while formalin fixed tissue sections were used for morphology, special stains and immunohistochemistry for H. pylori. Fresh stool samples from both groups were tested to correlate the gastrointestinal status. H. pylori was detected in 40.0 % (14/35) of cases and 8.5 % of controls (3/35) (p = 0.004) by immunohistochemistry. Amongst

Epidemiology and differential diagnosis of nasal polyps.

BACKGROUND: Chronic rhinosinusitis (CRS) is one of the most common chronic medical conditions, with a significant impact on patient quality of life. CRS is broadly classified into two groups: CRS with nasal polyposis (CRSwNP) and CRS without NP (CRSsNP). Clinically, the major subtypes of CRSwNP may be divided into eosinophilic chronic rhinosinusitis (e.g., allergic fungal rhinosinusitis and aspirin-exacerbated respiratory disease [AERD]) and nasal polyps associated with neutrophilic inflammation (e.g., cystic fibrosis [CF]). CF is characterized by mutation of the gene encoding the CF transmemb

Doxycycline or oral corticosteroids for nasal polyps.

The study by Van Zele et al reports positive results for a randomized, double-blind, placebo-controlled trial that used either 20 days of doxycycline (200 mg the first day, followed by 100 mg daily) or 20 days of a tapering schedule of methyl prednisolone (32 mg on days 1-5, 16 mg on days 6-10, and 8 mg on days 11-20) for the treatment of nasal polyps. The primary end point was reduction in nasal polyp score which was graded 0 to 4 on each side, resulting in a score range of 0 to 8; the score was determined at baseline and weeks 1, 2, 4, 8, and 12. Entry criteria required that the patient eith

Nasal polyps: still more questions than answers.

Nasal polyps are common, affecting one to four per cent of the population. Their cause, however, remains unknown and it is possible that it is not the same in all patients. They have a clear association with asthma, aspirin sensitivity and cystic fibrosis. Histologically they demonstrate large quantities of extracellular fluid, mast cell degranulation and an infiltrate of inflammatory cells, usually eosinophils. While this appearance would suggest an allergic pathology there is little conclusive evidence to support this in most patients. There is, however, some preliminary evidence to suggest

The estimated incidence of symptomatic nasal polyps.

The objective of the present study was to establish the estimated incidence of symptomatic nasal polyps within a defined geographic area. Over a 6-year period all polyp patients diagnosed for the first time (n = 252; 174 males, 78 females) were prospectively registered at the study clinic. An estimated background population from which the patients were recruited was defined. Polyps were diagnosed by endoscopic examination. The mean estimated incidences for all age groups were 0.86 and 0.39 patients per thousand per year for males and females, respectively. The incidence increased with age, rea

Increased epithelial cell proliferation in nasal polyps.

BACKGROUND AND PURPOSE: The mucociliary function of the sinonasal mucosa is an innate defense mechanism of the human nasal airway. Epithelial cell proliferation is required for cell renewal and repair of the sinonasal mucosa. In this study, the MIB-1 monoclonal antibody, which can detect the S-phase nuclear protein Ki-67, was used to evaluate the proliferation of the sinonasal mucosa in patients with various inflammatory conditions of the sinonasal mucosa. METHODS: Specimens from the inferior turbinates of patients with nasal allergy (n = 12), vasomotor rhinitis (n = 20), nasal polyps (n = 22

Nasal polyps, bronchial asthma and aspirin sensitivity.

The ASA triad comprises bronchial asthma, acetylsalicylic acid (ASA) sensitivity and nasal polyps. It presents as chronic rhinitis followed by bronchial asthma and ASA sensitivity, and later nasal polyps. The pathogenesis of the ASA triad may involve interrelationships between disease in the upper and lower airway and hypersensitivity to cyclo-oxygenase inhibiting medications. Treatment of the nasal polyps has been shown to improve the patients' asthma.
