[Nephrolithiasis-induced ESRD: frequency, causes and prevention].

Nephrolithiasis still remains a too frequent - and under-appreciated - cause of end-stage renal disease (ESRD), and this is all the most unfortunate since such an untoward course is now preventable in most cases. Among 1391 patients who started maintenance dialysis at Necker hospital between 1989 and 2000, nephrolithiasis was identified as the cause of ESRD in 45 of them, an overall prevalence of 3.2%. Infection stones accounted for 42.2% of cases, calcium stones for 26.7%, uric acid stones for 17.8% and hereditary diseases for 13.3%. The proportion of nephrolithiasis-associated ESRD declined

Recurrent calcium nephrolithiasis associated with primary aldosteronism.

Typical manifestations of hyperaldosteronism include salt retention, hypokalemia, and metabolic alkalosis. However, a consequence infrequently recognized and described is hypocitraturia. In combination with hypercalciuria, aldosterone-induced hypocitraturia can trigger calcium nephrolithiasis. The authors report a case of an individual with primary hyperaldosteronism from an adrenal adenoma that resulted in hypocitraturia. The patient had severe recurrent renal calcium calculi that corrected with adrenalectomy. The clinical physiology of renal calcium and citrate handling in hyperaldosteronism

X-linked recessive nephrolithiasis with renal failure.

BACKGROUND AND METHODS: Nephrolithiasis may occur as a consequence of a number of hereditary disorders. We describe a large kindred from northern New York with hereditary nephrolithiasis accompanied by urinary concentrating defects, nephrocalcinosis, renal insufficiency, and renal wasting of potassium, phosphate, calcium, and uric acid. The pattern of inheritance was established by examining the patients and their records and interviewing family members. Selected members of the family were evaluated in detail, with measurements of erythrocyte cation fluxes and carbonic anhydrase (carbonate deh

Tubulopathy in nephrolithiasis: consequence rather than cause.

To address whether a renal tubular dysfunction is encountered in a particular patient subgroup with urolithiasis, the following parameters of tubular function were measured in urine taken in the morning from 214 stone formers after fasting: pH, excretion of lysozyme and gamma-glutamyl transferase (gamma-GT); fractional excretion (FE) of glucose, insulin, Mg, K, and HCO3 after an alkali loading; and the renal threshold for phosphate (TmP/GFR). The following diagnoses were made in the patient group: primary hyperparathyroidism (N = 8), medullary sponge kidneys (N = 21), hyperuricemia (N = 10), c

Metachronous parathyroid adenoma characterised by recurrent nephrolithiasis.

Hyperparathyroidism is often diagnosed after identifying hypercalcaemia through routine testing in asymptomatic patients; however, many patients experience severe and painful symptoms related to their diagnosis, including nephrolithiasis and bone pain with osteoporosis. Double adenomata are an uncommon cause of hyperparathyroidism, and they occasionally occur metachronously with recurrent symptoms years apart. This case report examines a woman in her 60s with a history of parathyroid adenoma 9 years earlier who presented with recurrent nephrolithiasis. She was found to have a recurrence of pri

[Experimental nephrolithiasis: nephroprotective effect of calcium antagonists].

Increasing intracellular calcium ion concentration in the cytoplasm is the starting point in the activation of cell death. Regulation of these pathological processes in various organs and tissues is possible using agents from the group of calcium antagonists. This study was aimed at assessing the level of urinary excretion of two enzymes, lactate dehydrogenase (LDH, EC 1.1.1.27) and y-glutamyltransferase (GGT, EC 2.3.2.2), calcium antagonists in an experimental model of nephrolithiasis and finding ways to pharmacological protection of the kidneys. It established that the use of calcium antagon

Nephrolithiasis in autosomal dominant polycystic kidney disease.

BACKGROUND AND PURPOSE: Autosomal dominant polycystic kidney disease (ADPKD) manifests with renal and extrarenal abnormalities and is inherited in an autosomal dominant fashion. In addition to multiple renal cysts, abnormalities such as liver cysts (80%), pancreatic cysts, splenic cysts, pulmonary cysts, berry aneurysms in the distribution of arterial circle of Willis (8%), colonic diverticula, mitral valve prolapse, etc., can be present. The condition will develop in half of the offspring of affected persons because of its 100% penetrance. Nephrolithiasis in patients with ADPKD is not infrequ

Genetics of hypercalciuric nephrolithiasis: renal stone disease.

