Clinical outcomes of patient mobility in a neuroscience intensive care unit.

BACKGROUND: Patients treated in a neuroscience intensive care unit (NICU) are often viewed as too sick to tolerate physical activity. In this study, mobility status in NICU was assessed, and factors and outcomes associated with mobility were examined. METHODS: Using a prospective design, daily mobility status, medical history, demographics, Acute Physiology and Chronic Health Evaluation (APACHE) III score, and clinical outcomes were collected by medical records and database review. Depression, anxiety, and hostility were assessed before NICU discharge. Analyses included comparative statistics

Assessing Postvoid Residual to Identify Risk for Urinary Complications Post Stroke.

BACKGROUND: After a stroke, urinary dysfunction and urinary tract infection are estimated to be approximately 20%. Increased postvoid residual (PVR) is a risk factor for urinary dysfunction and urinary tract infections. These complications can negatively impact recovery weeks after the initial hospitalization. Early identification of risks and patient education are important. PROJECT DESCRIPTION: This project was initiated for those patients admitted for a diagnosis of stroke on a 55-bed neuroscience unit in an acute care hospital. Patients were scanned after the initial void post stroke. Nur

Day-to-Day Change in Pulsatility Index Describes Anterior Cerebral Circulation Disturbance and Functional Outcomes in Pediatric Traumatic Brain Injury.

BACKGROUND: Traumatic brain injury (TBI) is a significant source of morbidity and mortality in children. Transcranial Doppler (TCD) ultrasound measures the cerebral arterial circulation allowing for the calculation of pulsatility indices (PIs) that provide an assessment of cerebral blood flow alterations. However, the use of PI in children with TBI is poorly understood and may be an important measure for the nursing care of children. OBJECTIVES: The purpose of this article is to define day-to-day PI change and to describe its relationship to injury characteristics and functional outcomes in c

Infrastructural Limitations in Establishing Neurosurgical Specialty Services in Liberia.

Introduction Liberia recently employed the first neurosurgeon in the country's history. In a country with a population of 4.7 million people and staggering rates of cranial and spine trauma, as well as hydrocephalus and neural tube defects, neurosurgery is considered a luxury. Our study documents the experience of a team of neurosurgeons, critical care nurses, scrub technicians, nurses, and biomedical engineers who carried out a series of neurosurgical clinics and complex brain and spine surgeries in Liberia. Specifically, we aim to highlight some of the larger obstacles, beyond staff and equi

Neuroethics with regard to treatment limiting and withdrawal of nutrition and hydration in long lasting irreversible full state apallic syndrome and minimal conscious state.

INTRODUCTION: Epidemiology in Europe shows constantly increasing figures for the Apallic Syndrome (AS)/Vegetative State (VS) as a consequence of advanced rescue, emergency services, intensive care treatment after acute brain damage, and high standard activating home nursing for completely dependent end stage cases secondary to progressive neurological disease. Management of patients in irreversible apallic syndrome has been the subject of sustained scientific and moral-legal debate over the last decade. METHODS: Neuroethics coming more and more into consideration when neurological societies a

Validation of a new coma scale: The FOUR score.

The Glasgow Coma Scale (GCS) has been widely adopted. Failure to assess the verbal score in intubated patients and the inability to test brainstem reflexes are shortcomings. We devised a new coma score, the FOUR (Full Outline of UnResponsiveness) score. It consists of four components (eye, motor, brainstem, and respiration), and each component has a maximal score of 4. We prospectively studied the FOUR score in 120 intensive care unit patients and compared it with the GCS score using neuroscience nurses, neurology residents, and neurointensivists. We found that the interrater reliability was e

Early detection of oropharyngeal dysphagia in rehabilitation and long-term care sectors: the EDOR-Care cross-sectional study protocol.

INTRODUCTION: Oropharyngeal dysphagia (OD), or difficulty swallowing, is a common yet frequently underdiagnosed condition that can lead to serious complications, reduced quality of life and increased healthcare costs. Early identification of at-risk patients is therefore essential. Although screening tools such as the Volume-Viscosity Swallow Test (V-VST) are widely used, they remain time-consuming, require trained staff and are often validated only in specific populations (eg, patients who had a post-stroke). This study aims first to establish the prevalence of OD risk in rehabilitation and l

The efficacy of 5-HT3 receptor antagonists for the prevention of postoperative vomiting following craniotomy: two studies in children and young adults.

The purpose of this meta-analysis was to estimate the efficacy of prophylactic administration of 5-HT3 receptor antagonists for postoperative vomiting (POV) in pediatric craniotomy patients at 24 hours. By updating a previously published systematic literature search, we found a recently published pediatric study to combine with the one already identified. The two published randomized placebo-controlled trials were combined for a total of 135 participants aged 2 to 20 (79 treatment and 56 controls). The only study drug was ondansetron. The combined relative risk (RR) of vomiting was not statist

Outcome expectations and physical activity in persons with longstanding multiple sclerosis.

Research suggests that persons with multiple sclerosis (MS) are much less physically active than the general population and that increased physical activity in persons with MS is associated with numerous benefits such as improvements in fatigue, mobility, and quality of life (). Potentially modifiable theory-based determinants of physical activity behavior need to be identified so that researchers may study their effectiveness in randomized clinical trials and clinicians may integrate them into practice to promote physical activity in this population. The purpose of this study was to explore t

Family engagement on neuroscience units with Post-covid visiting policies: A retrospective chart review.

BackgroundFamily engagement is crucial for achieving successful outcomes for both patients and hospitals. It supports safe transitions between care settings, providers, and ultimately, as illness progresses. However, in the hospital setting, family engagement is poorly operationalized. While the existing literature acknowledges its benefits, it does not adequately define the specific domains of family engagement, the roles families play during inpatient care, or whether these factors differ across patient populations.AimsThis research aims to describe family engagement in the hospital setting

Variables explaining functional recovery following motor stroke.

Few well-designed descriptive studies focus exclusively on patients after motor stroke. This study describes a cohort of participants after motor stroke and assesses the extent to which five key variables explain the variation in functional recovery 3 months after stroke. Prospective data were collected (N=100) on age, lesion volume, motor strength, cognition, and poststroke function during the acute care hospital admission. Instruments included magnetic resonance imaging (MRI) to provide a measure of lesion volume, the Mini-Mental State Examination (MMSE) and the Neurobehavioral Cognitive Sta
