Anatomic basis of notch deformity in open rhinoplasty.

Notch deformity at the columella after the stairstep incision is an unsightly sequel that fuels negativism for open rhinoplasty critics. Obvious causes cited include surgical misadventures involving division of the foot of the medial crus and poor healing. The authors offer yet an additional etiology based on the contraction distortion caused by the depressor septi nasi muscle. The purpose of this study is to investigate the anatomic basis for notch deformity after stairstep technique in open rhinoplasty. For this anatomic study, 10 fresh cadavers were used. Dissections were performed, exposin

Anabolic steroid use by amateur athletes: effects upon psychological mood states.

Twenty-one male amateur athletes attending a Welsh needle-exchange clinic, were asked to complete the Buss-Durke Inventory on feelings of hostility/aggression, and a feeling state questionnaire. All were weight training in local gyms in order to increase their body mass. They were also using high doses of anabolic steroids during 6-14 week cycles, while between these cycles they were steroid free. Subjects reported significantly higher feelings of aggression, aggression towards objects, verbal aggression, and aggression during training (but not physical aggression towards people), during the o

Personal worth, self-esteem, anomia, hostility and irrational thinking of abusing mothers: a multivariate approach.

Eighteen mothers identified as the perpetrator of physical abuse of their children were matched with respect to race, income and educational level with control mothers of children in daycare centers. Tests included: The Sense of Personal Worth Scale of the California Test of Personality, the Srole Anomia Scale, the Rosenberg Self-Esteem Scale, three scales from the Buss-Durkee Hostility Inventory and the Rational Behavior Inventory (RBI). An overall MANOVA was performed on scores of the seven personality measures. Stepdown F-tests were conducted on scores of the seven test measures. With the R

Catatonia. A prospective clinical study.

We studied 55 patients admitted during 14 months to two inpatient psychiatric units of a municipal hospital who exhibited one or more of the catatonic signs of mutism, stereotypy, posturing, catalepsy, automatic obedience, negativism, echolalia/echopraxia, or stupor. Only four of the 55 patients satisfied our research criteria for schizophrenia, whereas over two thirds had diagnosable affective disorders, usually mania. The eight catatonic motor signs were nonspecific and homogeneously distributed among the various research diagnostic groups, with the number and type of individual signs unrela

Periodic catatonia in the elderly: A case series highlighting the diagnostic challenge between delirium and catatonia.

Differentiating catatonia from delirium in elderly patients presents significant diagnostic challenges due to overlapping clinical features. This case series describes three elderly patients with periodic catatonia characterized by alternating states of stupor and agitation. Specific features, such as negativism and refusal to eat, were pivotal in distinguishing catatonia from delirium. Febrile episodes in these cases appeared to exacerbate symptoms, suggesting a potential role for systemic inflammation and cortico-striato-thalamo-cortical (CSTC) circuit dysfunction in the pathophysiology. [The use of fluconazole in low-weight neonates with candidiasis].

Systemic Candida Albicans infections in premature newborns are characterized by high morbidity; early diagnosis and immediate treatment are therefore necessary. In this study we report the cases of fourteen newborns admitted to our Newborn Care Unit in a three-year period, in whom candidiasis was diagnosed. The mycetes was isolated from blood, liquor, peritoneal fluid, urine, oesophageal aspirate. Treatment was undertaken immediately with fluconazole, administered through the parenteral route or locally into peritoneal cavum and oesophageal stump. Duration of treatment varied from five to twen

Kidney organ donation knowledge and attitudes among health care professionals: findings from a Greek general hospital.

Identified barriers of organ donation advancement include lack of knowledge, personal beliefs, and a negative attitude from health professionals. This article reports on current knowledge and attitudes toward kidney donation among nurses and physicians in a Greek general hospital. A previously used questionnaire was applied. More physicians than nurses were donor card holders, with registration rates being lower than expected. Over half of the participants did not consider themselves well informed about registering as a kidney donor. Older nurses differed significantly from younger ones in the

Catatonia and mania: patterns of cerebral dysfunction.

We performed a factor analysis on research data from 55 consecutive hospitalized psychiatric patients who showed one or more of eight catatonic motor features. Two factors were extracted, accounting for 32% of the variance. Factor 1 (mutism, negativism, stupor) corresponded to the clinical syndrome of negativistic stupor and was unrelated to diagnosis, sex, age at onset, family history, or treatment response. Factor 2 (mutism, stereotypy, catalepsy, automatic obedience) corresponded to the classical description of catatonia, was associated with a research diagnosis of mania, and tended (p less

Neonatal lupus erythematosis: a five-year case review.

Neonatal lupus erythematosus is an infrequent disease seen in newborns. It is caused by transplacental maternal autoantibody passage. Cutaneous involvement and congenital heart block (CHB) are the most common affections, although it may involve multiple organs like the liver, lungs, blood, nervous or digestive systems. This article present a review of the four cases diagnosed in the past five years in a Neonatal Unit, which shows the different clinical spectrum which can develop around this disease (CHB, multisystemic affection and two cutaneous cases), different autoantibodies (specially anti

[Neurolues--clinical characteristics, diagnosis and therapy].

