Liepmann's description of apraxia still serves as a point of reference for neurological thinking about higher motor function. The extent to which this model is accessible to interpretation in the light of current neurophysiological knowledge has been examined. Planning and programming processes have been distinguished, and the separate roles of striopallidal and cerebellar loops in the generation of those processes have been considered in relation to the contribution of parietal and frontal association areas. It is emphasized that a new taxonomy of action is imperative in order that neurophysi

This chapter discusses the premotor neural mechanisms that control horizontal saccadic eye movements. Oculomotoneurons carry a pulse-step signal that underlies the pulse-step force driving the overdamped plant. The pulse and step are both generated by a common signal, arising from medium-lead burst neurons in the pons. Their burst signal encodes saccadic eye velocity, while the number of spikes in the burst relates to the saccade amplitude. The step component, which encodes the eye position, is obtained by neural integration of the burst. Several oculomotor neural disorders can be explained by

Neurophysiological studies, including electroencephalography (EEG) and evoked potentials (EPs), are helpful bedside tools for assessing neurologic function and helping with prediction of long-term neurodevelopmental outcomes following brain injury in preterm and term newborns. In this chapter, we describe the use of electroencephalography, including both amplitude-integrated EEG and continuous video EEG, and EPs, including visual, somatosensory, and brainstem auditory EPs, in the neonatal period. We review the current literature on the utility of these neurophysiological studies in the predict

Parkinson's disease (PD) is predominantly idiopathic in origin, and a large body of evidence indicates that gastrointestinal (GI) dysfunctions are a significant comorbid clinical feature; these dysfunctions include dysphagia, nausea, delayed gastric emptying, and severe constipation, all of which occur commonly before the onset of the well-known motor symptoms of PD. Based on a distinct distribution pattern of Lewy bodies (LB) in the enteric nervous system (ENS) and in the preganglionic neurons of the dorsal motor nucleus of the vagus (DMV), and together with the early onset of GI symptoms, it

Reflex syncope is responsible for 1-6% of hospital admissions and the economic burden of syncope is huge. A considerable part of these high costs is still spent on tests that are not indicated. Till now few neurologists have taken an interest in syncope and tilt table testing (TTT). However, reflex syncope and epilepsy are often in each other's differential diagnosis and require a similar emphasis on history taking and deductive reasoning. A TTT can be helpful for diagnosis and treatment. The pathophysiological rationale behind the TTT is the fact that it uses gravity to provoke a downwards sh

Event-related potentials (ERPs) are very small voltages recorded from the scalp which originate in the brain structures in response to specific events or stimuli. They appear as a series of peaks and troughs interspersed in the Electroencephalogram (EEG) waves. The exact neural origins and neuropsychological meaning of the P300 are imprecisely known, even though appreciable progress has been made in the last 25 years. In this review, we will focus on the possible neural generators of this potential. Given the attention and memory operations associated with P300 generation, the first human stud

Smooth pursuit impairment is recognized clinically by the presence of saccadic tracking of a small object and quantified by reduction in pursuit gain, the ratio of smooth eye movement velocity to the velocity of a foveal target. Correlation of the site of brain lesions, identified by imaging or neuropathological examination, with defective smooth pursuit determines brain structures that are necessary for smooth pursuit. Paretic, low gain, pursuit occurs toward the side of lesions at the junction of the parietal, occipital and temporal lobes (area V5), the frontal eye field and their subcortica

The nervous system structures involved in the control of the lower urinary tract (LUT) are usually divided using a neuroanatomical classification system into suprapontine, pontine, spinal and sacral. In all patients with LUT symptoms, after exclusion of local causes, a nervous system disorder needs to be considered. For the diagnosis of neurogenic LUT disorders, in addition to clinical assessment, neurophysiologic testing might be useful. Imaging and other laboratory studies (e.g., cystometry) often provide relevant additional information. Neurophysiologic tests are more useful in patients wit

Vaginal sexual arousal is a vasocongestive and neuromuscular event controlled by facilitatory parasympathetic and inhibitory sympathetic inputs. Autonomic preganglionic parasympathetic and inhibitory sympathetic fibers to the vagina and clitoris originate in the spinal cord in the sacral parasympathetic nucleus at the sacral level and in the dorsal gray commissure and the intermediolateral cell column at the thoracolumbar level, respectively. Parasympathetic fibers are conveyed by the pelvic nerve, and sympathetic fibers are conveyed by the hypogastric nerve and the paravertebral sympathetic c

