Inequities in prenatal neonatology consultation in high-mortality neonatal populations.

OBJECTIVES: To explore inequities in prenatal consultation and parental resuscitation decisions across high-mortality conditions. STUDY DESIGN: We conducted a retrospective chart review of pregnant people whose liveborn neonates were diagnosed with high-mortality conditions. We examined two cohorts: periviable infants (22 0/7-24 6/7 weeks) and infants with severe congenital anomalies. RESULTS: A total of 194 neonates met eligibility criteria for the periviable cohort, 197 for the congenital anomaly cohort. In the periviable cohort, 94% of White vs. 81% of Black pregnant people received neona

Strengthening Neonatology in Ethiopia: From Survey Data to System Improvement.

INTRODUCTION: Neonatal care in low-resource settings is hindered by shortages of trained staff, inadequate infrastructure, and limited equipment and medications that compromise the management of common neonatal conditions and reduce the quality of care. Our aim was to describe the collaborative efforts between the Italian Agency for Development Cooperation (AICS), the Union of European Neonatal and Perinatal Societies (UENPS), Doctors with Africa Collegio Universitario Aspiranti Medici Missionari (CUAMM), the Ethiopian Pediatrics Society (EPS), and the Ethiopian Federal Ministry of Health (FMo

[The wonderful world of steroid hormones in neonatology - the first 50 years].

Professor László Karmazsin established the contemporary neonatal intensive care at the Department of Pediatrics of the University of Debrecen. His research group’s achievements in the field of neonatal immunobiology contributed to advances in perinatology. Investigating the antioxidant system in neonates, he and his group played a major role in uncovering the pathomechanisms of conditions associated with prematurity. At the same time, corticosteroids, applied as antenatal prophylaxis, and another, endogenous steroid hormone, active vitamin D, have also been shown to significantly impact fetal

Lung organoids: a new frontier in neonatology and paediatric respiratory medicine.

Great strides have been made in pre-clinical research in recent decades using animal models and cell lines. However, traditional models may fail to translate to humans, resulting in substantial failure rates in drug development. Recent three-dimensional organoid models have borne a good resemblance to the architecture, development and function of tissues, especially for organs with complex cell interactions and dynamics such as the lungs. In 2022, the role of organoids as alternative to animal testing was recognised by the US Food and Drug Administration. We searched Medline and ClinicalTrials

Tele-support in breastfeeding: position statement of the Italian society of Neonatology.

Tele-support in breastfeeding can be defined as any support provided by a service that connects health workers and/or lactation consultants with breastfeeding parents through video visits, although a telephone contact with the breastfeeding mother remains the first method of remote breastfeeding support. The tele-support in breastfeeding has increased significantly during the SARS-CoV2 pandemic worldwide and, given its effectiveness, may be maintained also after the pandemic. The Italian Society of Neonatology on the basis of: 1) two focus group studies on the tele-support in breastfeeding con

[Paracetamol (acetaminophen) use in neonatology: a (re)appreciation of an old drug].

In neonates, paracetamol is mainly used for its analgesic action. This drug is actually preferred by neonatologists because of its broad therapeutic index. Recently, it has been demonstrated that paracetamol is also an anti-cyclooxygenase (COX) medication through its inhibitory action on the peroxidase arm of central and peripheral COX (Boutaud et al., 2002; Toussaint et al., 2010; Graham et al., 2013; Hinz et al., 2008; Hinz and Brune, 2011). As such, this drug interferes with the synthesis of prostaglandins. This inhibition of peroxidase is, however, limited to a low concentration of arachid

[Medication errors: analysis of a digoxin overdose case in a neonatology unit].

Neonatology and pediatrics are units where medication errors occur. Indeed, the complexity of nursing care, the lack of information and marketing authorization for drugs contribute to the occurrence of often underestimated iatrogenic events. Through a case of digoxin overdose in a neonatology unit, each stage of the drug circuit was analyzed. From prescription to administration to dispensation, the accumulation of individual errors put a newborn in danger. The analysis and declaration of such events can improve safety and the quality of patient care.

Ethics Education in Neonatology: Integrating Theory, Multimodal Methods, and AI Innovation.

This review examines current approaches to ethics education in neonatal-perinatal medicine (NPM), integrating adult learning theory, diverse teaching modalities, and emerging technologies to enhance training for neonatal clinicians. Foundational strategies include case-based discussions, simulation, interprofessional learning, and structured debriefing, which promote critical reflection, moral reasoning, and shared decision-making skills. Assessment of ethics education is challenging given the interplay of cognitive, emotional, and interpersonal domains. Tools such as knowledge tests, mileston

Knowledge, Attitudes, and Perceptions about Antibiotic Stewardship Programs among Neonatology Trainees.

OBJECTIVE: Antibiotic stewardship should be an essential component of neonatology training as neonatal intensive care units (NICU) have unique stewardship needs. Our aim was to assess neonatology fellowship trainees' knowledge, attitudes, and perceptions about antibiotic stewardship to inform sustainable curriculum development. STUDY DESIGN: We distributed an electronic survey to neonatology fellows in the United States over 4 months (January-April 2018) via Accreditation Council for Graduate Medical Education program directors. RESULTS: Of 99 programs in the United States with an estimated

[Aftercare for bereaved parents in neonatology: closing a gap in professional support].

Thanks to enormous medical advances, the survival rate of seriously ill newborns and premature babies improved significantly at the end of the last century. However, stagnation has been observed in recent decades. Neonatal palliative care therefore remains an important part of care for seriously ill preterm and newborn infants in neonatology. Parents who experience the loss of a newborn are at an increased risk of developing mental health disorders. As a result, bereaved families face unique emotional and psychological challenges. To date, Germany has not implemented a comprehensive, structure

An outbreak of RSV infections in a neonatology clinic during the RSV-season.

BACKGROUND: Respiratory syncytial virus (RSV) is the predominant cause of lower respiratory tract infections (LRTI) in infancy. Preterm infants with bronchopulmonary dysplasia (BPD) are at the highest risk of severe RSV-LRTI. This is a retrospective study that analyses a nosocomial outbreak of RSV infections in the Neonatology clinic of the University Hospital of Obstetrics and Gynecology, Sofia, 2019. METHODS: Two groups of infants without contact between them were diagnosed with RSV-infection: 14 infants were treated in the Department for healthy newborns - Group 1, and 7 preterm infants we

[Lung Ultrasound in Neonatology to diagnose a Pneumothorax (part two): A Guideline].

Lung ultrasound is a fast and reliable diagnostic tool in the detection of a neonatal pneumothorax. This tutorial provides basic information for understanding and detecting the generated artefacts, such as A-lines, B-lines, and the lung point. Essential knowledge of these artefacts allows for rule-in and rule-out criteria of a pneumothorax diagnosis. Lung ultrasound could help to reduce the use of radiation in neonatology in the future.