A female patient with neurolues was presented in this paper. The disease started 25 years after primoinfection by neurologic disturbances which corresponded to classic neurolues with the signs of meningovascular and parenchymatous disease. The diagnosis was confirmed by positive serologic tests, computerized brain tomography--whereby syphilomas were found in the brain and by histopathologic survey of one of peripheral syphilomas. Antibiotic therapy with high penicillin doses was initiated but without more significant clinical effects, it only led to negativization of serologic tests. The patie

Case of paediatric catatonia precipitated by antipsychotic withdrawal in a child with autism spectrum disorder.

A 13-year-old girl with moderate intellectual disability and autism spectrum disorder (ASD) was admitted to the paediatric high-dependency unit following an 8-week history of altered mental status and motor behaviour. Her symptoms emerged followed shortly after discontinuation of risperidone, an atypical antipsychotic previously commenced to manage disruptive behaviour associated with ASD. On physical examination, the patient presented with negativism, grimacing, automatic obedience, waxy flexibility and ambitendency. Blood tests, neuroimaging and lumbar puncture failed to reveal an acute infe

Electroconvulsive therapy for pediatric malignant catatonia with cerebellar dysgenesis.

Electroconvulsive therapy was successfully used to treat malignant catatonia in a 15-year-old male patient with congenital dysgenesis of the left hemisphere of the cerebellum and hypoplasia of the vermis and left pons due to a presumed cerebral vascular accident in utero. The patient experienced significant motor and communication delays with mild cognitive impairment, but was otherwise in good health until age 15 years, when he developed rigidity, posturing, stupor, unresponsiveness, repetitive self-injurious behaviors, and negativism, as well as autonomic abnormalities including profuse diap

Schizophrenia-like illness in velo-cardio-facial syndrome: a genetic subsyndrome of schizophrenia?

BACKGROUND: The study of genetic illnesses that have a behavioral phenotype resembling psychosis can provide important insights into the genetic basis of psychotic disorders and their patho-mechanisms. An important example of such a genetic disorder is the velo-cardio-facial syndrome (VCFS) associated with 22q11 microdeletion. CASE REPORT: The case of a 22-year-old male, who had the typical genotype and phenotype of VCFS and developed a psychotic illness characterized by withdrawal, catatonic posturing, inappropriate affect, stereotyped behavior, negativism and poverty of speech, is described

[Depression and aggression. A control group study on the aggression hypothesis in depressive disorders based on the Buss-Durkee Questionnaire].

PURPOSE: The aim of this study was to prove the hypothesis that covert or inhibited aggression/hostility as described in psychoanalytic literature is part of the psychogenetic development of depression. METHOD: We compared 50 consecutively admitted depressed inpatients to 50 healthy people of the regionally living general population, matched by sex and age, using the Buss-Durkee Hostility Inventory. RESULTS: Depressed patients have significantly higher values in the BDHI subscales "total aggression", especially in irritability, negativism, resentment, suspicion and guilt, also in "attitudion

Rumination is characterized by valence-specific impairments in switching of attention.

The ability to flexibly switch attention between emotional and non-emotional information in working memory is considered important in stress-resilience and is impaired in mood disorders. A recent theory claims that this component of attention is specifically related to ruminative thought. To further investigate this claim we report two new experiments using the internal shift task (IST). In Experiment 1 (N=27) we examined the stability of switching ability measured using the IST through examination of internal consistency (stability within the task) and test-retest-reliability (stability over

The impact of negative moods on self-enhancing cognitions: the role of reflective versus ruminative mood orientations.

This research program examined how self-focused attention to feelings affects the relation between mood negativity and self-enhancing thought. The primary hypothesis was that the particular manner in which people focus on their moods (reflective vs. ruminative) determines whether they reveal positive (i.e., mood-incongruent) or negative (i.e., mood-congruent) self-relevant thoughts in response to negative moods. Studies 1-4 revealed that social comparisons, temporal comparisons, and other self-enhancing cognitions (i.e., attributions, disidentification, relationship evaluations) are more likel

Cognitive behavioural therapies for fibromyalgia.

BACKGROUND: Fibromyalgia (FM) is a clinically well-defined chronic condition of unknown aetiology characterized by chronic widespread pain that often co-exists with sleep disturbances, cognitive dysfunction and fatigue. Patients often report high disability levels and negative mood. Psychotherapies focus on reducing key symptoms, improving daily functioning, mood and sense of personal control over pain. OBJECTIVES: To assess the benefits and harms of cognitive behavioural therapies (CBTs) for treating FM at end of treatment and at long-term (at least six months) follow-up. SEARCH METHODS: We

[Typology of early childhood autism].

On the basis of the study of 96 children with the syndrome of early childhood autism the author proposes a model of the pathopsychological structure of this syndrome with primary disturbances directly related to the disease and secondary manifestations of mental abnormalities and pathological hypercompensation. On the basis of this structure the author has identified four groups of children differing in the severity of primary disturbances, gravity of mental disorders, and forms of hypercompensation. The proposed typology may be useful for gaining better understanding into the regularities of

[Case report: respiratory infection due to Alcaligenes xylosoxidans in a patient with Mounier-Kuhn syndrome].

Mounier-Kuhn syndrome is a rare entity characterized by abnormal dilatation of the trachea and main bronchi (tracheobronchomegaly). Alcaligenes xylosoxidans is a non fermenting gram-negative pathogen common in extra-and intra-hospital environment, which may be related to immunosuppression states. We describe the case of a 75 years old male, ex-smoker with moderate functional obstruction, chronic respiratory failure and chronic colonization by Pseudomonas aeuriginosa. He had an infectious exacerbation of his disease, reason that previously required several hospital admissions. The patient was t
