The contribution made by Schistosoma infection to non-traumatic disorders of the spinal cord in Malawi.

In Malawi, although schistosomal myelopathy has been reported in visitors from overseas who have swum in Lake Malawi, the incidence of this disorder in local residents has never been investigated. Consecutive patients with non-traumatic disorders of the spinal cord were therefore recruited in a hospital and a rehabilitation centre in Blantyre. Of the 33 patients investigated, 16 were presumed to be cases of schistosomal myelopathy as they had the markers of past or current schistosomiasis and apparently no other conditions that could explain their clinical features. There was microscopical and

Parasite infections, neuroinflammation, and potential contributions of gut microbiota.

Many parasitic diseases (including cerebral malaria, human African trypanosomiasis, cerebral toxoplasmosis, neurocysticercosis and neuroschistosomiasis) feature acute or chronic brain inflammation processes, which are often associated with deregulation of glial cell activity and disruption of the brain blood barrier's intactness. The inflammatory responses of astrocytes and microglia during parasite infection are strongly influenced by a variety of environmental factors. Although it has recently been shown that the gut microbiota influences the physiology and immunomodulation of the central ne

Myeloradiculopathy associated to Schistosoma mansoni.

Neuroschistosomiasis caused by Schistosoma mansoni (Sm) is a rare and severe condition potentially leading to permanent neurological deficit. An 18-year-old Brazilian female was admitted due to a severe conus medullaris and cauda equina syndrome. MRI of thoracic/lumbar spine showed an expanded conus medullaris with patchy gadolinium-enhancement, needle electromyography revealed acute bilateral radiculopathy (L5-S1-S2), cerebrospinal fluid (CSF) showed lymphocytosis and increased proteins and lesion' surgical biopsy documented a lymphocyte infiltrate. Immunodiagnosis with cercariae hullen react

Label-free nanostructured biosensor for Schistosoma mansoni detection in complex biological fluids.

Schistosomiasis is a neglected tropical disease with a worldwide prevalence. Neuroschistosomiasis is the most severe presentation of the disease and affects the central nervous system. In this work, Schistosoma mansoni detection was based on self-assembled layers of 3-mercaptopropyltrimethoxysilane (MPTS) and electrosynthesized gold nanoparticles (AuNPs). The DNA probe was chemisorbed onto AuNPs. The biosensor was characterized by electrochemical impedance spectroscopy (EIS) and cyclic voltammetry (CV). The impedimetric response of the MPTS-AuNPs-DNAprobe system indicates an effective modifica

Spinal cord schistosomiasis: A case report.

BACKGROUND: Schistosomiasis is a common helminthic infection in the tropics and subtropics, particularly in sub-Saharan African countries including Ethiopia. It imposes major health problems in areas where schistosomiasis is endemic. Neuroschistosomiasis is a rare but severe disorder caused by schistosoma and can occur at any time during schistosomal infection. The prognosis depends largely on early diagnosis and treatment. CASE REPORT: A 30 years old male patient presented with bilateral lower extremity weakness and urinary retention (with urinary catheter in situ) of two weeks duration. He h

Schistosomal Myeloradiculopathy - A case report.

The most common neurological impairments related to schistosomiasis involve the lower portions of the medulla and the cauda equina. A 22-year-old woman, with no history, signs, or symptoms of hepatointestinal schistosomiasis, presented with lumbar pain associated with acute paresthesia and paresis of the right lower limb. Spinal schistosomiasis was suspected based on the disease progression and radiological findings, and the diagnosis was confirmed after cerebrospinal fluid analysis. The authors emphasize this pathology as important as a differential diagnosis in similar clinical scenarios, es

Schistosomal myelopathy in childhood: findings of magnetic resonance imaging in 26 patients.

We describe magnetic resonance image findings of 26 children (16 boys; average age, 9.4 years) with schistosomal myelopathy. All children lived in Pernambuco State, Brazil, an area of endemic mansoni schistosomiasis. Imaging abnormalities were identified in 92.3% of the children. The most frequent findings included: (1) enlargement of the spinal cord at the thoracic level, usually below T(8), in 23/24 (96%) patients; (2) hypointense signals in T(1)-weighted imaging; (3) hyperintense signals in T(2) imaging; and (4) heterogeneous enhancement with gadolinium. Although enlargement was evident at

Spinal cord schistosomiasis japonica: a report of 4 cases.

