The incidence of cytokine release syndrome and neurotoxicity of CD19 chimeric antigen receptor-T cell therapy in the patient with acute lymphoblastic leukemia and lymphoma.

Our objective was to summarize the side effect of chimeric antigen receptor (CAR)-T cell therapy in patients with acute lymphocytic leukemia (ALL) and lymphoma. Two independent reviewers extracted relevant data. A total of 35 hematologic malignancy studies with CD19 CAR-T cell were included (1412 participants). Severe cytokine release syndrome (sCRS) proportion was experienced by 18.5% (95% confidence interval [CI], 0.128-0.259; P = 0.000) of 982 patients with the National Cancer Institute/Lee/common terminology criteria for adverse events grading system. The pooled neurotoxicity proportion wa

Thiamine deficiency, oxidative metabolic pathways and ethanol-induced neurotoxicity: how poor nutrition contributes to the alcoholic syndrome, as Marchiafava-Bignami disease.

Ethanol is an important risk factor for the occurrence of several brain disorders that depend on the amount, period and frequency of its consumption. Chronic use of ethanol often leads to the development of neurodegenerative syndromes, which cause morphological and functional impairments such as foetal alcohol syndrome in newborns exposed to ethanol during pregnancy, Wernicke-Korsakoff Syndrome and, more rarely, Marchiafava-Bignami disease (MBD). MBD is characterized by primary degeneration of the corpus callosum, without inflammation and is associated with oxidative stress and hypovitaminosis

A Computerized Cognitive Test for Immune Effector Cell-Associated Neurotoxicity Syndrome in Chimeric Antigen Receptor T-Cell Therapy Recipients: A Pilot Study.

Cognitive dysfunction is a common manifestation of immune effector cell-associated neurotoxicity syndrome (ICANS) following chimeric antigen receptor (CAR) T-cell therapy. ICANS may be life-threatening and so necessitates rapid recognition and intervention. This study investigated the utility of a computerized test of attention and processing speed (CARTcog) for monitoring ICANS. In this prospective pilot study, 89 CAR-T patients (71% male, mean age at infusion = 67.18 yr) completed 1149 CARTcog tests and 5347 ICE scores during their inpatient stay. Of the 89 participants, 30 (34%) developed I

Immunotherapy-associated dysgraphia as an early neurocognitive manifestation of immune effector cell-associated neurotoxicity syndrome: clinical characteristics, mechanistic insights, and assessment challenges.

BACKGROUND: Immune effector cell therapies, including chimeric antigen receptor T-cell (CAR-T) therapy and T-cell-engaging antibodies, are associated with immune effector cell-associated neurotoxicity syndrome (ICANS), a clinically recognized immune-mediated neurotoxicity. Among its neurological manifestations, writing impairment (dysgraphia), including paligraphia and other forms of writing disturbance, has emerged as a potentially sensitive clinical marker of higher-order cortical dysfunction. OBJECTIVE: This review synthesizes current clinical evidence on ICANS-associated dysgraphia, chara

Management of chimeric antigen receptor T-cell-related toxicity of a patient affected by cytokine release syndrome and immune effector cell-associated neurotoxicity syndrome, followed by an intestinal perforation: a case report.

BACKGROUND: Mantle cell lymphoma is a diverse B-cell lymphoma with varying clinical behaviors. Treating relapsed or refractory mantle cell lymphoma is challenging, with Bruton's tyrosine kinase inhibitors proving effective but not curative. Post-Bruton's tyrosine kinase inhibitor failure, the prognosis remains unfavorable. Brexucabtagene autoleucel, a US Food and Drug and European Medicines Agency-approved anti-CD19 chimeric antigen receptor T-cell therapy, marks a significant breakthrough offering hope in this challenging scenario. Cognitive Recovery Following Immune Effector Cell-Associated Neurotoxicity Syndrome in the First 6 Months after Chimeric Antigen Receptor T-Cell Therapy.

Chimeric antigen receptor T-cell (CAR-T) therapy is associated with immune effector cell-associated neurotoxicity syndrome (ICANS), which can manifest as acute and sub-acute cognitive dysfunction. Few studies have investigated cognitive recovery post-CAR-T therapy. In this prospective longitudinal study, patients treated with CAR-T therapy at The Alfred Hospital, Melbourne, underwent cognitive testing at preinfusion and 3- and 6-months postinfusion. Psychometric tests measured attention, processing speed, visuospatial function, language, memory, and executive function. 197 cognitive evaluation

A Unique Presentation of Immune Effector Cell-Associated Neurotoxicity Syndrome (ICANS) Presenting as Foot Drop in B-cell Acute Lymphoblastic Leukemia: A Case Report.

Central nervous system (CNS) involvement in adult B-cell acute lymphoblastic leukemia (B-ALL) continues to pose treatment challenges, even with advancements in chemotherapy and targeted immunotherapy. Blinatumomab, a bispecific T-cell engager, has been shown to improve outcomes for patients with relapsed or refractory B-ALL; however, it can lead to immune-related neurotoxic effects, including immune effector cell-associated neurotoxicity syndrome (ICANS). We present an unusual case of a 38-year-old man with refractory B-ALL who experienced a parenchymal CNS relapse that manifested as foot drop

Glofitamab-Associated Immune Effector Cell-Associated Neurotoxicity Syndrome (ICANS) Presenting as Serial Seizures and Responding Positively to Antiseizure Drugs and Anakinra: A Case Report.