We present serial clinical, radiologic, and neurophysiologic findings of a patient with late-infantile metachromatic leukodystrophy who was first admitted at 30 months of age because of gait disturbance. The neurologic findings were consistent with mild spastic diplegia (occasionally with toe walking). Magnetic resonance imaging disclosed diffuse high intensity in the cerebral white matter on T2-weighted images. Nerve conduction velocity studies and evoked-potential studies were markedly abnormal. Assay of arylsulfatase A activity in leukocyte culture disclosed a marked deficiency of the enzym

Data on the character of evoked central responses of the soleus and the anterior tibial muscles, as well as the minor muscles of the foot sole and patients with the so-called discogenic lumbar radiculitis are presented. In addition, results of treating this pathology by the method of anti-algetic supracutaneous electric stimulation are described. On the basis of comparing all the data it is concluded that of importance in the neurophysiological mechanism of the pain in lumbar osteochondrosis are not only hyperimpulsation alone the thin pain-conducting fibres, but also insufficient conduction o

IMPORTANCE: Ketamine has well-known dissociative, analgesic, and antidepressant properties, but it is unknown whether the neurophysiologic effects that are associated with these properties can be modulated separately from one another. Considering that specific cortical oscillations have been associated with specific therapeutic effects, modulating selective aspects of ketamine neurophysiology could inform efforts to develop more targeted therapies.
OBJECTIVE: To determine whether the neurophysiologic signatures of ketamine are associated with removal of conscious awareness using general anest

Pelvic neurophysiology testing uniquely provides an evaluation of the sacral S2, 3 and 4 segmental somatic innervation. Since the tests are involved and some are invasive, a clear clinical question is essential in order to tailor the tests appropriately. Results should always be interpreted in conjunction with the clinical presentation and other investigation findings. The tests provide objective quantitative evidence for nerve injury and there are specific indications where these tests become of paramount importance such as the evaluation of suspected cauda equina syndrome, suspected MSA, chr

Artificial intelligence (AI) has the potential to transform clinical neurophysiology by enabling the automated analysis of complex physiological signals and large amounts of data. While traditional clinical interpretation relies heavily on expert visual inspection, which is vulnerable to subjectivity and inter-rater variability, AI models excel at identifying patterns in complex time series. This chapter provides an overview of current and emerging clinical applications of AI across various modalities in clinical neurophysiology, including EEG, EMG, NCS, and ultrasound. In EEG, AI demonstrates

The altered activity generated by corneal neuronal injury can result in morphological and physiological changes in the architecture of synaptic connections in the nervous system. These changes can alter the sensitivity of neurons (both second-order and higher-order projection) projecting pain signals. A complex process involving different cell types, molecules, nerves, dendritic cells, neurokines, neuropeptides, and axon guidance molecules causes a high level of sensory rearrangement, which is germane to all the phases in the pathomechanism of corneal neuropathic pain. This chapter discusses the neurophysiology and function of subcortical circuits and cortical areas involved in saccade generation. While cells within the different nuclei of the brainstem reticular formation shape the temporal details of ipsiversive horizontal and vertical/cyclotorsional saccade components, the cerebellar flocculus, vermis and fastigial nucleus are thought to modulate these saccadic waveforms. Burst neurons in the deep layers of the superior colliculus encode the saccade vector in the contralateral field by a localized population in a motor-error map. The complexity of the sac

Many structural and functional tests are used to explore the nature of neurodevelopmental and neurodegenerative diseases. Cognitive involvement has become more and more remarkable in many neurological and psychiatric diseases. This condition evoked a paradigm shift, and today disorders are addressed from a neuroscientific perspective, including silent symptoms. The spatial resolution of structural studies is lacking and is combined with the unique temporal resolution of EEG methods. In our current clinical practice, EEG does not have definitive diagnostic value in psychiatric disorders, but it

Oculopalatal tremor (OPT) is an acquired pathology characterized by continuous and rhythmical soft palatal movements combined with pendular nystagmus. Aside from vascular lesions, oncological masses affecting the dentatorubro-olivary pathway can impair brainstem and/or cerebellar pathways, manifesting as dyssynchronous movement. In this review, we delve into the neurophysiology of OPT along with oncological causes and treatment options based on the most recent clinical trial data. This literature review includes medication treatment data from clinical trials enrolling individuals with features