BACKGROUND: Neuroschistosomiasis is a severe presentation of schistosomal infection. Currently however, spinal cord schistosomiasis japonica is clinically rare, and very few cases are reported. METHODS: The purpose of this study is to retrospectively analyze the diagnosis and treatment of 4 patients who presented with a lower cord syndrome of acute progression characterized by motor, sensory, and autonomic dysfunctions. The patients were examined by MRI, and the biochemical and immunologic changes of blood and CSF of the patients were also analyzed before surgery. Treatments including surgica

Spinal schistosomiasis.

A 23 year old female presented to Townsville General Hospital seven months after a positive urine test for Schistosomiasis with conus modularize signs. MRI confirmed a conus medullaris enhancing lesion. Serology was positive for Schistosoma IgG : IgM, 3.4 : 1.8. Urine screening for ova, stool sample, rectal biopsy and diagnostic investigations were negative. She was treated empirically for Schistosoma without biopsy with marked resolution of her symptoms and signs. She remains neurologically well one year after presentation. Diagnostic Challenges in Cerebral Schistosomiasis: A Case of Schistosoma japonicum Misinterpretation.

BACKGROUND Schistosomiasis is a zoonotic parasitic disease affecting over 240 million people in 77 countries. Cerebral schistosomiasis (CSM) occurs when parasitic eggs form granulomas within the central nervous system. This condition, although generally considered rare, is one of the most severe and under-recognized clinical manifestations. The scarcity of systematic studies on its clinical characteristics often leads to CSM being misdiagnosed as other brain lesions. A definitive diagnosis often requires invasive histopathological examination after craniotomy, underscoring the diagnostic chall

Temporal evolution of neuropathological changes in the brain of Schistosoma mansoni-infected mice: A histopathological study.

Cerebral schistosomiasis is a severe complication of chronic Schistosoma mansoni infection, yet the temporal relationship between parasitic burden and central nervous system pathology remains incompletely understood. This study investigated the temporal dynamics of worm burden and corresponding histopathological changes in experimental murine CNS schistosomiasis from 7 to 23 weeks post-infection. Adult worm counts (male, female, copula, and total) were determined at nine time points (weeks 7-23 post-infection) using hepatic portal perfusion. Histopathological evaluation of brain sections was p

Delayed presentation of cerebral schistosomiasis presenting as a tumor-like brain lesion.

Schistosomiasis is the second most common parasitic infection worldwide. North America is a nonendemic area. However, there are occasional case reports among travelers and immigrants from endemic regions. We describe a case of a 55-year-old Canadian woman who presented with first episode of seizure. Her magnetic resonance imaging scan revealed a mass-like lesion involving the left anterior temporal lobe. The lesion showed T1 hypo- and T2 hyperintense with perilesional brain edema. On post-gadolinium-enhanced T1-weighted sequence, the lesion showed multiple small nodular and linear enhancements

Schistosomiasis mansoni presenting as a cerebellar tumor: case report.

The Manson's schistosomiasis tumoral form rarely affects the brain. There are only 12 cases prior related with a mean age of 25 years and a male predominance. We describe a 16-year-old Brazilian Northeastern boy with a cerebellar mass lesion. The radiological aspect was considered compatible with glioma and a gross total resection was performed. Microscopic examination disclosed intraparenchymal granulomas surrounding Schistosoma mansoni eggs. The case is compared with the literature findings and some peculiar aspects of this trematode infection are reviewed.

[Schistosomiasis of the spinal cord. Case report].

The authors report a case of a 34 year-old woman who, in the last four years, had travelled to Africa frequently, and developed a progressive clinical (about three months) dysfunction of the conus medullaris mainly consisting in sensitive disturbances. The MRI showed an intramedullary spinal cord lesion, and specific laboratory tests were negative. A biopsy for histologic diagnosis, showed a granulomatous tissue with eggs of S. Hematobium. After this diagnosis she was treated with praziquantel. We emphasize the relative frequency of the spinal cord location of schistosomiasis in endemic countr
