The prevalence of double- and multiple crush syndromes in patients surgically treated for peripheral nerve compression in the upper limb.

OBJECTIVE: The double crush syndrome describes a condition characterized by multifocal entrapment of a nerve. In the upper limb, the high prevalence of carpal tunnel syndrome makes it a common diagnosis of assumption in the setting of median neuropathy. More proximal compressions may tend to be overlooked, under-diagnosed and under-treated in the population. This study aims to map the prevalence of peripheral upper limb nerve compressions among patients undergoing peripheral nerve decompression. METHODS: A prospective case series was conducted on 183 patients undergoing peripheral nerve decom

Long-Term Results of Macrovascular Decompression Using Suture-Pexy Transposition Technique for the Treatment of Cranial Nerve and Brainstem Compression Syndromes.

BACKGROUND AND OBJECTIVES: Macrovascular compression caused by a dolichoectatic vertebral or basilar artery represents a distinct subset of cranial nerve (CN) and brainstem compression disorders. Standard interpositional microvascular decompression (MVD) may not adequately decompress a large dolichoectatic vessel, necessitating vessel transposition. This study reports 2 decades of institutional experience with macrovascular decompression (MaVD) using a vascular mobilization and suture-pexy technique and evaluates long-term outcomes. METHODS: We retrospectively reviewed all patients undergoing

The Association Between Concomitant Ulnar Nerve Compression at the Elbow and Carpal Tunnel Syndrome.

Background: Many patients treated for ulnar nerve compression at the elbow (UNE) are concomitantly treated for carpal tunnel syndrome (CTS). We sought to investigate the association between the conditions. Methods: The Statewide Planning and Research Cooperative System (SPARCS) database was used to determine the number of patients with UNE concomitantly treated for CTS in New York State from 2003 to 2014. We then retrospectively reviewed each patient who received surgical treatment for UNE (n = 222 patients) or CTS (n = 1063 patients) at our tertiary care institution in 2014 and 2015 to assess

Carpal tunnel syndrome: ultrasonographic imaging and pathologic mechanisms of median nerve compression.

Median nerve compression is a well-known cause of carpal tunnel syndrome (CTS). Yet, reasons why the most common idiopathic form of CTS develops in certain individuals are not well understood. To further understand the compressive mechanisms at work in CTS development, the authors used ultrasonographic imaging of the median nerve to evaluate 2 patients with CTS. Findings were compared to those of 2 control subjects who did not have CTS. In the patients who had CTS, the transverse carpal ligament was pulled taut by thenar muscle contraction as the flexor tendons tightened, compressing the media

Unusual complication of an opposition tendon transfer at the wrist: ulnar nerve compression syndrome.

Restoration of thumb opposition by tendon transfer may be necessary in cases of severe thenar atrophy caused by long-standing carpal tunnel syndrome. Routing the extensor indicis proprius transfer subcutaneously around the ulna to reanimate thumb opposition is an accepted procedure and is considered safe. Ulnar nerve compression leading to palsy is possible, however, as shown in the patient presented. Neurolysis failed to improve the palsy. Rerouting of the transfer deep to the ulnar nerve was necessary to treat the iatrogenic condition. Possible nerve compression should be kept in mind when p

[Compression syndrome of the radial nerve in the area of the supinator groove. Experiences with 110 patients].

Both radial tunnel syndrome and posterior interosseous nerve (PIN) compression syndrome are caused by compression of the posterior interosseous nerve. There is a controversy about certain features of PIN compression especially with regard to diagnostic criteria and therapy as well as differentiation from tennis elbow. From 1992 to 1997, we operated 110 patients because of PIN compression. Diagnosis was based on clinical examination only. As a rule, we decompressed the PIN directly using an anterior approach. With regard to radial tunnel syndrome, we could review 69 from 103 operations with an

[Heterotopic ossification and ulnar nerve compression syndrome of the elbow. A report of two cases].

Heterotopic ossification is a well known complication in patients after longterm coma following CNS trauma. The association of heterotopic ossification and a nerve compression syndrome situated near the calcifications is less common. The authors present 2 CNS traumatised patients who secondarily developed the association of heterotopic ossification of the elbow with a compressive syndrome of the ulnar nerve. Although heterotopic ossification does not appear to be the direct cause of the nerve compression, it certainly promotes compression it because of the flexion it causes.

