Intraductal tubulopapillary neoplasm (ITPN) of the pancreas associated with an invasive component: a case report with review of the literature.

BACKGROUND: Intraductal tubulopapillary neoplasm (ITPN) depicts a distinct entity in the subgroup of premalignant epithelial tumors of the pancreas. Although the histomorphological and immunophenotypical characterization of ITPN has been described by several authors in terms of report of case series in the past, the rarity of that tumor subtype and similarity to other entities still makes identification of ITPN a challenge for radiologists and pathologists. To date, little is known about tubulopapillary carcinoma that can evolve from ITPN. CASE PRESENTATION: In the present work, we analyze on

Validating a simple scoring system to predict malignancy and invasiveness of intraductal papillary mucinous neoplasms of the pancreas.

BACKGROUND: The objective of the present study was to identify reliable preoperative factors predicting malignancy or invasiveness of intraductal papillary mucinous neoplasm (IPMN) of the pancreas and the effectiveness of a diagnostic scoring system based on these factors. METHODS: Between August 1994 and December 2007, 204 patients underwent pancreatic resection for IPMN at a single institute. Medical records were reviewed retrospectively, and a new diagnostic scoring system for predicting malignant IPMN preoperatively was designed. RESULTS: Univariate analysis revealed nine significant pre

[Assessment for development of superficial colorectal neoplasm--endoscopic and clinicopathologic analysis of 149 submucosal invasive carcinomas].

We performed the endoscopic and clinicopathologic analysis for the development of superficial colorectal carcinoma, using 149 submucosal (sm) invasive colorectal carcinomas. It was observed that superficial colorectal carcinomas had a tendency to rise by their sm massive invasion. In this study, we judged that the sm colorectal carcinomas originated from superficial colorectal carcinoma were 37 (25%) of 149 lesions, and their distribution in the colon and rectum was similar to that of advanced colorectal carcinomas, although the lesions originated from non-superficial (polypoid) colorectal car

CT imaging and clinicopathologic differentiation of intracholecystic papillary neoplasm with associated invasive carcinoma and conventional gallbladder adenocarcinoma.

PURPOSE: To investigate the different imaging and clinicopathologic features between intracholecystic papillary neoplasm with associated invasive carcinoma (ICPN-IC) and conventional GB adenocarcinoma (c-GBA). METHODS: We retrospectively reviewed medical records for clinicopathological variables and CT scans of patients who underwent surgery and pathologically confirmed ICPN-IC or c-GBA between 2006 and 2022. Two radiologists independently evaluated CT features, including wall thickness, enhancement pattern, irregularity of inner wall, pericholecystic infiltration, gallstones, lymphadenopathy

Dynamic assessment of myelofibrosis progression in myeloproliferative neoplasm mouse model using a minimally invasive evaluation system.

Myelofibrosis (MF) is the most common complication of myeloproliferative neoplasms (MPNs),which is markedly correlated with a dismal prognosis. Murine model such as the thrombopoietin receptor (MPL)W515L-mutant MPN mouse model functions as a crucial vehicle in disease research, is widely used in basic and applied research on MPNs and MF.However, the lack of methods for dynamic observation of MF progression hinders mechanistic studies and drug development for MF. Here we develop a sensitive, stable and minimally invasive MPN evaluation system to assess the evolution of myelofibrosis in murine m

Grossly visible non-invasive neoplasm of the gallbladder: method of classification and its relationship to pyloric gland adenoma.

OBJECTIVES: To devise a classification method for grossly visible non-invasive neoplasms (GVNINs) of the gallbladder and examine their relationship to pyloric gland adenoma (PGA), since clinicopathological features of GVNINs are not well known to date and the relationship between PGA and GVNINs remains unknown. METHODS: Eighty-five GVNINs were classified into pedunculated (PE), sessile type 1 (SE1), and sessile type 2 (SE2) groups, and into histologic subtypes. Clinicopathologic data, immunohistochemical data surrogating gene abnormalities, and mutational data of CTNNB1, KRAS, and GNAS were o

[Case of Intraductal Papillary Neoplasm of Bile Duct with Invasive Carcinoma after Choledochoduodenostomy for Intrahepatic Duct Stones].

Endoscopic retrograde cholangiopancreatography or percutaneous techniques are performed for bile drainage and removing bile duct stones. Nevertheless, surgical stone removal can be performed in cases where cholelithiasis with concomitant choledocholithiasis is checked. While forming an anastomosis for bile drainage, the anatomical structure may change from its normal state. Choledochoduodenostomy is one classical and commonly used method for bile drainage anastomosis. The data on long-term complications, including malignancy, associated with this method are limited. Some reports suggest that c

[A Case of Invasive Intraductal Papillary Mucinous Neoplasm(IPMN)Where Determining the Intraoperative Resection Margin Was Challenging].

