The authors present a newly designed device for ultrasonic guidance of neuroendoscopic procedures. It consists of a puncture adapter that attaches to a rigid endoscope having an outer diameter of 6 mm and is mounted on a small, bayonet-shaped ultrasound probe. This adapter directs the movement of the endoscope precisely within the ultrasonic field of view. The targeted region is identified by transdural insonation via an enlarged single burr-hole approach, and the endoscope is tracked in real time throughout its approach to the target. The procedure has been performed in 10 patients: endoscopi

BACKGROUND: Neuroendoscopic premammilary third-ventriculocisternostomy in non-communicating hydrocephalus represents, to date, the less invasive and effective procedure, whereas the neuroendoscopic approach to complex-hydrocephalus is limited to several small anecdotal series.
METHODS: Among 57 pediatric patients affected by obstructive hydrocephalus, ranging in age from prenatal diagnosis to 3 years (mean 1.2 year+/-11 months) and recruited over a 2-year period, we identified 11 cases with presurgical neuroradiological complex-hydrocephalus, at admission to our Department. In two cases Magne

BACKGROUND: Intraparenchymal hemorrhage (IPH) involving the cerebrum, cerebellum, and brainstem is a critical condition with high mortality. While minimally invasive surgical techniques are widely utilized, the comparative effectiveness of neuroendoscopic surgery (NS) vs. stereotactic aspiration (SA) across different anatomical locations remains underexplored. This study aims to retrospectively compare the effectiveness and safety of NS and SA in a cohort encompassing different IPH locations.
METHODS: A single-center retrospective analysis was conducted on 199 patients with IPH (NS: n = 97; S

Orbital cavernous hemangiomas are the most common benign primary neoplasm of the orbit in adults and can lead to proptosis and optic neuropathy. Resection is the primary treatment for symptomatic cases. Choice of surgical approach depends on the lesion's size, location, and relation to intraorbital structures. The authors present the case of a 40-year-old male with a large symptomatic orbital hemangioma resected through a combined endoscopic transorbital and endonasal approach. This case highlights the advantages of the combined approach, enhancing visibility and minimizing disruption of orbit

External ventricular drainage (EVD) is a mainstay for intraventricular hemorrhage (IVH) treatment, improving ventricular clearance and reducing mortality but with suboptimal complications and morbidity. Meanwhile, several studies have shown that neuroendoscopic drainage (NED) provides superior results and is a promising therapy. Thus, we aimed to compare NED and EVD in patients with IVH. A systematic literature review was conducted using Embase, PubMed, and Cochrane databases. The primary outcome was shunt dependency. Secondary outcomes were infection, length of intensive care unit (ICU) and h

PURPOSE: The optimal management for neonatal intraventricular haemorrhage (IVH) remains uncertain. Neuroendoscopic lavage (NEL) has gained interest as a method for removal of intraventricular haematoma and debris, with outcomes suggesting it to be safe and efficacious for reducing the requirement for ventriculoperitoneal shunt (VPS) insertion. We present a large series assessing outcomes from NEL for neonatal IVH.
METHODS: A retrospective review was carried out, between January 2011 and November 2023, identifying infants who underwent NEL following germinal matrix haemorrhage at our instituti

BACKGROUND: Neurocysticercosis is parasitic infection responsible for intraventricular cysts and consequent obstructive hydrocephalus. While the cysts are sometimes treated endoscopically, the efficacy and safety of neuroendoscopic management for intraventricular neurocysticercosis (IVNCC) remains unclear. Therefore, we conducted a systematic review and meta-analysis to evaluate management outcomes of endoscopic IVNCC.
METHODS: Following PRISMA guidelines, a search of MEDLINE, Embase, and Lilacs databases was performed (inception to September 10, 2024), focusing on the neuroendoscopic managem

Skull base chordoma is a rare bone tumor that is predominantly found in the clival area and considered to originate from the notochordal remnants. Chordoma is characterized by an aggressive nature and has a high risk of repeated recurrence despite multimodal treatments, including extensive surgical resection and high-dose radiotherapy. Thus, extensive surgical resection of the tumor and adjacent bony structures is strongly recommended. However, surgery was difficult because of the deep location of the lesion and involvement of important anatomies such as the cranial nerves and internal carotid

BACKGROUND: A neuroendoscope is a technical advance that allows surgeons to visualize certain regions of the brain that was previously inaccessible through the use of a surgical microscope. Several neuroendoscope designs have been implemented by other neurosurgeons over the past 5 years. The advantage of a neuroendoscope is the addition of a flexible and narrow tip that allows for safe entry into intracranial structures for clinical observation. However, there are some limitations to this approach. Here, we report the use of a modified angioscope as a newly developed neuroendoscope to be emplo

OBJECT: Analysis of the safety and morbidity of neuroendoscopic biopsies (NEBs), as well as the reliability in obtaining an accurate diagnosis, has until now been based on studies with relatively small sample sizes. Through the cooperative efforts of several international medical centers, authors of the present study collected data on a large number of patients to obtain better insight into this issue. When possible, they compared pathology obtained through an NEB with the "gold-standard" pathology obtained in open surgery.
METHODS: Thirteen randomly chosen medical centers in 9 countries coll