Renal stone disease (nephrolithiasis) affects 5% of adults and is often associated with hypercalciuria. Hypercalciuric nephrolithiasis is a familial disorder in more than 35% of patients, and may occur as a monogenic disorder, or as a polygenic trait involving 3 to 5 susceptibility loci in man and rat, respectively. Studies of monogenic forms of hypercalciuric nephrolithiasis in man, for example, Bartter syndrome, Dent's disease, autosomal dominant hypocalcemic hypercalciuria (ADHH), hypercalciuric nephrolithiasis with hypophosphatemia, and familial hypomagnesemia with hypercalciuria have help

Nephrolithiasis during pregnancy secondary to primary hyperparathyroidism.

Nephrolithiasis secondary to primary hyperparathyroidism infrequently complicates pregnancy. It can cause severe maternal and fetal complications. We present a case of a pregnant woman with nephrolithiasis and primary hyperparathyroidism. We reviewed the management of nephrolithiasis due to primary hyperparathyroidism during pregnancy. We believe that early recognition and timely intervention can significantly reduce the incidence of complications.

Renal tubular defects in recurring bilateral nephrolithiasis.

A metabolic study was performed in 28 patients with bilateral recurrent calcium-containing renal staghorn calculi and chronic pyelonephritis ("obstructive nephropathy"). Fourteen had normal GFR and 14 mild renal insufficiency. Ten normal subjects were used as controls. Under basal conditions, polyuria and negative sodium balance were commonly observed in patients with obstructive nephropathy and normal renal function. After an acute acid load (NH4Cl) an acidifying defect, i.e. high values for urine pH and reduced excretion of titratable acid and ammonium, was observed in 64% of patients with n

Contemporary Review of Overlap in Overactive Bladder Patients with Nephrolithiasis: Fluid Reduction in Overactive Bladder and Associated Factors Related To Nephrolithiasis.

PURPOSE OF REVIEW: This contemporary review paper analyzes the impact of fluid reduction in overactive bladder and associated factors related to nephrolithiasis. RECENT FINDINGS: Historically, overactive bladder and nephrolithiasis have been considered separate urologic pathologies. However, patients with OAB symptoms often restrict fluid intake to prevent episodes of urinary incontinence with resultant supersaturated urine, which precipitates nephrolithiasis formation. Coexistent pathologies promote the relationship between overactive bladder and nephrolithiasis, including urinary tract infe

The Impact of Water and Other Fluids on Pediatric Nephrolithiasis.

Pediatric nephrolithiasis cases have been on the rise in the past several years, resulting in increased healthcare costs and other burdens on the juveniles with this ailment. Recent research has shown that present trends in pediatric nephrolithiasis have changed as a result of fluid intake, including water consumption, nutrition, obesity prevalence, lifestyle, and imaging procedures. A specific cause, meanwhile, is still elusive. Trends in pediatric nephrolithiasis need to be thoroughly researched. Furthermore, variables specific to pediatric nephrolithiasis that could cause greater difficulti

Safety of megadose of vitamin D in patients with nephrolithiasis.

OBJECTIVE: This article describes two patients with renal lithiasis who received a megadose of 25-hydroxy vitamin D (25[OH]D) and had a good outcome. METHODS: The first case reports a 74-year-old man with a long-term history of renal lithiasis and about four episodes of renal crisis. He was treated once with extracorporeal shock wave lithotripsy. He also had a history of dyslipidemia, myocardial infarction, and stroke. Laboratory tests demonstrated 25(OH)D of 28 ng/mL (normal range (nr): >30 ng/mL), normal lipid levels, creatinine of 1.1 mg/dL, and homocysteine of 26.6 mcmol/L (nr: 5-15 mcmol

Stone composition and vascular calcifications in patients with nephrolithiasis.

Kidney stone disease is associated with cardiovascular outcomes; it is unclear whether stone composition is associated with differential cardiovascular risk. To analyze such association, we performed a cross-sectional study in which data were collected for patients who underwent at least one stone composition analysis from January 01 2015 to May 30 2018. The original dataset was linked with the imaging database to identify those patients with at least one abdominal CT scan examination during the period of interest. In total, 180 patients were included. The outcome of interest was the presence

Metabolic and Hypertensive Complications of Pregnancy in Women with Nephrolithiasis.

BACKGROUND AND OBJECTIVES: Kidney stones are associated with future development of hypertension, diabetes, and the metabolic syndrome. Our objective was to assess whether stone formation before pregnancy was associated with metabolic and hypertensive complications in pregnancy. We hypothesized that stone formation is a marker of metabolic disease and would be associated with higher risk for maternal complications in pregnancy. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: We conducted a retrospective cohort study of women who delivered infants at the Massachusetts General Hospital from 2006