Soleal sling syndrome (proximal tibial nerve compression): results of surgical decompression.

BACKGROUND: Although distal tibial nerve compression is well recognized, proximal tibial nerve compression remains a rarely recognized clinical condition. This report defines the presentation, diagnosis, surgical decompression technique, and clinical outcome of neurolysis of the tibial nerve at this soleal sling compression site. METHODS: Forty-nine patients with 69 proximal tibial nerves (20 bilateral) were stratified retrospectively into three groups: neuropathy (n = 10), failed tarsal tunnel syndrome (n = 25), and trauma (n = 14). Pain level, strength of the flexor hallucis longus muscle,

Typewriter tinnitus: a carbamazepine-responsive syndrome related to auditory nerve vascular compression.

Six subjects with similar unilateral tinnitus that was fully suppressed by carbamazepine have been identified. Their ages at the time of the sudden onset of their tinnitus ranged from 39 to 87 years (mean 67). The 3 men had right ear tinnitus. Two of the 3 women had left ear tinnitus. All 6 described a staccato quality of their intermittent tinnitus ('like a typewriter in the background, pop corn, Morse code'). Five of the 6 subjects had no other hearing or vestibular complaints; their audiograms were symmetric and consistent with their ages. Vascular compression of the auditory nerve ipsilate

What symptoms are truly caused by median nerve compression in carpal tunnel syndrome?

OBJECTIVE: This study aims to identify the symptoms, signs and distributions which are associated with neurophysiological carpal tunnel syndrome (N-CTS), defined by the finding of a median nerve lesion at the wrist through neurophysiologic studies, and to compare them with those of patients with sensory or motor complaints in the upper limbs whose electrophysiologic investigation did not show evidence of this syndrome. METHODS: A cross-sectional study was performed, with prospective gathering of data, following a predetermined protocol. We included all patients older than 12 years who were re

Compression syndrome involving the lateral antebrachial cutaneous nerve in high level quadriplegia.

This report describes an etiology for a compression syndrome involving the lateral antebrachial cutaneous nerve (LACN) that is unique to high level quadriplegia. Two cases are presented that involve this syndrome in C5-C6 quadriplegia. Electrodiagnostic techniques and normal values have previously been established for the LACN, but not involving high-level spinal cord injured patients. A series of asymptomatic high-level quadriplegics at various times since their spinal cord injury was studied for appropriate comparison with the symptomatic cases and previously reported normal values. In the c

Atypical carpal tunnel syndrome with compression of the ulnar and median nerves. Case report.

Clinical signs of ulnar nerve involvement at the wrist level were found in a 51-year-old man. The electrophysiological changes were indicative of a median nerve involvement in the carpal tunnel. At operation, the compressed ulnar nerve was found in the carpal tunnel, together with the median nerve. This localization of the ulnar has not been described previously.

[Differential diagnosis of distal peripheral nerve compression syndrome of the tibial nerve. Case report of primitive neuroectodermal tumor].

Tumors of the tibial nerve are not sufficiently taken into account in the diagnosis of tarsal tunnel syndromes (TTS). They may present with atrophy of the plantar flectors and with disturbances of sensory functions, but far more commonly with pain. Diagnosis is often only achieved after 1-2 years. Taking tumors into account when examining patients presenting with TTS may lead to a more rapid diagnosis and to improvement in patients' prognoses. We present the case of a 37-year-old female in whom a primitive neuroectodermal tumor (PNET) was found to be the cause of long-standing pain in the righ

Some Misconceptions in the Treatment of Cubital Tunnel Syndrome, Radial Tunnel Syndrome, and Median Nerve Compression in the Forearm.

This article discusses ulnar, median, and radial nerve compression in the proximal forearm and elbow and some possible common misconceptions. In particular, the ligament of Struthers extremely rarely causes ulnar neuropathy. Lacertus syndrome and flexor superficialis-pronator syndrome can be diagnosed separately. Surgical release can be through a small incision. Acronyms for compression to radial nerve in proximal forearm can be simplified to radial tunnel syndrome, which includes a mild type (classical radial tunnel syndrome) and a severe type (posterior interosseous nerve (PIN) compression).