An 75-year-old man was diagnosed with a 4-mm branch-duct type intraductal papillary mucinous neoplasm(IPMN)in the pancreatic tail and placed under observation. After 3 years and 6 months, endoscopic ultrasound(EUS)revealed a suspected pancreatic mass, prompting referral. Contrast-enhanced CT showed a 7-mm dilation of the main pancreatic duct without clear enhancing nodules. ERCP detected a 2-cm region with poor visualization of the branch ducts and ductal narrowing. MRCP revealed a faint high-intensity signal on diffusion-weighted imaging. EUS identified a 7-mm hypoechoic mass with irregular m

Pancreatic intraductal tubulopapillary neoplasm with associated invasive cancer successfully treated by total pancreatectomy: A case report.

A 74-year-old male was admitted to Departments of Surgery, Toyonaka Municipal Hospital (Osaka, Japan) for treatment of a pancreatic tumor. Contrast enhanced computed tomography (CT) revealed a mass with small cystic lesions in the pancreatic head and body. Fluorodeoxyglucose-positron emission tomography/CT revealed an abnormal uptake of fluorodeoxyglucose, corresponding to the mass lesions. Upper gastrointestinal endoscopy revealed rough mucosa near the opening of the accessory pancreatic duct, and the mucosa biopsy exhibited adenocarcinoma with no mucin observed. The preoperative diagnosis wa

Intracholecystic Papillary Neoplasm With Associated Invasive Carcinoma, Accompanied by Tumor Thrombi of Hepatoid Adenocarcinoma: A Case Report.

Hepatoid adenocarcinoma (HAC) is a rare extrahepatic adenocarcinoma characterized by hepatocellular differentiation. HAC arising in the gallbladder is rare. We report a case of intracholecystic papillary neoplasm (ICPN) with HAC, most of which were tumor thrombi. The patient was a 78-year-old Japanese woman diagnosed with a gallbladder mass, detected by ultrasonography. She underwent cholecystectomy with lymph node dissection. Histopathological examination revealed exophytic papillary growth consistent with ICPN with associated invasive carcinoma. Notably, many tumor nests displayed characteri

Rapid transformation of branched pancreatic duct-derived intraductal tubulopapillary neoplasm into an invasive carcinoma: A case report.

BACKGROUND: Intraductal tubulopapillary neoplasm (ITPN) is a rare disease accounting for approximately 3% of all intraductal pancreatic tumors, with intraductal papillary mucinous neoplasm (IPMN) being one of the most common differential diagnoses. Both ITPN and IPMN display slow growth. A branched pancreatic duct type is commonly observed in IPMN, whereas ITPN derived from the branched pancreatic duct has been reported in a limited number of cases; hence, its pathogenesis remains unclear. CASE SUMMARY: Here, we present the case of a patient with ITPN localized in a branched pancreatic duct,

Retrospective Review of Outcomes in Non-Invasive Mucinous Appendiceal Neoplasms with and without Peritoneal Spread: A Cohort Study.

Patients treated surgically for local non-invasive mucinous appendiceal neoplasm (NI-MAN) may recur with the development of peritoneal dissemination (PD). The risk of recurrence and predictive factors are not well studied. Patients with NI-MAN, with or without peritoneal dissemination at presentation, were included. Patients with limited disease underwent surgical resection only. Patients with peritoneal dissemination underwent cytoreductive surgery (CRS) with or without hyperthermic intraperitoneal chemotherapy (HIPEC). Patients without PD (nPD) were compared to those who presented with PD. T

Intracholecystic papillary neoplasm associated with invasive carcinoma of the remnant gallbladder after subtotal cholecystectomy: a case report.

BACKGROUND: Intracholecystic papillary neoplasm (ICPN) of the gallbladder is a rare tumor and a relatively new concept. Therefore, the natural history and imaging characteristics of ICPN have not yet been fully documented. Moreover, cases who underwent curative resection for remnant gallbladder cancer, including ICPN with associated invasive carcinoma, have been rarely reported. We report a resected case of ICPN of the remnant gallbladder with associated invasive carcinoma for which we could observe a temporal change in imaging findings until malignant transformation. CASE PRESENTATION: A 79-

Surgical outcomes of robotic transanal minimally invasive surgery for selected rectal neoplasms: A single-hospital experience.

BACKGROUND: Rectal neoplasm is one of the most common malignancies worldwide. Screening programs for rectal neoplasm result in early diagnosis and a decrease in disease-related mortality and morbidity. In selected patients, early rectal cancer may be treated with local excision. Owing to poor exposure during conventional transanal excision, transanal minimally invasive surgery (TAMIS) was developed, and TAMIS is feasible for the local excision of selected rectal neoplasms. However, the limited range of motion is a major disadvantage of this operation. Therefore, robotic TAMIS was developed to