Immune effector cell-associated neurotoxicity syndrome (ICANS) is a well-known side effect of chimeric antigen receptor (CAR) T-cell therapy but has occasionally been described with immune checkpoint inhibitors as well. Glofitamab-associated ICANS with a bispecific monoclonal antibody has rarely been reported. The patient is a 63-year-old male with a history of mantle cell lymphoma, diagnosed at age 37, and aggressive large-cell B-cell lymphoma, diagnosed at age 50. Despite adequate chemotherapy, immunotherapy, autologous stem cell transplantation, and CAR T-cell therapy, there were several re

Long-Term Quality of Life, Cognitive Function, and Symptom Burden Among Chimeric Antigen Receptor T-Cell Recipients and Associated Cytokine Release Syndrome and Neurotoxicity.

PURPOSE: Immediate side effects after chimeric antigen receptor (CAR) T-cell therapy are well documented and include cytokine release syndrome (CRS) and immune effector-cell-associated neurotoxicity (ICANS). However, long-term patient-reported outcomes are understudied. Using a social determinants of health (SDoH) framework, we described the long-term health-related quality of life (HRQoL), cognitive function, and symptom burden of patients in sustained remission after CAR T-cell therapy and examined the relationship between acute CRS and ICANS and long-term cognitive function and symptom burd

Clinical Presentation, Risk Factors, and Outcomes of Immune Effector Cell-Associated Neurotoxicity Syndrome Following Chimeric Antigen Receptor T Cell Therapy: A Systematic Review.

Chimeric antigen receptor (CAR) T cell therapy is a novel therapy for patients with relapsed or refractory hematologic malignancies. Most CAR T cell therapy recipients will experience clinical features of the immune effector cell-associated neurotoxicity syndrome (ICANS), a potentially life-threatening condition. Here we describe the clinical, biological, and radiological findings associated with ICANS in adults with hematologic malignancies treated with CAR T cell therapy, as well as the acute and long-term outcomes of ICANS. Paradoxical lithium neurotoxicity: a report of five cases and a hypothesis about risk for neurotoxicity.

There have been many reports of probable lithium-induced organic brain syndromes occurring when serum lithium levels are within or close to the therapeutic range. The authors report on five patients who developed clinical syndromes suggestive of severe neurotoxicity during lithium treatment. In all cases lithium levels were between .75 and 1.7 mEq/liter. The patients who developed neurotoxicity had markedly higher global ratings of psychotic symptomatology and anxiety in the pretoxic period than did patients who never deveoped neurotoxicity. When the acute manic state is characterized by marke

Predictors and Outcomes of Immune Effector Cell Associated Neurotoxicity Syndrome in Patients Receiving Chimeric Antigen Receptor T Cell Therapy for Aggressive B-Cell Non-Hodgkin Lymphoma.

Chimeric antigen receptor (CAR) T-cell therapy has revolutionized the treatment for patients with aggressive relapsed or refractory B-cell non-Hodgkin lymphoma (B-NHL). One notable complication is immune effector cell-associated neurotoxicity syndrome (ICANS) which can be life-threatening. Treatment includes supportive care along with corticosteroids, often for protracted time periods. While ICANS presentation can be variable, it was previously unknown whether patients experiencing ICANS and treated with steroids had reduced CAR-T cell activity and worse outcomes. In addition, the optimal pred

Prolonged Neurologic Symptoms Following Immune Effector Cell-Associated Neurotoxicity Syndrome in Patients With Large B-cell Lymphoma Treated With Chimeric Antigen Receptor-Modified T Cell Therapy.

While immune effector cell-associated neurotoxicity syndrome (ICANS) is a well-defined adverse effect associated with chimeric antigen receptor-modified T cell (CAR-T) therapy, some patients develop prolonged neurologic symptoms. Few studies have examined characteristics and outcomes of patients who develop such symptoms. The objective of this study was to provide an analysis of patients who developed ICANS in a single-center cohort of patients with large B-cell lymphoma (LBCL) who received commercial CAR-T and compare characteristics and outcomes between patients with vs. without subsequent p

Multifocal demyelinating leukoencephalopathy and oligodendroglial lineage cell loss with immune effector cell-associated neurotoxicity syndrome (ICANS) following CD19 CAR T-cell therapy for mantle cell lymphoma.

Immune effector cell-associated neurotoxicity syndrome (ICANS) is a prevalent condition seen after treatment with chimeric antigen receptor T-cell (CAR T) therapy and other cancer cell therapies. The underlying pathophysiology and neuropathology of the clinical syndrome are incompletely understood due to the limited availability of brain tissue evaluation from patient cases, and a lack of high-fidelity preclinical animal models for translational research. Here, we present the cellular and tissue neuropathologic analysis of a patient who experienced grade 4 ICANS after treatment with anti-CD19
