[Options for reconstruction after injuries in the head and neck region].

Reconstruction of lesions in the head and neck region must be both functionally and esthetically adequate, as the exposed anatomic position can easily lead to social stigmatization after injury. Distortion of symmetry, e.g., by a crooked nose, enophthalmos, or a (partial) amputation of the outer ear, is easily visible. On the other hand, limitations to nasal breathing and olfaction or diplopia may significantly reduce quality of life, and restoration of form and function continues to be challenging. This review discusses the treatment options for trauma of the external nose and the lateral mid

Iatrogenic Injuries in Percutaneous Pinning Techniques for Fifth Metacarpal Neck Fractures.

BACKGROUND: There is little information regarding anatomic structures at risk during closed percutaneous treatment of fifth metacarpal neck fractures. This study evaluates a variety of common percutaneous techniques with the hypothesis that each approach presents unique risks to tendons and neurovascular structures. METHODS: Seven cadaveric hands were used for this study. The senior author, a board-certified hand surgeon with more than 20 years of experience, used a mini-C arm to pass 1.6-mm Kirschner wires (K-wires). The 4 percutaneous techniques employed were anterograde, retrograde, cross-

Our treatment approaches in head-neck injuries caused by animal bites.

Several approaches exist for the treatment of animal attacks targeting the head and neck region. The treatment options and timing vary depending on the animal species, the nature of the defect, and the experience of the surgeon. In this study, early surgical treatment options used in head-neck injuries caused by domesticated or wild animal attacks are presented. We consider 12 patients who were admitted to our clinic between June 2006 and May 2010 with head-neck injuries caused by animal attacks. Tissue defect had developed in 10 patients due to half-wild dog bite and in 2 patients due to wolf

A Systematic Review of Head, Neck and-Facial Injuries in Cricket.

This systematic review was conducted to identify the incidence, nature and mechanisms of head, neck and facial (HNF) injuries in cricket and the reported use of helmets. Five databases were searched up to 30th November 2020. From peer-reviewed cricket injury studies published in English, studies reporting on HNF cricket injuries as per the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines were selected. Twenty-nine studies were included. HNF injuries had a cumulative total of 794/5,886 injuries equating to 13% of all injuries. Non- specified HNF injuries (n

Motorcycle-related head and neck injuries: increased risk among ethnic minorities.

BACKGROUND: Ethnic disparities have been associated with injury and mortality. The impact of ethnicity on head and neck injury (HNI), traumatic brain injury (TBI), in-hospital mortality and resource utilization following a motorcycle crash (MCC) is undetermined. This study explored the influence of ethnicity in these aspects and the effect of helmet use on HNI and TBI following a MCC. METHODS: The National Trauma Registry provided hospitalization data on motorcycle riders and passengers between 2008 and 2017. Ethnicity was classified as Jews or Arabs, the two major ethnic groups in Israel. Un

Airway management in laryngotracheal injuries from blunt neck trauma in children.

Pediatric laryngotracheal injuries from blunt neck trauma are extremely rare, but can be potentially catastrophic. Early diagnosis and skillful airway management is critical in avoiding significant morbidity and mortality associated with these cases. We present a case of a patient who suffered a complete tracheal transection and cervical spine fracture following a clothesline injury to the anterior neck. A review of the mechanisms of injury, clinical presentation, initial airway management, and anesthetic considerations in laryngotracheal injuries from blunt neck trauma in children are present

Vessel injuries of the head and neck presenting in mandibular fractures.

OBJECTIVE: Extracranial vascular injuries of the head and neck are relatively underdiagnosed in patients with acute trauma and can carry devastating sequelae. We wished to identify the correlation between mandibular fractures and injuries to major vessels of the head and neck. STUDY DESIGN: A retrospective review of our trauma registry was performed for the 1993-2007 period. The data on all mandibular fractures were collected. RESULTS: A total of 2288 patients presented to Parkland Memorial Hospital with mandibular fractures, of whom 47 (2%) had an injury to a major vessel of the neck. Nearl

Gunshot injuries in the neck area: ballistics elements and forensic issues.

The neck is an interesting structure as far as penetrating trauma is concerned because of the multiple vital structures that are concentrated in a small anatomic area. Gunshot wounding is an interaction between the penetrating projectile, the anatomy of the wounded subject, and the chance occurrences that determine the exact missile path. The mass and velocity of the projectile establish the upper limit of possible tissue damage. Management of gunshot neck injuries depends on a clear understanding of the anatomy of the neck. The radiologist can contribute substantially to the successful treatm

Management of penetrating neck injuries: a new paradigm for civilian trauma.

PURPOSE: Improvements in imaging technology, particularly computed tomographic angiography (CTA), have altered the management of patients with penetrating neck injuries. Although some centers still advocate routine exploration for all zone 2 neck injuries penetrating the platysma, many civilian centers in the United States have adopted a policy of selective exploration based on clinical and radiographic examination. The purpose of this retrospective study is to evaluate our 5-year experience with the management of penetrating neck injuries, to further elucidate the role of CTA in clinical deci

Two unusual stab injuries to the neck: homicide or self-infliction?

A 31-year-old woman was found dead by her daughter, lying in the living room which showed a large pool of blood, secondary droplets and stains from arterial blood spatter, dropping and contact. This bloody scene and two puncture wounds at the anterior aspect of the neck, one of them transecting the left common carotid artery leading to exsanguination, arose suspicion of homicide. However, the wound morphology including notches and a parallel skin incision as well as microradiography demonstrated that the two puncture wounds had been produced by glass. At the scene, a broken wine glass with two

Blunt vascular injuries of the head and neck: is heparinization necessary?

BACKGROUND: Blunt vascular injuries to the head and neck (BHVI) represent some of the most devastating and morbid injuries seen by a trauma surgeon. This series reviewed the experience of a single institution to determine if diagnostic and therapeutic guidelines can be established for these uncommon injuries. In particular, the utility of anticoagulation in the treatment of these injuries is examined. METHODS: The institutional trauma registry of a single state-designated Level I trauma center was examined for patients with BHVI. Patients were identified and their charts reviewed individually

Acute injuries to cervical joints. An autopsy study of neck sprain.

A comparative study of cervical spines from 16 subjects who died of major trauma and 16 control subjects who died of natural causes, showed clefts in the cartilage plates of the intervertebral discs in 15 of 16 spines from the trauma victims. These were quite distinct from the uncovertebral clefts and central disc fissures that are a normal feature of aging in cervical discs. Posterior disc herniation through a damaged anulus fibrosus and hemarthrosis in facet joints were also observed. No directly comparable lesions were found in the control subjects, but two discs in this group showed "rim l
