Effect of cold air cooling on the incidence of postinflammatory hyperpigmentation after Q-switched Nd:YAG laser treatment of acquired bilateral nevus of Ota like macules.

OBJECTIVE: To investigate the effect of cold air cooling on the incidence of postinflammatory hyperpigmentation (PIH) after laser treatment in Asian patients. DESIGN: Randomized, controlled, split-face study. SETTING: Skin laser center of a university hospital. PATIENTS: Twenty-three Thai women with acquired bilateral nevus of Ota-like macules. INTERVENTIONS: Patients were treated using a 1064-nm Q-switched Nd:YAG laser at an average fluence of 7.0 J/cm(2) using a 3-mm spot size. The same laser fluence was used on both sides of the face in individual patients. One randomly selected face si

Model-guided non-whitening endpoint for 755-nm picosecond laser treatment of acquired bilateral nevus of Ota-like macules (ABNOM) in pigmented skin: A randomized split-face trial.

BACKGROUND: Post-inflammatory hyperpigmentation (PIH) is a major concern in laser treatment of dermal melanocytosis in pigmented skin, particularly when immediate whitening is used as the therapeutic endpoint. OBJECTIVE: To compare a computational model-guided non-whitening endpoint with the conventional whitening endpoint for treating acquired bilateral nevus of Ota-like macules (ABNOM) using a 755-nm picosecond alexandrite laser. METHODS: In this prospective, randomized, evaluator-blinded, split-face trial, 30 Asian patients (Fitzpatrick skin types III-V) underwent 3 sessions at 6-month in

Effectiveness of an epidermal growth factor-containing cream on postinflammatory hyperpigmentation after 1064-nm Q-switched neodymium-doped yttrium aluminum garnet laser treatment of acquired bilateral nevus of Ota-like macules (Hori's nevus) in Asians: A split-face, double-blinded, randomized controlled study.

BACKGROUND: Epidermal growth factor (EGF) may promote wound healing and decrease laser-induced postinflammatory hyperpigmentation (PIH). OBJECTIVES: To evaluate the effectiveness of an EGF-containing cream on PIH, post-laser erythema, and transepidermal water loss (TEWL) after 1,064-nm Q-Switched Nd: YAG laser treatment of Hori's nevus. METHODS: This is a split-face, double-blinded, randomized, controlled study conducted in 30 subjects with bilateral Hori's nevus. After laser treatment, participants were randomized to apply EGF cream on one facial side and placebo on the other side for 8 wee

An in vivo trial comparing the clinical efficacy and complications of Q-switched 755 nm alexandrite and Q-switched 1064 nm Nd:YAG lasers in the treatment of nevus of Ota.

BACKGROUND: Q-switched 755 nm alexandrite (QS alex) and Q-switched 1064 nm Nd:YAG lasers are effective in the treatment of nevus of Ota. Our previous in vivo study indicated that patients better tolerate QS alex than QS 1064 nm Nd:YAG laser. However, in terms of clinical efficacy and long-term complications, the study did not indicate which laser is superior. Although both machines may appear to be similar in effectiveness, the low number of treatment sessions may contribute to this apparent lack of difference. OBJECTIVE: The aim of this study was to compare the clinical efficacy and complica

The efficacy and adverse reactions of 755 nm picosecond alexandrite laser on the treatment of nevus of Ota at different endpoints: A split-lesion, single-blinded, randomized controlled comparison study.

BACKGROUND: The picosecond alexandrite laser (PSAL) is a first-line treatment for nevus of Ota. However, low-fluence PSAL (LF-PSAL) that does not produce immediate whitening as treatment endpoint, remains unexplored. OBJECTIVE: To compare the efficacy and safety of LF-PSAL versus conventional high fluence PSAL (HF-PSAL) for treating nevus of Ota. METHODS: Each lesion of 64 participants (38 aged <5 years, 26 aged >5 years) with nevus of Ota was randomly split and assigned to receive either HF-PSAL or LF-PSAL. Each section underwent 3 treatment sessions at 6-month interval. Efficacy was assess

A split-face, single-blinded, randomized controlled comparison of alexandrite 755-nm picosecond laser versus alexandrite 755-nm nanosecond laser in the treatment of acquired bilateral nevus of Ota-like macules.

BACKGROUND: Q-switched alexandrite lasers (QSALs) have been used for the treatment of acquired bilateral nevus of Ota-like macules (ABNOMs). Currently, picosecond alexandrite laser (PSAL) pulses have become available for pigmentary disorders. However, no studies have compared PSAL and QSAL in the treatment of ABNOM. OBJECTIVE: We sought to compare the efficacy and safety of PSAL and QSAL in the treatment of ABNOM. METHODS: Each patient (n = 30) received 3 treatments at 6-month intervals. Matching areas were delimitated on the face of each patient (left/right comparison); 1 side was treated w

Association Between Acquired Bilateral Nevus of Ota-Like Macules Patients' Clinical Characteristics and Melasma Induction/Exacerbation After Q-Switched Laser: A Decadal Comparative Study of 2675 Chinese Women in Two Different Periods.

BACKGROUND: Post-laser melasma induction/exacerbation remains a critical challenge in treating acquired bilateral nevus of Ota-like macules (ABNOMs). While laser parameters are well-studied, patient-specific risk factors across evolving therapeutic eras require elucidation. METHODS: This retrospective cohort analyzed 2675 ABNOMs patients treated with Q-switched lasers in two eras: C1 (2010-2013; 1064 nm-only, n = 1069) and C2 (2020-2023; multi-wavelength, n = 1606). Risk factors were assessed through multivariate logistic regression, incorporating laser wavelength stratification. RESULTS: Er

Plaque-type blue nevus of the oral cavity.

BACKGROUND: The blue nevus of the oral cavity is a rare lesion with important differential diagnoses. The plaque-type blue nevus is an uncommon variant of the blue nevus. Because of its particular clinical appearance, it can easily be confused with satellite metastases from malignant melanoma. The diagnosis usually requires a biopsy. OBJECTIVES: To describe the clinical and histological features of a plaque-type blue nevus of the buccal mucosa in a 20-year-old white woman, to review all intraoral blue nevi and all plaque-type blue nevi reported in the literature so far and to compare the crit

In vivo reflectance confocal microscopy for evaluating common facial hyperpigmentation.

BACKGROUND: In clinical settings, atypical facial hyperpigmentation such as nevus of Ota, acquired bilateral nevus of Ota-like macules (ABNOM), melasma, and café-au-lait spots are often missed and misdiagnosed. Summarizing in vivo reflectance confocal microscopy (RCM) features of the hyperpigmentation is helpful in the diagnosis of ambiguous lesions. METHODS: We recruited 196 patients referred for unequivocal facial hyperpigmentation, including 55 patients with nevus of Ota, 45 patients with ABNOM, 62 patients with melasma, and 34 patients with café-au-lait spots. The RCM images were evaluate

Malignant melanoma transformation within a nevus of Ito.

The mongolian spot, nevus of Ota, and nevus of Ito are the most common morphologic forms of the dermal melanocytoses, a group of benign pigmented lesions histologically characterized by the presence of melanocytes within the dermis. Nevus of Ito is clinically distinct, presenting with unilateral, bluish gray, patchy discolorations in the skin within the distributions of the posterior supraclavicular and lateral cutaneous brachial nerves. Although all dermal melanocytoses are generally considered benign, rare cases of malignant transformation associated with nevus of Ota have been described. On

Treatment of 153 Japanese patients with Q-switched alexandrite laser.

We have recently used Q-switched alexandrite laser for the treatment of various kinds of pigmented skin lesions. We retrospectively compared therapeutic outcomes of 153 Japanese patients who consulted our department. This approach was not very efficient for nevus spilus/café-au-lait spots, which seemed laser-resistant, especially when the pigmentation had appeared after 1 year of age, was treated after 5 years of age, was located on the face, was oval with a smooth border, and the patient was male. This approach was equally effective for senile lentigo, nevus of Ota, and Mongolian spots, but l

[Combined naevus flammeus and naevus fuscocoeruleus: phacomatosis pigmentovascularis type IIa].

The association of nevus flammeus with mongolian spot, nevus fuscoceruleus, nevus spilus and, with variable frequency, with nevus anemicus has been termed phacomatosis pigmentovascularis, a genodermatosis first described by Ota and co-workers. Four different combinations have been specified. Most cases are reported from the Japanese literature. Phacomatosis pigmentovascularis may constitute an exclusively cutaneous disorder, but overlapping with other syndromes like Klippel-Trenaunay syndrome or Sturge-Weber syndrome is also possible. We report a 30-year-old woman with nevus flammeus on the ba

[Laser treatment of pigmentation disorders].

Pigmentation disorders are among the most common cutaneous changes and occur in up to 60% of all people. They can be genetic or acquired, functionally or cosmetically disturbing, or the expression of a medical disorder. The best treatment options for hyperpigmentation are usually lasers and intense pulsed light (IPL). However, they can also worsen some conditions and are occasionally associated with side effects. A correct diagnosis is therefore essential, taking into account the skin type, the type of pigmentation and how deep the pigmentation is located in the skin. The most common indicatio

Congenital Dermal Melanocytosis on the Foot: A Case Report and Review of the Literature.

Dermal melanocytosis is a common pigmented skin disease, characterized by an increased number of ectopic melanocytes in the dermis. Rare variants of dermal melanocytosis that do not belong to these four typical groups-nevus of Ota, nevus of Ito, blue nevus, and Mongolian spots-are called dermal melanocyte hamartoma, or congenital dermal melanocytosis (CDM) as it mostly appears from birth. We report a case of CDM on the foot of a young woman with a literature review of previously reported cases of CDM.

Clinically distinct form of acquired dermal melanocytosis with review of published work.

Dermal melanocytosis is most commonly found in the skin of Asians and other darkly pigmented people. It is histologically characterized by the presence of dermal melanocytes, with or without presence of dermal melanophages. Mongolian spot, nevus of Ito, nevus of Ota, nevus of Hori, and blue nevus are most common and represent distinct types of dermal melanocytosis. Other clinical patterns of acquired dermal melanocytosis have also been described. Herein, we report a unique case of acquired dermal melanocytosis diffusely affecting the entire back of a 50-year old African-American male and also
